Allegation investigated: Uncleared individual is providing care and supervision. Staff do not ensure that personnel records are complete.

On 8/5/26 at 10:00am an initial complaint visit was conducted by Licensing Program Analyst (LPA) J.Leffall. LPA met Assistant House Manager (HM) Christa Rowland. HM greeted LPA and LPA stated purpose of the visit. The department conducted interviews with staff and reviewed employee files. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did occur, therefore the allegation is Unsubstantiated. No deficiencies were issued. Exit interview conducted. A copy of this report was distributed to House Manager which confirms signature of this report. Today, Licensing Program Analyst L. Xiong arrived at the facility unannounced to conduct the Annual Inspection. LPA met with staff Britney Lopez and inform her the purpose of the visit. Staff brought records for review and provided the facility tour for LPA. Facility appeared clean with no obstruction or fire clearance issues. All common areas have adequate seating and lighting. Resident bedrooms toured, rooms observed to have all required accommodations. Kitchen toured, LPA observed a 2-day supply of perishable and a 7-day supply of non-perishable food available for residents. Smoke detector and carbon monoxide detectors observed operational during inspection. Fire extinguisher present with a service date of 1/2026. Water temperature observed to measure at 110 degrees F. Allegation investigated: Staff did not prevent resident from sexually abusing another resident in care

On this date, Licensing Program Analyst (LPA) L. Xiong arrived unannounced to deliver findings on the above allegations. Interviews with residents revealed that residents enjoy being at the facility, feel safe, and stated that staff are friendly and caring. The Porterville Police Department investigated the above allegation and determined no crime occurred. The Department investigated the complaint alleging: Staff did not prevent resident from sexually abusing another resident in care. Based on interviews and record review the above allegation is UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are unsubstantiated. Licensing Program Analyst (LPA) L. Xiong conducted the complaint investigation visit to the facility. During the course of this investigation LPA interviewed staff and resident, and reviewed facility files relevant to the complaint investigation. It was determined that the above allegation: Facility staff pinch residents, Facility staff verbally abuse residents, and Facility staff force residents to eat meals are UNFOUNDED. The evidence from the investigation indicated facility staff did not physically or verbally abuse residents. Facility staff did not force fed resident. This agency has investigated the complaint alleging (Facility staff pinch residents, Facility staff verbally abuse residents, and Facility staff force residents to eat meals). Today, Licensing Program Analyst L. Xiong arrived at the facility unannounced to conduct the Annual Inspection. LPA met with staff Ofelia Alverarado and Licensee Elysia Boyd and inform them the purpose of the visit. LPAs observed a central entry point with a supply of hand sanitizer and a sign in policy that includes documented routine symptom screening for resident's, staff and visitors. LPA introduced self and allowed entrance by staff. All COVID-19 guidelines are in place. Visitor log-in/temperature check was observed upon entry. Licensees brought records for review and provided the facility tour for LPA. Facility appeared clean with no obstruction or fire clearance issues. All common areas have adequate seating and lighting. Resident bedrooms toured, rooms observed to have all required accommodations. Kitchen toured, LPA observed a 2-day supply of perishable and a 7-day supply of non-perishable food available for residents. Smoke detector and carbon monoxide detectors observed operational during inspection. Fire extinguisher present with a service date of 12/2023. Water temperature observed to measure at 110 degrees F.
