Type of visit: Case Management - Annual Continuation

On September 1, 2026, at 9:45 AM, Licensing Program Analyst (LPA) Sulma Lopez arrived unannounced at the facility to conduct an annual required inspection. LPA Lopez met with Administrator (A1) Glenda Molinyawe and explained the purpose of today's visit. The Administrator holds current certificate #7033450740 expires on August 8, 2027. The facility is licensed for (5) non-ambulatory residents and (1) bedridden resident ages 60 and over. The facility has an approved hospice waiver for (2) residents. At 9:55 AM, LPA Lopez reviewed (5) resident and (3) staff files. The staff files contained all the required components and training records were up to date. Resident files contained all the necessary components and LPA observed updated Needs and Services Plans available. LPA reviewed (2) resident medication records. The facility maintains current records on a paper Medication Administration Record (MAR) sheet. The facility's fire extinguishers were serviced annually on November 13, 2025. At 10:30AM, LPA toured the facility with A1. LPA observed the living room and dining areas which were clean, free of odors, and free or hazards. The dining table and living room areas contained enough seating to accommodate the current census. Furnishings were in good repair. The living room contained books and puzzles available for resident use. LPA observed a public telephone available in the living room. The facility temperature was 72 degrees. LPA inspected the facility kitchen which was clean and free of odors. The facility's appliances were in working order. The facility contained at least 2 days of perishable food items and at least one week of non-perishable food supply. Continued on LIC 809-C. LPA observed sharps and toxins were stored in a locked kitchen drawer and a locked kitchen cabinet under the sink. The facility's hot water temperature was 107 degrees. The facility’s toilets, hand washing, and bathing areas were sanitary and operational. All showers had non-slip mats and grad bars for resident safety. The restrooms were maintained clean and free of odors. LPA toured the residents' bedrooms which were clean and free of hazards. The bedrooms furniture was in good repair. LPA observed clean bedding available for each resident. Room windows and window screens were in good condition. The facility backyard contained a shaded seating area available for resident use. The walkways were free of hazards. LPA observed the fence to be in good repair. LPA Lopez requested the following documents to be emailed/ faxed to the Regional Office: LIC 610 Emergency Disaster Plan, LIC 308 Designation of Facility Representative, LIC 500 Personnel Report. As a result of this annual inspection visit, the facility is in compliance with California Code of Regulations, Title 22 and Health and Safety Code, there were no deficiencies cited at this time. On 8/6/2025, Licensing Program Analyst (LPA) Cynthia Tamayo arrived unannounced to conduct a Required - 1 Year visit. LPA met with administrator, Glenda Molinyawe and explained the purpose of the visit. Administrator Lianna Sisayan is out on vacation. Current LIC 308 designated S1 as Designated Facility Responsible person. This facility is licensed to serve six (6) non-ambulatory residents. Hospice approved for two. The current census is 5. There is currently one resident on hospice. LPA toured the facility with Glenda Molinyawe. LPA toured the physical plant including but not limited to the common area, kitchen, dining area, resident bedrooms; resident bathrooms, laundry area, and outside courtyards of the facility to ensure compliance with Title 22 regulations. LPA observed the facility to be free of odor, clean and in good repair. LPA observed that all rooms are equipped with the required furniture and sufficient lighting throughout the facility. LPA observed supplies of nonperishable foods for a minimum of one week and perishable foods for a minimum of two days maintained on the premises. The hot water temperature was observed to be within the required range of 105-120*F. The temperature inside the facility measured at 75*F which was within the required range of 68-85*F. LPA observed the centrally stored medications area to be locked and inaccessible to residents. LPA observed the fire extinguisher(s) and first aid kits were up to date. Smoke and carbon monoxide detector(s) in the facility were in good repair. continued on 809-D LPA observed an activities calendar posted in the dining room bulletin board. LPA observed security cameras in the hallways and common areas, there were no security cameras in private areas such as resident bedrooms or bathrooms. Resident, GaryHart (room#1) has a pet dog that resides in their bedroom. LPA requested resident and staff files for review. LPA reviewed (3) staff files and (5) resident files, including criminal record clearances. A review of staff records indicates that all facility staff or other individuals who require caregiver background checks are fingerprint cleared and associated to the facility. LPA discussed LIC 624 and reporting requirements with S1. The following forms and documents were requested to be submitted within 15 days: Copy of Administrator Certificate, LIC 610 Emergency Disaster Plan and Proof of Current Liability Insurance. Per the California Code of Regulations, Title 22, no deficiencies were cited. Administrator Lianna Sisayan arrived a bit later. Administrator certification expires on 8/8/2025. This facility is licensed to serve six (6) non-ambulatory residents. Hospice approved for two. The current census is 5. LPA toured the facility with Glenda Molinyawe. Allegation investigated: Facility staff handled resident in a rough manner. Facility staff fed a resident food that had dropped on the floor.

On 1/18/24, Licensing Program Analyst (LPA) Tung Truong conducted an unannounced visit at this facility to commence a complaint investigation with the allegations above. LPA met with administrator designee Glenda Molinyawe and explained the purpose of the visit. During today’s visit, LPA conducted interviews and reviewed records. Based on interviews and records review, it was determined that the allegations above were false. LPA interviewed resident (R1), R1 corroborated that she has reported false accusations about staff because she was having a bad time. R1 stated that facility staff were very good to her. As a result of this investigation, LPA finds the allegations above to be UNFOUNDED, meaning that the allegations were false, could not have happened and/or was without a reasonable basis. Allegation investigated: Resident was inappropriately touched by staff in a sexual manner

Licensing Program Analyst (LPA)s Jamie Ivey Canady arrived at the facility unannounced to deliver complaint investigation findings. LPA Ivey Canady explained the purpose of the visit and was met by Glenda Molinyawe. The investigation was conducted by the Department. The investigation consisted of resident medical and file review, in addition to facility incident reports and file review. The Department has determined the following as it relates to the allegations: Resident was inappropriately touched by staff in a sexual manner. Continued on LIC 9099 - C... Unsubstantiated On 11/06/2023 LPA Ivey Canady reviewed Department findings regarding current allegations. The Department findings are based on staff, resident and witness interviews as well as facility and medical file documents. Based on facility file documents dated 9/12/2023, R1 was admitted to the hospital on 09/08/2023. According to witness statements, medical documentation and medical file review, R1 has a history of dementia. On 9/12/2023, the Department received a report of possible abuse to R1 from the facility. On 9/08/2023 the facility contacted the Sacramento County Sheriffs Office and made a report of possible abuse to R1. On 9/14/2023 RP contacted the Department to recant current facility allegations. According to facility physician’s report dated 7/25/2023 R1 has a diagnosis of dementia and is listed as nonambulatory with both a physical and mental condition. According to witness statements, medical file documents and facility staff interviews, it was learned that R1’s family history and medical diagnosis include bi-polar and schizophrenia diagnosis. Based on facility staff interviews, and facility file review, R1’s incontinence care has been performed by a skilled and trained staff with no history of abuse and no abuse has been imparted on R1. Based on interviews, file reviews to include physician reports and behavior intervention plans, the Department has deemed the allegation Resident was inappropriately touched by staff in a sexual manner to be Unsubstantiated. An unsubstantiated finding means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
