Licensing Program Analysts (LPA) Jose Tan met with Administrator Rosario Soriano for a One (1) Year Required visit for this facility. LPA explained the reason for the visit. A tour of the physical plant was conducted at 1:02 PM and the following was noted: There is only one entrance being utilized at the facility. The facility had submitted and approved Mitigation plan. The facility has a designated visitors' area at the backyard. The facility has sufficient stock of PPE in the garage. The facility has five (5) bedrooms and three (3) bathrooms currently occupying six (6) residents. One (1) additional bedroom is designated for staff use. The facility is fire cleared for six (6) non-ambulatory residents, Hospice waiver for two (2) residents. Living and dining room furniture were also checked. The living room is neat and clean along with the family room. The facility maintains a comfortable temperature at 72°F. The smoke detectors are hardwired and interconnected and observed to be operational. There are carbon monoxide installed at the facility. Fire extinguisher is located in the kitchen and observed to be full and last checked on 02/21/26. The backyard of the facility has outdoor furniture, with a covered shaded area for clients. There is no body of water in the facility. There is an structure at the backyard being used as old equipment storage and was observed to be locked during the visit. There is no body of water at the facility. (continued on LIC 809-C) (continued from LIC 809) The garage is currently being used as tools, frozen food and other supplies storage. Laundry room is located in the garage. Laundry detergents, cleaning agents and other toxins are stored in a locked cabinet in the garage. Food Service/Kitchen area was sufficiently stocked with two (2) days perishable and seven (7) days non-perishable food. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked in a kitchen drawer and inaccessible to residents. The Clients' rooms are adequately furnished with appropriate furniture and lighting system. Hall ways/passage ways are lit. Clients have sufficient amounts of personal hygiene product which is provided by the licensee. Staff Room: Staff room was observed to be locked, located adjacent to the garage and has no access from the inside. No medications are observed in the staff room. The bathroom was checked for cleanliness and proper operation. LPA observed the appropriate grab bars in the toilet and shower. The hot water temperature was measured a range of 106.2°F to 107.1°F. Towels and washcloths are not shared. There was enough clean linen available in stock at the cabinet. Medications: LPA observed medication cabinet in the kitchen to be locked and inaccessible to residents. There is a complete first aid kit located inside the medication cabinet. Disaster drill was last conducted on 05/13/26. Required posting observed in facility (complaint hot line poster, personal rights, etc). At 9:30am, Licensing Program Analyst (LPA) Angela Panushkina arrived at the facility to conduct an unannounced annual inspection. Upon arrival, LPA met with the Staff #1, who granted access to the facility. The Designee, Felix Soriano, arrived shortly after and LPA explained the reason for the visit. At 9:40am LPA conducted a tour of the physical plant and observed the following: The facility is licensed for capacity of six (6) non-ambulatory residents. The facility also has approved hospice waivers for two (2). There are four (4) private and one (1) shared bedrooms designated for residents’ use. Bedrooms are appropriately furnished and have appropriate lighting. The facility has awake staff. Bathrooms have soap, paper towels and hand washing signs were observed. Extra towels and linens were readily available. The hot water temperature measured at 107.7.0°F. Facility maintains a temperature of 75°F. LPA observed there to be sufficient stock of one-week perishable foods and two-day non-perishable foods. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. The fire extinguisher is located in the kitchen and was last serviced on 02/11/2025. The laundry is located in the garage. The washer/dryer appears to be in good condition. Laundry supplies are kept inaccessible when not in use with supervision. All chemicals, detergents and medications are kept in the laundry room and inaccessible to residents in care. Smoke detectors and carbon monoxide monitors were tested at 10:30am and observed to be functional. At 10:40am, LPA observed appropriate outdoor furniture, with a covered shaded area for residents. Continue on LIC809-C LPA discussed the importance of maintaining care and supervision to meet the needs of residents. Between 11:00am to 11:30am, LPA reviewed records of five (5) residents and four (4) staff. Client and staff records appeared to be complete and updated. The annual fee is paid in full. LPA observed Administrator certificated renewed on 012/17/2024 with an expiration date of 12/16/2026. LPA collected the Certificate of Liability Insurance and LIC500. Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted an annual required visit and inspection of the facility. LPA met with staff, Aguida Roche and explained the reason for the visit. Approximately, around 10:18 AM administrator Rosario Soriano arrived and was explained for the reason of the visit. At 10:34 AM, with the assistance of administrator, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are operational. There are carbon monoxide detectors that functions properly. The fire extinguisher is in the kitchen. The charge date is 1/10/2024. During the visit the facility is at 73 degrees Fahrenheit. The facility is fire cleared for six (06) non-ambulatory; cleared for 2 hospice waiver. Kitchen: The kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found enough at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrap, dated, and stored properly as well. Knives were stored in a locked drawer in the kitchen. Office space is in the kitchen area beside the pantry. Properly labeled medications were locked in the dining area cabinets in the kitchen. Bedrooms: There were six (6) bedrooms designated for residents' and staff use. Five (5) of the that are in use by residents were properly furnished with appropriate dresser, beddings, and linens with sufficient lighting. Room #1, #2, #3, and #4 are properly furnished and occupied by only one (1) resident. Room #5 is shared room, but has only one (1) occupant. There is one (1) staff bedroom in the facility and is located beside the garage, staff bedrooms are locked with no medication in sight. Continue to LIC 809-C Bathrooms: There are three (3) bathrooms designated for residents' and staff use. Bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured at 105.1 degrees Fahrenheit for bathroom #1 located in the hallway beside bedroom #3. Bathroom #2 measured 106.1 degrees Fahrenheit inside bedroom #1. Bathroom #3 is located in the hallway beside bedroom #4, hot water measured at 106 degrees Fahrenheit. Cleaning supplies are being stored in a locked cabinet in the laundry room in the garage. Towels and washcloths are not shared. There was enough clean linen available in the cabinets at the end of the hallway.. Common Areas: These included the living room and dining area for residents. The common areas were properly furnished. The auditory alarms on all exit doors were on and functional at the time of the visit. Residents dining table fits enough for six (6). Surrounding Grounds: Entry and exits were free of obstruction. There was furniture appropriate for outdoor use. The outdoor area was free of hazards. The laundry area and detergents are located by the laundry room in the detached garage. The facility does not have a swimming pool or body of water. The garage is detached and is used for storage for incontinence for residents and staff refrigerator. Fire place in the living room is closed, block-off and non-operational. Resident Files: LPA conducted a file review of resident records to ensure compliance of licensing forms. Staff Files: LPA also conducted a file review of staff records to ensure forms and training are up to date and compliance with licensing forms. Medications: Medication and Medication Records (MMR) were review for proper documentation. First aid kid is also current and complete. Pursuant to Title 22 Division 6 of the CA Code of Regulations, no deficiencies observed during the visit.
