On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of delivering findings into the investigation of the allegation listed above. LPA was greeted and granted entry by caregiving staff after introducing himself and stating the purpose of the visit. Administrator Eileen Tecson was notified by telephone and presented with the findings remotely. The initial complaint investigation visit was conducted alongside a health and safety check on September 16, 2024. A tour of the physical plant and review of resident records was conducted along with an interview of the facility’s administrator via telephone.
