Allegation investigated: Staff keeps facility doors locked 24 hours of the day.
LPA Parks arrived on Thursday October 9, 2025, to deliver findings for a complaint investigation regarding the above allegation. LPA met with Laurie and explained the purpose of the visit. Throughout interviews, LPA learned that all doors to the outside are alarmed. Residents can leave at any time. The receptionist is on duty from 7am - 7pm every day to assist visitors entering the building. Families were notified of the change via email on August 14, 2025. There is approximately a 15-30 second wait for the door to be opened. After 7pm, care staff are able to monitor the front door via camera. They have the ability to answer and unlock the door remotely. Based on the evidence provided, the preponderance of evidence standards was not met, therefore, the above allegations is found to be UNFOUNDED. Allegation investigated: Staff forced a resident to shower Staff handled resident in a rough manner
Licensing Program Analyst (LPA) Hiratsuka conducted this complaint visit. LPA interviewed staff and witnesses. The above incident did occur where two staff members approached a resident who will refuse showers and staff were caught attempting to force the resident into the shower with one holding the resident by the wrist and one holding the resident by the waist. A third staff member intervened and the other two staff members left the room. Resident did not sustain any injuries. Based on the above the allegation is substantiated. Based on the information obtained for the allegations above, the allegations are SUBSTANTIATED- A finding that the complaint is Substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. LPA Parks arrived on Thursday September 19, 2024, to deliver findings for a complaint investigation regarding the above allegations. LPA met with RCD Vandhana to deliver the findings. Throughout the course of the investigation, LPA interviewed the Administrator, Resident Care Director, AL Coordinator, Maintenance Director, staff and R1’s POA. The result of the investigation is as follows: Allegation: Staff does not assist residents in a timely manner. LPA interviewed R1’s POA who stated that they visit every day or every other day. They stated that they have been very happy with the care R1 has received. They stated that they did not have any concerns regarding care. LPA interviewed staff who stated that R1 is a two person assist. Staff stated that, at times, they must wait for another caregiver to assist because R1 requires a hoyer lift, but that R1’s care needs are being met. LPA Parks arrived on Monday July 8, 2024 to open a complaint investigation regarding the above allegation. LPA discussed the allegation with Administrator Laurie. LPA discussed the allegation with R1. LPA reviewed R1's file and obtained the following copies: care plan, emergency and ID form, Advanced directive, and admission agreement. Additionally, LPA interviewed R1's family and staff regarding the allegation. Based on interviews, LPA determined that R1 was able to vocalize if they wanted to see certain visitors. The facilty would then honor R1's decision. Based on the evidence provided, the preponderance of evidence standards was not met, therefore, the above allegation is found to be UNFOUNDED. An unfounded allegation means that the allegation was false, could not have happened and/or is without a reasonable basis. Exit interview conducted. A copy of this report was left at the facility.
