Licensing Program Analyst (LPA) Christian Gutierrez conducted a subsequent complain visit in regard to the allegations listed above. LPA met with Caregiver Sonia Lopez who assisted with today’s visit. Administrator Lalaine Campbell and Licensee Charles Campbell were notified by telephone. The investigation consisted of the following: During the initial visit conducted on 12/31/2024, LPA Seo Jeon conducted a tour of the interior/exterior areas of the facility, conducted a review of records, obtained, and requested copies of pertinent documentation. LPA interviewed residents and staff. LPA did not find any health and safety concerns. The Administrator was advised that at the time of visit additional time is needed and that follow-up visits and or telephone calls are necessary before reaching investigation findings. During today’s visit LPA Gutierrez reviewed four (4) residents files, and inspected kitchen refrigerator and pantry for expired food. Licensing Program Analyst (LPA) Yolanda Delgado arrived unannounced to the facility to conclude an investigation into the allegations listed above. LPA met with Charles Campbell and explained the purpose of the visit. During the course of the investigation, records were reviewed, and interviews were conducted with facility staff members, residents, and witnesses. On October 19, 2023, Community Care Licensing received a complaint alleging that resident is being financially abused while in care, resident is being neglected while in care, resident is being isolated while in care, resident’ personal rights are being violated while in care, resident was left without oxygen for 30 minutes while in care. Licensing Program Analyst (LPA) Rayshaun Nickolas visited the facility unannounced to deliver the finding on the above allegation. LPA met with Licensee Lalaine Campbell and explained the purpose of the visit. This allegation was investigated by department staff. The allegation alleged that resident #1 (R1) has lived in the facility for five (5) years. The allegation alleged that on December 30, 2020, R1 was sent to the hospital, and the facility refused to take R1 back. The allegation alleged that the reporting party (RP) did not receive an eviction notice for R1. Department staff interview with the Licensee revealed that R1 does not live at this facility but at another facility that the Licensee owns. Department staff file review confirmed that R1 did not reside at this facility but at another facility that the Licensee owns. This agency has investigated the complaint allegations. Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced visit to deliver the findings of the above allegation. LPA met with Caregiver Elsie Viray and toured the facility. The investigation included interviews with staff, medical personnel, Resident One (R1), and a review of facility records and medical documentation. Licensee Charles Campbell arrived inside the facility at the time of visit. Regarding the allegation, “Resident sustained multiple pressure injuries while in care.” It was alleged that R1 was admitted to the facility on 10/1/2021 with a tiny open sore (pressure wound) but it was healing. On, or about 11/15/2021, R1 had open sores (pressure wound) on the buttocks, ears, ankle, shin and legs. Through information obtained via confidential interviews and document review, R1 was admitted to the facility on 10/1/2021 with a wound that required positioning/rolling.
