On June 15, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Deanna Landon was notified via telephone and later arrived to assist with the inspection. LPA observed that Deanna Landon has a valid Administrator certificate which expires on December 27, 2027. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for five. The facility consist of six resident bedrooms, six resident bathrooms, one staff bedroom and one staff bathroom located on the second floor, a living room, a dining room, a kitchen, and an attached two car garage. LPA, accompanied by the AD, conducted a tour of the interior portions of the facility. On today's visit, LPA observed six residents in care and two care giving staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the six resident bedrooms and observed them to be clear of any hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway cabinets. LPA inspected the six resident bathrooms and observed them to be clean. Resident bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 114.4 and 115.5 degrees Fahrenheit. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. LPA observed the staff bathroom to be free of any hazards. CONTINUED ON LIC809-C LPA observed the facility has a two day perishable and a seven day non-perishable food supply in the kitchen. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed two fire extinguishers to be mounted in the facility which were observed to be charged and serviced as of May 5, 2026. LPA tested the wired smoke detectors and individual carbon monoxide detectors which tested operational. LPA observed the facility conducted their most recent emergency disaster drill on March 19, 2026. LPA observed the centrally stored medication to be kept in a locked cabinet located in the kitchen. LPA observed there is also a first aid kit stored in the locked cabinet which has all the required components. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage. LPA observed chemicals and toxins to be stored in the garage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA, accompanied by the AD, conducted a tour of the exterior portions of the facility. LPA observed the exterior to be clear of any obstructions or hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gate of the facility to be self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all six resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed the residents' medication and medication administration records. LPA reviewed four staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. On June 5, 2025 at 8:00am, Licensing Program Analyst (LPA) Eboni Bentley arrived at the facility to conduct an unannounced required 1-Year annual visit using the CARE Inspection Tool. LPA announced self and stated the purpose of the visit to Caregiver (CG) Yuriria Rebollar and was granted entry. Administrator (AD) Deana “Chris” Landon was contacted by telephone, arrived at the facility a short time later, and was present throughout the inspection. Administrator certificate for Deana Landon expires on December 27, 2025. There are five residents on census. Two residents on Hospice and two residents with dementia currently living at the facility. During today’s visit, five residents and six staff are present. LPA obtained copies of pertinent documents for clients and staff, including facility records: residents/staff rosters, Personnel Record (LIC500), resident and staff records. The facility is a two-story home licensed for 6 non-ambulatory residents and has a Hospice waiver for 5. There are 6 resident bedrooms, 1 caregiver bedroom located upstairs, a living room area, a kitchen, a dining room area, and attached two car garage. There are 7 bathrooms, one located inside each bedroom. Around 8:45am, LPA conducted a tour of the facility with AD Landon and the following was observed: Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 110.3 degrees F. and 115.7 degrees F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Continues on LIC 809-C Common areas were clean and clear of hazards, doorways were free of obstructions. Kitchen was inspected and found clean and sanitary. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed knives and sharps locked in a kitchen drawer. LPA also observed toxin substances to be secured and locked and inaccessible to residents in care. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample seating with shade and the exit gate is self-latching and operational. During tour, LPA observed Resident #1(R1), Resident #4 (R4), and Resident #5 (R5) beds all have 1/2 bed rails attached. Licensee stated she did not have orders for the three bed rails and did not know a physician's order was required for 1/2 bed rails. LPA also observed, scissors unlocked/unsecured in Resident #1 (R1), Resident #2 (R2), and Resident #3 (R3) dresser drawers, desk, and shelf. (See LIC809-D attached) Carbon monoxide/smoke detectors in common areas and bedrooms are operational. The facility’s last fire drill was conducted on March 16, 2025. Emergency food and water supply observed in the garage. First aid kit had all the required elements. Two fire extinguishers are fully charged with a service date of May 5, 2025. A working telephone (714-351-7800) remains available, and the facility has a device that can be used for video teleconference purposes. Liability Insurance is effective February 1, 2025, through February 1, 2026. LPA Bentley conducted an audit of five (5) resident files (R1-R5), five (5) staff files (S1-S5), and conducted three (3) staff interviews, and four (4) resident interviews. Residents’ medication was found locked, and a review of the Medication and Medication Administration Record (MAR) was conducted. Based on today’s observations, deficiencies are being cited during this visit as per Title 22 Division 6 Chapter 8 of the California Code of Regulations. Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to Newport Senior Living III. The purpose of today’s visit was to conduct the Annual Required inspection. LPA was allowed entry into the facility and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents. Facility has an approved hospice waiver for 5 residents and the home currently has 6 residents, with 5 on hospice. Administrator Chris Landon arrived during the visit. LPA Lyman along with Administrator Chris Landon toured the facility at 12:45 PM. LPA toured the physical plant, checked food service, and the first aid kit. Facility appears to be clean, safe, and sanitary. The home consists of six resident bedrooms, 6 resident bathrooms, one staff room, living room, dining room, and kitchen. Facility has an apartment connected to facility and LPA observed occupant has criminal record clearance. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. LPA observed six residents with a half bed rails. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 106.5 and 107.4 degrees F in all facility bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the elements including thermometer, tweezers and scissors. LPA observed a locked storage area for cleaning supplies under the kitchen sink. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps locked in a kitchen drawer. Smoke detectors and Carbon Monoxide detectors are hardwired and tested operational during today's visit. Fire extinguisher is fully charged. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample shaded seating for residents. Exit gates are unlocked. LPA observed ample emergency food and water supply. LPA reviewed the emergency disaster plan during the visit. Plan is thorough and complete. Facility provided documentation of last fire drill conducted on 02/26/2024. CONT ON LIC809-C DATED 06/13/2024 Facility provides activities in the form of exercise and games. At 1:30 PM, LPA reviewed six resident files and two staff files. Resident files contained required documents including admission agreements, physician reports and resident appraisals. Staff files reviewed contained required documentation of annual training, medical clearance/ TB, CPR training and criminal record clearance. At 2:00 PM, LPA reviewed medication storage and administration. Medications are stored in a locked cabinet. Medications are being administered per physician order. Based on the observations made during today's visit ,NO violations are being cited.
