Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct the required annual visit today at 09:15 a.m. Upon arrival, the LPA contacted the Administrator, Donald Brodt telephonically and informed them of the visit. The Administrator arrived at approximately 10:00 a.m. Entrance interview conducted. Beginning at 10:01 a.m., the LPA along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards, and facility is in compliance with Title 22 Regulations. The following was observed: Kitchen: The LPA inspected the kitchen/food service area at approximately 10:12 a.m. Knives and sharps were observed in a locked drawer at the time of the visit. Cleaning supplies were observed locked and inaccessible under the kitchen sink. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. The LPA observed an adequate amount of emergency food and water. At 10:13 a.m., the hot water temperature was measured in the kitchen sink at it measured at 112.8 degrees Fahrenheit. Common Areas: At the time of the visit, furniture in the common areas was observed to be in good condition. The facility maintained a comfortable temperature. The LPA observed a fireplace in the living room; adequately covered. Report Continued to LIC 809… Report Continued from LIC 809... The LPA observed several fire extinguishers to be fully charged with a purchase date of 04/01/2026. Required postings were observed throughout the common space. There is a working telephone on premises. First aid kit was observed to be complete. No hazards/obstructions observed inside or out. Restrooms: There are six (6) restrooms for resident use. Each bedroom has its own bathroom. Bathrooms were clean and sanitary and in operating condition with non-skid surfaces and grab bars. The bathrooms were sufficiently stocked with supplies and paper towels. Starting at 10:04am, the hot water temperature was measured in resident bathrooms, and they measured between 112.6 and 113.7 degrees Fahrenheit which is within the required range of 105 – 120 degrees Fahrenheit. Bedrooms: There are six (6) bedrooms for resident use. Bedrooms are designated as private / single bedrooms. Bedrooms were observed to be furnished appropriately and had sufficient lighting. Additional clean linens and towels were observed in a closet by the hallway. Staff bedroom observed on premises. Utility Room: Detergents and cleaning supplies were observed locked and inaccessible to residents in care at the time of the visit. The LPA observed an additional refrigerator and freezer in good condition. Laundry Room: The LPA observed a washer and dryer in good condition. Detergents and toxins were observed locked at the time of the visit. Outdoors: The backyard and front yard have a shaded area with adequate furniture for resident use. Emergency passageway was observed to be clear of any obstructions. There is a side gate with latching mechanisms for emergency purposes. No bodies of water noted at the time of the visit Records: Record review began at approximately 10:30am. Six (6) resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, consent for treatment form, preplacement appraisals, appraisals, and current needs and services plan. All files were in order. Report Continued on LIC 809C... Report Continued from LIC 809C... Five (5) personnel files were reviewed for, but not limited to: personnel record, health assessment with negative TB test results, criminal record clearance, first aid/CPR training, and the appropriate yearly training. All personnel files were complete. The Administrator’s certificate is valid until 07/21/2027. During today’s visit, the LPA conducted interviews with three (3) residents and no concerns were noted. Medications: The LPA conducted a medication review for three (3) residents. Medications are centrally stored and kept in a locked cabinet adjacent to the dining room. All medications including PRNs were labeled, stored, and locked inaccessible to residents in care. Medications appear to be administered as prescribed at the time of the visit. Emergency Disaster Planning: During today’s visit, the LPA reviewed the facility's emergency disaster plan, which was observed to be complete and recently reviewed/updated. Emergency disaster drills conducted quarterly as per regulation; last disaster drill conducted on 05/02/2026. No citations issued at this time. Exit interview conducted. Licensing Program Analyst (LPA) Angela Barutyan conducted a pre-licensing visit to the facility noted above at 10AM. LPA met with Applicant Ribka Brodt and Applicant Representative Donald Brodt. The applicant has obtained fire clearance for a total capacity of six (6) residents. LPA inspected facility for Fire Safety, Personal Accommodations and Services, and Food Service. The facility is a two (2) story home. The facility has six (6) private resident bedrooms of which two (2) are on the first floor and four (4) are on the second floor. All resident bedrooms are cleared for bedridden and have direct exits to the exterior. The hard-wired smoke alarms and carbon monoxide detectors were tested and function properly. LPA observed fire extinguishers to be fully charged and purchased on 03/26/2025. LPA toured the kitchen area at approximately 10:13AM. The hot water temperature was measured in the kitchen sink, and it measured 108.9 degrees Fahrenheit. Appliances and all equipment appeared to be clean and in good repair. The kitchen has a sufficient supply of plates, cups, cookware, and utensils. Kitchen knives and sharps will be locked in a drawer by the stove. Resident bedrooms are equipped with clean mattresses, pillows, and bedding. Bedrooms have sufficient lighting. There is a sufficient supply of linens, including blankets, bath towels and wash cloths. Bedrooms were equipped with functioning auditory exit alarms and bed sensor pads. The facility has six (6) bathrooms for resident use. All bedrooms have an attached bathroom. Bathrooms contained appropriate slip-resistant surfaces and grab bars. Hot water temperature was measured in bathrooms and were between 107.8-109.9 degrees Fahrenheit, which is within the required range. Report Continued on LIC 809C... Report Continued from LIC 809C... The living areas and dining areas are clean and properly furnished. All window screens and coverings are in good repair. LPA observed enough seating for six (6) residents at the same time in the dining room table. A working telephone is present. There are activity supplies. There is one (1) fireplace which was observed adequately screened. Night-lights were present in the hallways. Medications will be stored and locked in a cabinet in the kitchen. Facility records will be kept in the staff office on the first floor. First aid kit will be stored in the laundry room and was observed complete. Additional cleaning supplies, toxins, a washer and dryer, disinfectants, cleaning supplies, and an additional refrigerator and freezer were observed in the utility room. There will be no firearms/ammunition stored on the property. The facility has required postings, including Emergency Exit Plan, Resident Personal Rights, and Theft and Loss Policy. The facility is on the hillside and has multi-level outdoor passageways which were free of obstructions. There are no bodies of water on the premises at the time of the visit. LPA observed a covered outdoor area with a table and chairs for client use. Facility has an emergency water tank in the backyard. All exits had functioning auditory exit alarms. Physical plant is consistent with the submitted facility sketch/floor plan. Comp III conducted with Applicant. Pre-Licensing is complete and this facility has no deficiencies. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect approval of your license. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
