Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced inspection visit to initiate complaint investigation and deliver findings into the above allegations. LPA explained the reason for the visit with Executive Director Nancy Rodriguez. During the course of the investigation, LPA toured facility, reviewed resident records, and conducted staff interviews. During investigation LPA requested pertinent documentation such as Resident Medication Administration Record, Needs & service plan, Active medication list, and Discharge paperwork and orders. Regarding the allegation Staff do not ensure resident medications are given as prescribed, during visit LPA reviewed Resident 1’s (R1) records and observed that on 9/25/25 R1 went out of facility to Rehabilitation center for physical therapy, recommended durable medical equipment, speech therapy, occupational therapy and pain management. Allegation investigated: Staff are not delivering meals to the resident timely

Licensing Program Analyst (LPA) Jerome Haley made unannounced visit regarding the complaint allegation above. LPA Haley was greeted, granted entry, and explained the reason for the visit. During the visit LPA Haley toured the facility, interviewed staff, residents, and collected and reviewed relevant documents. Regarding the allegation: Staff are not delivering meals to the resident timely. 11 of 12 individuals interviewed including facility residents and staff were unable to corroborate the allegation above. During an interview with multiple staff members, it was discovered staff took orders for breakfast and lunch a day early so meals could be prepared and served at a reasonable time. Multiple staff members did acknowledge there were some residents who complained they did not receive their food, but after checking a master list of orders and meals delivered to residents, the resident received a meal every time. Regarding the allegation: Staff is not meeting the need of the resident. 12 of 12 individuals interviewed including facility residents and staff were unable to corroborate the allegation above. All 7 residents interviewed denied the allegation. When residents were asked if staff assist them when needed, Resident 5 (R5) said, yes, but I don’t call. We all need assistance here… before adding, staff do a good job. Resident 7 (R7) was asked about receiving assistance from staff, and said, I’m what they call independent. I don’t have one of those (referring to a call button). When Resident 6 (R6) was asked the same question about receiveing assistance from the staff, R6 stated, they always have. Licensing Program Analyst (LPA) Jenifer Tirre made an unannounced visit to deliver findings on the complaint investigation. LPA met with and discussed the purpose of the visit with Executive Director Sheryl McCaskill. The department investigated the above allegations and the investigation consisted of interviews, observations and documentation. It was alleged that facility staff did not provide toilet paper to residents. Based on interviews with staff and residents the investigation revealed that Residents in both Memory Care and Assisted Living received toilet paper as part of their house keeping services. Investigation also revealed that Residents that live in the independent living section are asked to provide their own toiletries if they are specifying for a certain type of toilet paper or soap that facility is not providing. The department investigated the above allegations and the investigation consisted of interviews, observations, and documentation. It was alleged that facility staff did not meet residents showering needs. Based on interviews with staff, all residents have showers inside apartment restrooms and residents that require shower assistance are on showering schedules. Residents shower schedules are individualized based on Admission Agreement which is discussed during service care plan upon admittance to facility. Interviews reveal that showers are agreed upon set days and schedule of morning or night. Per Service Plan showers cost additional if residents want more than twice a week. On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of investigating the allegation listed above. LPA was greeted and granted entry by Patrick Frazier, Executive Director after explaining the purpose of the visit and detailing the allegation. LPA requested, obtained and reviewed the resident records for individuals living in both units mentioned in the initial complaint (Rooms 225 and 228 respectively), as well as the facility census with room assignments, staff roster and pest control records. LPA conducted four (4) staff interviews and interviewed resident R1's Durable Power of Attorney as well as conducted a tour of the affected unit. The investigation into the allegation listed above revealed the following: During the first half of December 2022, caregivers on the night shift noticed the presence of what was alleged to be bed bugs inside unit 225.
