Licensing Program Analyst (LPA) Jerome Haley conducted an unannounced visit for the purpose of conducting a required one-year annual inspection. LPA was greeted, granted entry by staff and explained the reason for the visit. Structure: The facility is a two level structure and the second level of the home is off limits to the residents in care. The facility currently has two residents, and both residents were present during the visit. There’s a total of 6 bedrooms and 2 restrooms available. There’s a living room space, a dining space, laundry area, backyard area and a detached garage. Bedrooms: All bedrooms have the required furnishings: bed, lamp, chair, and storage space. Bathroom(s): Bathrooms are equipped with a working toilet, wash basin, and shower. Hot water measured in between 117.6 – 117.8 degrees F. Kitchen: 4 of 4 burners and the warmer were operational on the gas stove. Sharps are stored in a drawer near the stove equipped with a lock. Soaps and cleaning chemicals are stored below the sink in a locked cabinet. Food Service: A food supply that meets regulation requirements was observed. Client & Staff Files: Resident and staff files are located in locked cabinet across from the dining room table. File Review: Two resident files and two staff files were reviewed during the inspection. Medications/First-Aid Kit: Resident medications are stored in a kitchen drawer equipped with a lock. A first aid kit with all the required elements was observed in a cabinet across from the dining table. Medication Review: One resident medication was reviewed during the visit, as only one resident is prescribed medication. Continued on LIC809C Linens & Hygiene Supplies: Hygiene items were observed in the main bathroom area stored below the sink. Additional hygiene items were observed in the main hallway closet. Additional linens are stored in a cabinet in one of the bathrooms. Garage Area: The garage is used to store miscellaneous facility items like mattresses, incontinent care items, and tools. The garage is locked and off limits to residents. Backyard/Exterior: The backyard is clean and organized. Walkways are free of obstruction. A table and chairs was observed under a shaded patio area near the detached garage. Bodies of Water: The swimming pool is urrounded by a self-latching fence that meets regulation requirements. Smoke/Carbon Monoxide Detectors: Smoke and carbon monoxide detectors tested operational. Fire Extinguisher: A fire extinguisher was observed in the kitchen near the laundry area An emergency evacuation drill: Was conducted July 1, 2026. Emergency Phone Numbers, House Rules, Exit Plan & Menu: Several facility postings are posted and available for review on the main postings board in the kitchen. Two residents present during the visit and both residents were sleeping during the visit. Right next to the front door there’s a closet with shoes and jackets, and PUB 475 poster hanging next to the front door. Resident bedrooms were clean, organized, and had all the necessary requirements: nightstand, chair, lamp, and storage space. Resident bathrooms were clean and organized. Hot water temperature was measured in the range of 117.6 – 118 degrees Fahrenheit. The kitchen was clean and organized. Knives and sharp objects were locked in a drawer near the stove. All burners on the stove were operational. The facility has a two-day supply of perishable food items and seven-day supply of nonperishable food items. There’s a laundry area in the back of the kitchen with a washer, dryer, locked cabinets with a supply of cleaning chemicals, and a pantry area with an additional supply of nonperishable food items. The backyard had a shaded area with tables and chairs for the residents to enjoy. There's a pool is surrounded by a 5-foot self-latching gate. The garage is locked and off limits to residents. The garage is used as a workshop for the licensee. In the garage, several tools, bicycles, and a motorcycle was observed. All smoke detectors were tested and are operational, and the carbon monoxide detector tested operational. Emergency evacuation drills are conducted quarterly, and the last drill was conducted in July 1, 2025. A review of 2 of 2 resident files and a review of 2 of 2 resident medications was conducted. Two staff files were reviewed and two staff were interviewed. No deficiencies are being cited as a result of today’s annual inspection. Licensing Program Analysts (LPA) Jerome Haley and Samer Haddadin conducted an unannounced visit for the purpose of conducting a required one-year annual inspection. LPAs were greeted, granted entry by staff, and explained the reason for the visit before entering the facility. Administrator Maria Hunter contacted Licensee/Administrator Kenneth Hunter who arrived a short time later and was present the remainder of the visit. Around 09:20 am LPAs began the tour of the facility with staff. There were two residents present for the visit. Right next to the front door there’s a closet with shoes and jackets, and PUB 475 poster hanging next to the front door. Resident bedrooms were clean, organized, and had all the necessary requirements: night stand, chair, lamp, and storage space. Resident bathrooms were clean and organized. Hot water temperature was measured in the range of 112.1 – 118.4 degrees Fahrenheit. The kitchen was clean and organized. Knives and sharp objects were locked in a drawer near the stove. All burners on the stove were operational. The facility has a two-day supply of perishable food items and seven-day supply of nonperishable food items. There’s a laundry area in the back of the kitchen with a washer, dryer, locked cabinets with a supply of cleaning chemicals, and a pantry area with an additional supply of nonperishable food items. The backyard had a shaded area with tables and chairs for the residents to enjoy. There's a pool in the backyard surrounded by a 5-foot self-latching gate. There’s a locked storage shed in the backyard used to store various facility items. In the lock storage area, there was an additional refrigerator with a an additional supply of perishable food items, walkers, wheelchairs, and other items. The garage is lock, off limits to residents, and used as a workshop for the licensee. In the garage, several tools, bicycles, and a motorcycle was observed. Continued on LIC809C All smoke detectors were tested and are operational, and the carbon monoxide detector tested operational. Emergency evacuation drills are conducted quarterly, and the last drill was conducted in April 2024. Licensee/Administrator Kenneth Hunter was advised to keep a log of all the evacuation drills that are conducted and include the name of all staff and residents in attendance. A review of resident files and medications was conducted, as well as a review of 2 staff files. Deficiencies are being cited as a result of observations made during todays annual inspection.
