On December 2, 2025, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted the facility administrator (AD) Julie Gorordo about the visit. For this visit, there are two staff members on duty, both of which are background cleared and associated. AD later arrived to assist with the inspection. The PUB475 ‘See Something, Say Something’ poster was observed to be located in the living room. Resident Rights is posted in the dining room. LPA observed the Administrator's Certificate for Julie Gorordo, which expires on July 17, 2027. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six residents, six of which may be non-ambulatory, none of which may be bedridden, and a hospice waiver for six. LPA toured the interior and exterior portions of the facility with AD. For this visit, there are a total of three non-ambulatory residents in care, one of which is on hospice, and none are bedridden. The facility is a single story home. There are a total of six bedrooms, four of which are private resident bedrooms, one shared resident room, and one bedroom for staff. LPA toured each bedroom with the AD and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and free of any hazards. LPA observed the staff room is kept locked and inaccessible to residents in care. Smoke and carbon monoxide detectors as well as auditory exit alarms were tested and operational. There are a total of three full bathrooms. Continued on LIC 809-C. Bathrooms were observed to be in good repair, toilets and faucets were operational and showers were equipped with grab bars and non-skid floor mats. Water temperature in the bathrooms were measured to be between 113 and 113.1 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in a kitchen cabinet, inaccessible to residents in care. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguisher was dated and tagged on November 4, 2025. LPA observed the emergency disaster and evacuation plan, which is posted in the dining room. LPA observed the facility conducted their last emergency disaster drill on December 1, 2025. Facility had back-up emergency food and water supplies, located in the garage. LPA observed that the First Aid kit had all the required components. Medications were observed to be locked in a medication cabinet in the kitchen, inaccessible to residents in care. Chemicals were observed to be locked underneath the kitchen sink and in the garage. LPA observed the door leading to the attached two car garage is kept locked and inaccessible to residents in care. The garage is used for storage and laundry. For the exterior portion, LPA observed patio furniture under shading, and the grounds were free of any hazards or obstructions. There is one self-latching gate in the backyard that can be opened in case of an emergency. There is a private Accessory Dwelling Unit (ADU) being rented to a tenant in the backyard. No bodies of water were observed. During this visit, three resident files and three staff files were reviewed. All staff are background cleared and associated with the facility. LPA reviewed residents’ medication and medication records and two resident interviews were conducted. Based on today's observations, there are no deficiencies being cited per Title 22 of the California Code of Regulations. An exit interview was conducted with Julie Gorordo. Licensing Program Analysts (LPAs) Joseph Alejandre and Brandon Lopez made an unannounced visit to conduct the required annual inspection. LPAs were greeted and granted by entry by caregiving staff after explaining the purpose for the visit. Administrator Julie Gorordo and Arda Kardijian were notified via telephone and later arrived to assist with the inspection. LPAs observed Administrator Arda Kardijian has a valid Administrator certificate which expires on September 19, 2025. The facility is a Residential Care Facility for the Elderly (RCFE) which is licensed for six nonambulatory residents and has a hospice waiver for six. The facility is a single story home with five resident bedrooms, one staff bedroom, three bathrooms, a living room, a dining room, a kitchen, and a two car garage. LPAs, accompanied by Administrators conducted a tour of the physical plant. On today's visit, LPAs observed three residents in care and two care giving staff present. LPAs observed the See Something, Say Something poster (PUB 475) mounted on a closet door by the entryway. LPAs inspected the five resident bedrooms. All resident bedrooms had the required furnishings of a bed, a chest of drawers, a lamp, and a chair. Resident bedrooms are clean and spacious. LPAs inspected resident bathrooms. Bathrooms are clean. Toilets and faucets tested operational. Bathrooms were equipped with grab bars and non-skid floors. Hot water temperature measured 115.9 degrees Fahrenheit. LPAs observed the facility has a minimum two day perishable and seven day nonperishable food supply on hand in the kitchen. LPAs observed kitchen appliances were clean. LPAs observed the five burner electric cook top is operational. Cleaning supplies were observed to be locked under the sink cabinets. Knives were observed to be locked in a kitchen cabinet. LPAs observed a fire extinguisher mounted in the dining room. Fire extinguisher was charged and up to date on service. LPAs observed the facility conducted their last emergency disaster drill on December 10, 2024. LPAs tested the dual smoke/carbon monoxide detectors which tested operational. The door leading to the garage is kept locked and inaccessible to residents. The two car garage is used for storage and laundry. LPAs observed the facility has a three day emergency food and water supply stored in the garage. LPAs, accompanied by the Administrators toured the exterior portion of the facility. No bodies of water observed. LPAs observed a shaded seating area with furniture for resident use. The backyard has one gate and is operational. Continued on LIC809-C The front yard of the facility is undergoing plumbing repairs. The facility has all utilities and is not being impacted by the repairs in the front yard. The area under repair is off limits and poses no risk to the residents. The construction in the backyard is off limits and poses no risks to the residents. LPAs inspected the First Aid Kit which has all the required components. The facility has a internet device which is dedicated for resident use. LPAs inspected three resident records and three staff records. No discrepancies observed. LPAs reviewed three resident medications and medication records. No discrepancies observed. All staff are background cleared and associated to the facility. Based on today's observations, there are no deficiencies being cited per Title 22 Division 6 of the California Code of Regulations.
