08/27/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There are currently three residents in care with one resident on hospice. Facility has an approved fire clearance for 6 non-ambulatory. Facility has a hospice waiver granted for 2. LPA was greeted by Administrator, Helen Rabago. LPA and Administrator toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Refrigerated food was found to be stored in a safe manner. Outside exits were free from obstruction. All rooms were furnished and in good repair. Extra hygiene products and linens were available. Water temperature in sinks accessible to residents in care were measured within the range of 105 to 120 degrees F. Fire extinguishers were last inspected 03/2026. Smoke/Carbon Monoxide detectors located throughout the facility are hardwired and were tested and operational. Sharps are located in the kitchen and were found to be secured. Chemicals were found to be secured. Medications were found to be centrally stored, however LPA observed pre poured medications in a plastic container (Deficiency Cited). LPA conducted spot medication count and found all prescription medication to be properly recorded on the Centrally Stored Medication Record. LPA conducted a review of three resident records. All records had the required documentation along with hospice care plan. LPA conducted review of two staff records/training. Upon a review of staff records, LPA found all staff to have required training as well as current 1st Aid & CPR certification on file. continued on LIC809C Facility conducts quarterly fire drills with the last one being conducted on 6/16/2026. Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. 08/07/2025, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. Facility has an emergency disaster plan as required. Facility has an infection control plan as required. There are currently 5 residents in care. Facility approved/cleared for 6 non-ambulatory. Facility has a hospice waiver for 2. LPA was greeted by staff member. Administrator, Helen Rabago arrived shortly after. LPA and administrator toured the building and grounds. The facility was found to be at a comfortable temperature. LPA observed a 2 day supply of perishable and 7 day supply of non-perishable food. Refrigerated food was found to be stored in a safe manner. LPA observed residents interacting along with a workout video. Administrator stated they have other activities available for residents such as board games, coloring, and drawing. All rooms were furnished per regulation. All rooms were in good repair. Extra hygiene products and linens were available. Water temperature in sinks accessible to residents in care were measured at 111.9 and 107.6 degrees F which is within the range of 105 to 120 degrees F. Fire extinguishers were last inspected 01/2025. Smoke/Carbon Monoxide detectors located throughout the facility were tested and operational. Sharps are located in the kitchen and were found to be secured. Chemicals were found to be secured. Medications were found to be centrally stored. LPA conducted spot medication count and found all prescription medication to be properly recorded on the Centrally Stored Medication Record. Facilities last fire/emergency drill was conducted 05/19/2025. continued on LIC809C LPA conducted a review of 5 resident records. All records had the required documentation. However, LPA observed residents R1 and R2 to not have a needs and service plan on file. Administrator ensured LPA they will have R1s and R2s needs and service plan completed and on file. LPA observed R2 to not have an updated physicians report on file. Per conversation with administrator, they have been having trouble getting R2s physician report to be completed and filled out by R2s physician. Administrator will reach out to physician to get form completed. LPA observed a hoyer lift that is used for R4. Administrator to ensure facility gets a doctors order and proof of all staff training for hoyer lift to be used for R4, along updating R4s care plan indicating the use of the hoyer lift. LPA conducted review of 3 staff records/training. Upon a review of staff records, LPA found all staff to have required annual and initial training as well as current 1st Aid & CPR certification on file. No deficiencies cited during today's inspection. Licensing Program Analyst (LPA) M. Cuadra conducted an unannounced Annual Required 1 Year inspection and met with Administrator Helen Rabago. There are residents receiving hospice services and diagnosis of dementia. Annual fees are current. Required postings were observed. LPA/Administrator toured the facility inside and outside at approximate 10:00am and observed the following: Facility is clean, in good repair and at a comfortable temperature with all exits free from obstruction. Fire extinguisher charged and serviced February 2024. Smoke detectors and carbon monoxide detector were found to be all operational during the visit. Bathrooms were equipped with necessary grab bars and non-slip floors/mats. All three bedrooms used by residents did not have chairs as indicated per regulation. Administrator placed chairs on resident's rooms as stated per regulation (technical violation issued). Hot water temperature measured between 114.12 and 113.0 degrees F, in resident's bathrooms, which is within regulation of 105 to 120 degrees F. Last disaster drill was conducted on June 14, 2024. There is a minimum of one week supply of nonperishable foods and two days of perishable foods and it was stored and handled per regulation. However, during today's visit LPA did not observe any snacks been provided to residents in care. Per Administrator, sometimes they don't provide residents with morning snacks just afternoon. LPA discussed with Administrator that between meal nourishment or snacks shall be made available for all residents unless limited by dietary restrictions prescribed by a physician. Also, LPA did not observed any activities been encouraged to residents. Per Administrator, sometimes they go outside to exercise. There was an ample supply of hygiene products and paper products available for residents. Medication centrally stored and locked. Auditory alarms were present and operational. Continues on LIC809C... During tour of the facility LPA/Administrator at approximate 10:30am observed a container of sharps including needles was sitting on top of the fireplace accessible to residents in care. Administrator removed the container immediately and placed it in the garage. At approximate 10:45am LPA initiated file review. Five resident's and three staff files were reviewed. Two out of five residents who has a diagnosis of dementia do not have a current medical assessment. Four out of five resident's (R1, R2, R3 & R4) care plans needs were updated, but not signed by their responsible party. LPA/Administrator discussed the importance of having resident's responsible party to acknowledge any changes of care plans. Resident (R5) sharing room with a person who is terminally ill does not have a current agreement to grant access to the shared living space to others on file (technical violation issued). Staff have required CPR/1st aid certificates and additional training hours. Administrator Certificate of Helen Rabago #6021326740 expires 10/28/2024. Medication and medication records were reviewed. Contact information was reviewed. Administrator agreed to submit copies of the following by not later than 9/20/24: LIC 308 Designation of Administrative Responsibility, LIC 500 Personnel Summary, LIC 610E Emergency Disaster Plan (if there are any changes), Copy of Liability Insurance Certificate and control of property. Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Licensing Program Analyst (LPA), Carol Fowler is conducting an Required-1 Year inspection, on 9/28/2023 at approximately 02:00pm, and met with Helen Rabago, Administrator. LPA observed two on duty during the inspection. Currently four (4) client in care. Facility has an approved fire clearance four (4) ambulatory and (2) two bedridden clients. The facility has required emergency disaster plan. Facility scheduled evacuation fire drill was conducted on 07/12/2023, including staff & client. Client,1 out of 1, special diets regarding food are followed per staff interviews, and per LPA's observations during the inspection. The LPA reviewed four (4) staff files. Administrator certificate for Helen Rabago is current-#6021326740, expires 10/28/2024. All staff have required criminal record clearance. All staff have required training. The LPA reviewed four (4) client files. Client files were complete. The LPA toured the facility with the Administrator. All exits were unobstructed. The facility fire extinguisher was serviced and tagged as required expires 04/08/2023. Facility had six (6) smoke alarms, and all were working properly when checked during the inspection. Facility had one (1) carbon monoxide detector that were working properly when checked during the inspection. Facility had a first aid kit stored in the hallway storage, it did have a required first aid booklet. The facility had a sufficient supply of perishable and nonperishable food. Continue on LIC809C The facility had food, water, and emergency supplies to meet the 72 hour shelter in place requirement. The facility had a sufficient supply of personal protective equipment(PPE) for use as needed. The facility had a sufficient supply of hygiene supplies, cleaning supplies, and paper products for use as needed. The LPA observed the facility to be clean and orderly during the visit. The LPA observed that resident rooms, common areas, hallways, and bathrooms had sufficient lighting for clients in care. Clients rooms had required accommodations per regulations. Facility had all medications locked up and inaccessible to clients in care as required. During the tour LPA observed the following deficiencies: unlocked toxins (cleaning supplies) no staff member who has cardiopulmonary resuscitation (CPR) training and first aid training on duty LPA is requesting the following forms be updated and submitted to CCL by 10/11/23: · LIC 500 -Personnel Report · LIC 610 - Disaster Plan · LIC 308 - Designation of Responsibility · LIC 308 - Copy of Administrator Certificate · Infection Control Plan If updated Control of property Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation.
