Type of visit: Required - 1 Year
On 06/20/2026,, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required visit. LPA introduced self, stated the purpose of the visit, and allowed entrance by Licensee/Administrator Shelby Price. . Currently, there are 5 residents in care. Two residents were present at start of inspection and observed to be participating in individual activities of choice, the other 3 residents were on a community outing with staff. The facility was observed to be at a comfortable temperature, clean, and free of odor. LPA observed adequate seating available for all residents in common area. Resident bedrooms toured, LPA observed resident's bedroom to have required furnishing available. Bathrooms observed to have grab bars, shower chairs, and slip resistant mats available. Bathroom fixtures observes operational, hot water temperature was tested at 113 degrees F. Kitchen toured, LPA observed the sharps and knives to be locked and secured in kitchen drawer. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available for residents. Medications are locked and secured in kitchen cabinet. Medications reviewed observed to have original labels and to be administered as prescribed. Fire extinguisher present with a service date of 2/23/2026, carbon monoxide and smoke detectors tested and observed operational at time of inspections. Outside of facility toured. Side gate was self-closing and self-latching. Facility has large shaded patio with outdoor seatings available for residents. All exits observed to open freely and are free of obstruction. LPA reviewed resident and staff files during inspection. LPA requested a copy of facility liability insurance, administrator certificate, LIC 500 and LIC 9020 to be submitted to Fresno Regional office no later than Friday, July 3, 2026.
