Allegation investigated: Facility is charging for basic hygiene products

On 04-20-2026, Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted a subsequent visit to investigate the allegation that the facility charges residents for basic hygiene products. LPA met Wellness Director Diana Vera who was informed of the purpose of the visit. LPA interviewed staff and residents and reviewed facility practices related to resident access to hygiene supplies. LPA interviewed multiple staff members and the information obtained explained that to encourage engagement in activities, the facility uses an incentive program called “Sunny Rose Bucks ,” which functions similarly to bingo bucks. Residents earn these tokens by participating in scheduled activities, and they may later use the tokens in the resident store, which operates weekly. Unfounded Staff members stated that the resident store provides items such as shampoo, body wash, deodorant, tissues, toothbrushes, toothpaste, and other hygiene products. They clarified that the Sunny Rose Bucks is not a form of payment and is not required to obtain hygiene products. Facility staff also stated that the facility keeps hygiene supplies on hand and provides them free of charge to any resident who requests them, regardless of whether the store is open or whether the resident has Sunny Rose Bucks. Interviews with residents revealed that the facility does not charge for hygiene products. LPA reviewed the process and confirmed that residents are not billed, invoiced, or required to use personal funds to obtain hygiene products. The incentive program is intended only to promote social participation and does not replace the facility’s obligation to provide basic hygiene items at no cost. No evidence was found to indicate that the facility charges residents for hygiene items or denies access to those who do not participate in activities. Based on interviews, observation and records review , the preponderance of evidence standard has not been met, and the allegation is determined to be UNFOUNDED. Meaning the allegation is false, could not have happened, and/or is without a reasonable basis. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Yolanda Delgado arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry by Ruby Valencia, Community Liaison Director. LPA began inspection with introduction, visit purpose and provided Community Liaison Director with LPA identification and business card. Due to active COVID cases at the facility, no interviews were conducted. Resident record review began- Eight (8) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. This facility is meeting documentation requirements. Employee records review began- Ten (10) records were reviewed. LPA reviewed employee record for first aid certification, fingerprint clearance, personnel/job application, health screening and TB test results, criminal record statement, employee rights, training verification, and current administrator certification. CPR and requirements have been met. The facility employs enough staff to maintain cleanliness and meet the needs of the clients in care. Administrator certification is present and current 03/10/2027. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The facility is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 109.0 degrees F. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals in the designated areas. All outdoor and indoor passageways are free of obstruction. A locked area is provided for medications and sharp objects. LPA verified there is a telephone working at this location. (Continued on LIC809, Page 2) (Continued from LIC809, Page 1) Food Service- Food supply meets the of one week supply of nonperishable and 2-days supply of perishables food on hand. A menu is posted, foods are dated to assure safety. Food prep areas are clean and organized. LPA made observations throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation, and the structure remains unchanged according to the approved floor plan. Smoke detectors and carbon monoxide detectors were not tested due to quarantine for COVID, however LPA requested and received documentation of Fire Safety Service, Inc. report of Inspection/Test done on 12/27/2024 that are conducted yearly and found to be operational. Fire extinguishers are tested or replaced annually and were last done so on 09/17/2025. The facility conducting emergency disaster drills quarterly; last done on 09/04/2025. There are no known firearms and ammunition stored on the property and no bodies of water observed. Corporation is active and in good standing. Based on the information received during this visit today, there are zero (0) deficiency is being cited per Title 22, Division 6 of The California Code of Regulations. Type of visit: Prelicensing

On 10/14/24 Licensing Program Analyst (LPA) Javina George made an announced visit to the facility to conduct a prelicensing inspection-Change of Ownership (CHOW) LPA was greeted and granted entry by Executive Director Karen Roper. Karen accompanied LPA on a tour of interior and exterior areas of the facility The facility is a single story structure consisting of residents age range 60 and above, that will reside in either memory care or assisted living designated building. The facility has an approved hospice waiver for (30), with (28) residents currently receiving hospice services. The Menifee Fire Department issued an approved fire clearance for (20) bedridden ridden, and delayed egress on 2/13/24. Exterior: There is designated smoking area along the back wall, with a fountain filled with rocks, an outdoor fireplace and grill in the middle of both the assisted living and memory care buildings. The grass was observed to be green and freshly cut. In addition to the shrubbery being maintained. Interior: The facility was observed to have the pull cords inside the resident bathrooms as well and bedrooms that were observed to be operable in both buildings. Memory care (MC): was observed to have a total of 21 bedroom/bathroom combo style rooms. There is a satellite kitchen, dining area, designated area for activities, locked medication room which contains resident charts, and laundry room. The hot water temperature was checked and found to be within regulatory limits measuring 112.2-116.6 degrees Fahrenheit. Assisted living (AL): This building was observed to have staff offices, locked medication room, main kitchen dining room, and food supply which was sufficient (2 day of perishable and a 7 day supply of nonperishable food items). Conference room, salon, activities room, laundry room, front lobby/living room. The hot water temperature was checked and measured at 105.8-111.8 degrees Fahrenheit. The facility was observed to be clean and clutter free, there were some visible stains on the carpet throughout the facility in both the assisted living as well as memory care building. The facility is currently remodeling areas throughout the facility and has already had the tile replaced in the main kitchen and carpet inside of some of the resident bedrooms, replaced with wood flooring. The facility was observed to have fully charged fire extinguishers which were serviced on 2/23/24. The smoke and carbon monoxide detectors were observed to be operable and were last services on 10/3/24-Carbon monoxide and 10/4/24-Smoke detectors. There are no known guns or ammunition on the premises. The facility does not have any pools or bodies of water. The facility had the required postings, and an adequate amount of personal protective equipment (PPE) supplies. The facility was observed to have liability insurance which needs to be renewed before 10/18/24. The facility has an ample amount of activities available for residents in care and to promote socialization. The facility posts the individual activities outside of the resident bedrooms and has a enlarged activities calendar posted in both buildings for residents to see. Based on today's inspection the facility the facility is ready for licensure.
