LPA Mann met with Andrea Perez, Executive Director and explained the purpose of the visit. The allegation that Staff handled resident in a rough manner. Five (5) staff interviewed denied handling a resident in a rough manner, they have not grabbed a resident by their arm and squeezed them badly. Nine (9) residents interviewed denied staff handling them in a rough manner, the staff have not grabbed them by their arm and squeezed them badly. The allegation that Staff did not properly maintain centrally stored medications. Five (5) staff interviewed stated that they do properly maintain centrally stored medications. The five (5) staff denied of any missing medications in the med room. LPA randomly reviewed 4 resident medications to their MAR (Medication Administration Record). LPA introduced herself and disclosed the purpose of the visit to Executive Director Andrea Guiterrez. On March 17, 2026, it was alleged that staff did not ensure resident received timely medical care. According to the allegation received, on February 14, 2026, Resident #1 (R1) choked on their food in the dining room and another resident immediately came to the aid of R1 while staff did not. The Department’s investigation consisted of an unannounced facility visit, records review, and staff, resident, and outside source interviews. The Department’s investigation consisted of an unannounced facility visit, records review, and staff and resident interviews. According to the allegations received, Resident #1 (R1) attempted to ask Staff #1 (S1) for assistance with unloading items from their vehicle. S1 replied to R1 by telling them they needed to hire a private caregiver. Review of R1’s medical assessment dated March 12, 2025, revealed that R1 is independent and does not require assistance with Activities of Daily Living (ADLs). Interviews with staff and residents did not reveal that S1 spoke rudely to R1 or told R1 they needed to hire a private caregiver.
