On October 19, 2025, the Community Care Licensing (CCL) Licensing Program Analysts (LPA) Ernand Dabuet conducted a subsequent unannounced complaint visit. Administrator Antonia Dionisio greeted the LPA, who explained that the visit was to investigate the allegations mentioned above. The investigation included interviews, record reviews, and a tour of the facility. Interviews with Staff members #1 (S1) and Resident members #2 to #5 (R2-R5) and Witness #1-Witness #2 (W1-W2) . Administrator Antonia Dionisio greeted the LPA, who explained that the visit was to investigate the allegation mentioned above. On 4/9/2025, at 8:32 AM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to deliver findings for the alleged allegation. LPA identified herself and met Susan Parungao-Caregiver who was informed of the purpose of the visit. The investigation consisted of the following: On 4/3/2025 at 2:45 PM, LPA Allen obtained and reviewed files for Resident 1 (R1), which included face sheet, medication list, appraisal, needs and services plan, physicians report, admissions agreement with personal property valuables list, and staff and client roster. LPA Allen conducted interviews with Staff 1- Staff 2 (S1 - S2), Residents 1- resident 3 (R1 -R3). On 03/03/23 Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced subsequent visit to the facility and was greeted by care staff (S1: Elmer Carlos). LPA spoke to (S1) prior to entering the facility to conduct a risk assessment. (S1) informed LPA that the facility has no COVID cases nor do any of the residents or staff have symptoms. LPA explained the purpose of this visit is to deliver the findings pertaining to the above-mentioned allegation. Carlos contacted administrator (S2: Antonia Dionsio) by telephone who later arrived at the facility. The investigation consisted of the following:LPA Dabuet conducted the 10-day visit on 09/09/22 with Administrator Dionsio.
