Licensing Program Analyst (LPA) Mariana Agban arrived at the facility to conduct an unannounced subsequent complaint investigation based on further information obtained for this complaint. Upon arrival, LPA was greeted by the administrator, and the purpose of the visit was explained. Today's investigation consisted of an interview with the Administrator, and obtaining additional documents. Allegation: Staff administered resident medication without consent It was alleged that staff administered Seroquel to R1 from April 20, 2023, to April 23, 2023, without consent from R1's responsible party. Records review and interviews revealed that the administrator didn't notify R1's responsible party regarding R1's change of condition and the new medication prescription. Administrator did administer Seroquel medication on April 20, 2023, before getting consent from the R1 responsible party. This is an amended report of the prior investigation report delivered on 8/18/23. Due to new information obtained allegations findings were changed from Unsubstantiated to Substantiated. Licensing Program Analyst (LPA) Mariana Agban arrived at the facility to conduct an unannounced subsequent complaint investigation. Upon arrival, LPAs was greeted by the administrator, and the purpose of the visit was explained. Today's investigation consisted of interview with the Administrator, and obtaining additional documents. At 10:18 AM, Licensing Program Analyst (LPA), Mariana Agban,conducted an unannounced complaint visit to investigate the above stated allegation. LPA met with the Administrator and explained the reason for the visit. During course of the investigation, interviews and record review were made. At 10:45 AM, LPA requested resident and staff roster. At approximately 11:00 AM, LPA conducted a physical plant tour, to ensure health and safety of the residents are protected per Title 22 Regulations. At 12:00 PM, LPA requested copies of pertinent information which include, but not limited to Physician’s Report, Admission Agreement, Appraisal Needs and Services Plan, etc., relevant to the investigation. Between 11:00 AM – 12:00 PM, LPA conducted an interview with the Administrator, and one (1) staff member out of three (3).
