On March 4, 2026, Licensing Program Analyst (LPA), Mia Lankford arrived at the facility unannounced to conduct the Annual Inspection and met with the House Manager, Imelda Villar. The facility file review was conducted and additional records were requested and reviewed on site. The facility is licensed for six Elderly Adults and is currently operating at a capacity of two adults. LPA Lankford toured the facility along with the House Manager, Imelda Villar and made observations pertaining to the annual visit. LPA inspected the facility inside and outside there were no obstructions or debris to the indoor or outdoor passageways at the time of this visit. Additionally, there were bodies of water on the premises with a locked gate around it. The facility isf a single story home. Physical Plant: LPA Lankford observed the two(2) resident’s bedrooms, and each was furnished with required fixtures as per Title 22. LPA Lankford inspected the facility bathrooms, and the hot water temperature tested within regulations, and logged at 111.4 degrees. The bathrooms were clean, and appliances were operable at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers dated 2/18/26. LPA Lankford observed required postings such as "If you See Something, Say Something,” the "Personal Rights," and the LTCO poster in hallway. The cleaning supplies and sharp items were locked and inaccessible to the residents in care. There were designated storage spaces for the residents’ and staff’s files, and the file were locked in a cabinet. Medications: The medications were locked in the kitchen cabinet. The medication records were maintained and keep with the medications in locked cabinet. There were no documented errors observed on the Medication Administration Record.The LPA observed the pantry to be clean, neat, and organized. CONT...809C Food Service& furniture: The kitchen was clean and free of any seen pest, clutter, or unsightly debris. The non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for the residents in care at the time of this visit. Dishes and utensils were in sufficient supply and stored properly, and sharp items are locked. The facility heating system and other appliances were operable at present time of visit. The House Manager informed the LPA there were safety lights for night throughout the facility, and the LPA observed these as indicated. Care & Supervision/Administration: Currently there are adequate staff present for the care and supervision of the residents in care. There are two staff available at the time of this annual visit. The listed Administrator Derick F Villar possesses a current administrator’s certificate with an expiration date of 10/02/2026. The staff files reviewed have criminal clearance, updated training, along with current First Aid certification. Resident files reviewed possessed the required paperwork as per Regulations at the present. Including current physical and TB test, along with updated IPP’s. Disaster preparedness: LPA Lankford reviewed the facility's emergency and disaster plan as well as disaster training binder. LPA observed the last fire drill met the Department standards and was conducted as required per standards, it was given on 12/5/2025. Infection Control: LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility in laundry room. LPA reviewed the facility's infection control plan and found required infection control measures met the Department requirements. An exit interview was conducted. Licensing Program Analyst (LPA) Seo Jeon conducted an unannounced annual required visit. Upon entry, LPA was greeted by Imelda Villar, House Manager, and informed them of the purpose of the visit. At the time of the visit, there were two (2) staff members and three (3) residents present. Facility Overview: The facility is a one-story home with three (3) bedrooms and three (3) bathrooms, including an attached garage. There are no known firearms on the premises. There is a swimming pool with fence and locked gate which is in compliance with local building codes. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 108°F. Fire extinguisher located at kitchen has current inspection tag. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Continued on LIC809-C..... Record Review and Resident/Staff Files: LPA reviewed files for three (3) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Three resident files were reviewed and contained all required documentation. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPA reviewed medications for three (3) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last fire drill conducted on March 3, 2025, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. On 3/29/2024, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced visit to the facility for a required annual inspection. LPA met with House Manager (HM), Imelda Villar who was informed of the purpose of the visit. LPA toured the facility’s interior and exterior with HM Villar. The facility has a fire clearance for six (6) non-ambulatory elderly residents and an approved hospice waiver for two (2). During today’s visit, there was four (4) residents and two (2) staff present. The facility is made up of three (3) resident bedrooms and two (2) full bathrooms along with a kitchen, living room, dining room and attached garage. During the tour, LPA observed the facility has more than a 2-day supply of perishable food and 7-day supply of nonperishable foods that are stored in a safe and healthful manner. Medications are secured in a kitchen cabinet. Disinfectants and cleaning solutions are secured in a locked cabinet in the laundry room. The outside area provides shaded seating available for resident use. Indoor and outdoor passageways are free of obstruction. The facility has an in-ground pool, which is appropriately gated and secured with a master lock, making it inaccessible for the residents in care. HM Villar tested the smoke alarm/carbon monoxide detectors, and LPA found them to be operational. LPA observed charged fire extinguishers mounted throughout the facility. LPA reviewed random resident files. Files reviewed had signed admission agreements and updated physician’s reports. The facility does not safeguard residents' cash resources. LPA reviewed random staff files. Files reviewed had the Department’s required training records and a valid first aid certification. During today’s visit, LPA did not observe any issues or concerns. An exit interview was conducted, and a copy of this report was reviewed and provided to HM Villar via email due to LPA experiencing technical difficulties.
