Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Seo Jeon conducted an unannounced annual required visit. Upon entry, LPA was greeted by Alegria Serrano, Caregiver, and informed them of the purpose of the visit. At the time of the visit, there were two (2) staff members and four (4) residents present. Bernarda Rocha, Administrator, later joined the annual inspection. Facility Overview: The facility is a one-story home with five (5) bedrooms and five (5) bathrooms, including an attached garage. There are no pools or known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility has infection control plan in file. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 116°F. Fire extinguisher located at kitchen had current inspection tag. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPA reviewed files for four (4) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Four (4) resident files were reviewed and contained all required documentation. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPA reviewed medications for four (4) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last fire drill conducted on 09-18-2025, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Seo Jeon made an unannounced visit to the facility to conduct a Case Management visit regarding a deficiency cited on 10-1-2024. LPA spoke with Bernarda Rocha, Administrator, and explained the purpose of today's visit. LPA conducted a tour of interior and exterior of the facility and determined that there were no health and safety concerns. Fire safety inspection shows bedroom #5 is approved for a bedridden resident. LPA verified Resident #1 (R1) who is a bedridden person is residing at bedroom #5. No deficiencies are being cited and no civil penalties per California Health & Safety Code and Code of Regulations, Title 22, Division 6. Licensing Program Analyst (LPA) Seo Jeon and Andrei Castillo conducted an unannounced annual required visit. Upon entry, LPAs were greeted by Joshua Rocha, licensee and informed them of the purpose of the visit. At the time of the visit, there were two (2) staff members and three (3) residents present. Facility Overview: The facility is a one-story home with (6) bedrooms and (5) bathrooms, including an attached garage. There are no pools or firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department requirements. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Three (3) resident files were reviewed and contained all required documentation. Based on observation and record review, the facility does not have an approved fire clearance for bedridden residents. Two (2) of three (3) residents in care were identified as bedridden. Licensee did not ensure 2 out of 3 residents have current appraisals on file. Citations were issued. Health-Related Services/Incidental Medical Services: All resident medications were securely locked. LPA reviewed medications for four residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last fire drill. All facility exits were clear of obstructions. Two (2) deficiencies were cited during the visit. Type of visit: Prelicensing

On Tuesday, 10/10/2023, Licensing Program Analyst (LPA) Janette Romero conducted an announced visit to conduct a pre-licensing inspection. The facility is undergoing a change of ownership. LPA was informed by Administrator Bernarda Rocha that Applicant Joshua Rocha was unavailable to meet with LPA at the time of LPA's visit. Fire clearance has been granted for (6) non-ambulatory elderly residents. Applicant Rocha's administrator certificate expires on 1/23/2025. LPA conducted of a tour of the facility’s interior and exterior. The facility is made up of five (5) resident bedrooms and four (4) resident bathrooms along with a kitchen, living/family room, and garage. LPA did not observe bodies of water on the premises. The physical plant is in good repair. Indoor and outdoor passageways are free of obstructions. The facility has an outside shaded seating area available for the residents. LPA observed a charged fire extinguisher, operating smoke alarms, carbon monoxide detectors, and a working land line. LPA observed a locked storage area for cleaning solutions, centrally stored medications, and knives/sharp instruments. Resident and staff files are secured in a file cabinet stored near the kitchen. Continued on LIC809-C.. Continued from LIC809. LPA toured the bedrooms. Resident bedrooms had the required bedding, furniture, and functional lighting. Additional linen and towels are available in the hallway cabinet. LPA toured the kitchen. Food was stored in a safe and healthful manner. The facility had a 2-day supply of perishable food and 7-day supply of nonperishable food items. LPA also observed emergency food and water stored in the kitchen. LPA toured the resident bathrooms. Resident bathrooms are equipped with grab bars and non-skid mats in the shower. The hot water temperature in resident bathrooms measured at 106-degrees Fahrenheit. Emergency disaster plans, personal rights, and complaint procedures were posted in living/family room wall. LPA observed two complete first aid kits. Living/family room has a working television and adequate seating in common areas. During today's visit, LPA did not observe any issues or concerns. Administrator Rocha is scheduled to complete the Comp III on Thursday, 10/12/2023, at 9:00 a.m., at the Riverside Regional Office. Final approval of licensure will be granted by the Centralized Application Bureau analyst.
