At approximately 8:55AM, Licensing Program Analyst (LPA) Felias, arrived unannounced to deliver findings for a complaint investigation regarding the above allegation and met with Licensee, Silvana Daniel. During the course of the investigation, the Department requested and reviewed documents, conducted interviews, and made observations. The following allegation was investigated, “Staff do not ensure facility is free of bed bugs.” Complaint alleged that Resident 1 (R1) has bed bugs and that it has been an ongoing problem at the facility for at least 6 months. Per complaint, the facility has only treated R1’s room and no other rooms in the facility. Complaint also stated that R1 was going to be evicted because the facility didn’t have issues with bed bugs until R1 moved to the facility. Licensing Program Analyst (LPA) Cuadra arrived unannounced to the facility met with Licensee Silvana Daniel to deliver findings regarding the complaint allegation above. There was an allegation of Staff did not seek medical attention to resident in care. Per Reporting Party, resident (R1) was admitted to the Emergency Department of Marin Health for pain related to wounds. R1 was on Hospice prior to hospitalization. R1’s responsible party have revoked hospice services and brought R1 for treatment disclosing not taking R1 to medical appointments due to R1's limited mobility and been ineligible to receive transportations resources from the Veterans Affair (VA). Also, it was disclosed that R1 has been bedbound since January 2023. Licensing Program Analyst (LPA) Cuadra arrived unannounced to initiate a complaint investigation regarding the allegation listed above and met with Administrator Silvana Daniel. It was alleged that staff was not administering medications to resident (R1). Reporting party concerns were that Administrator is refusing to give R1 their pain medication when R1 is in pain at all times. After reporting party inquired about this situation with Administrator who have allegedly stated their acknowledgement that R1 has not been receiving pain medications because Administrator does not believe in pain medications and R1 doesn't look like they are in pain. Through the process of this investigation the LPA reviewed records and interviewed staff. Licensing Program Analyst Bertozzi arrived unannounced to deliver findings regarding the above-mentioned complaint allegation and met with Licensee, Silvana Daniel. Furniture in disrepair - Complaint alleges that an unstable chair was observed at the dining room table. Per interviews, a visitor sat on the chair and indicated it was wobbly. Interview indicates that staff were told and responded that they would fix the chair later but visitor insisted that the chair was removed from the dining room. Per interview with Licensee, the chair needed to be tightened and it is fine now. LPA observed during visit on 11/10/2022 that all chairs were stable. LPA confirmed through interviews that nobody was hurt. The allegation that Furniture is in disrepair is Substantiated.
