Licensing Program Analyst (LPA) Jill Nakagawa conducted a 1-year required inspection. There are four residents, two were at dental appointments, 2 were at Day Program at the time of the inspection. There was one staff member at the facility at the time of inspection. Administrator was at the dental appointment with clients but arrived after appointment was complete.. LPA toured the facility with Administrator. Facility was found to be clean, orderly, and at a comfortable temperature with all exits free from obstruction. The kitchen was clean and well organized. Toxins and sharps for the kitchen are stored and locked in laundry room. There was an adequate supply of perishable and non-perishable food, which were stored appropriately and dated and bug-free. Medications were stored in locked medication closet making them inaccessible to residents and staff that do not handle medications. All bathrooms had grab bars, and non-slip mat/flooring for bathing as needed. There was a sufficient supply of hygiene products, cleaners, and paper products for use as needed. Each bathroom had soap and paper towels. Facility has a sufficient supply of personal protective equipment (PPE). Facility has an Infection Control Plan. The fire extinguisher was serviced on 08/29/2025 and was fully charged. The carbon monoxide detector was tested and operational. The facility has a central fire alarm system. Fire drills are done monthly; the last one was on 12/15/2025. The water temperature was 117.1 degrees F. The outdoor area of the facility was free of debris. There is an outdoor dining area with covered table and chairs. There are also abundant plants and planters for the residents to enjoy gardening in the warmer months. LPA reviewed four (4) of (4) resident files and found them to be complete. Three (3) of (3) staff files were reviewed and found to be complete. Continued on 809-C..... Continued from 809.... LPA observed areas of concern addressed in the Department of Developmental Services (DDS) onsite report dated 07/14/2025. Areas addressed were observed to be in compliance with Title 22 Regulations. LPA discussed new definition of Dementia and the ongoing training requirements. Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct the Annual Continuation of the facility. There are four residents residing at the facility however all four were attending Day Program at the time of inspection. There was one staff present. LPA inspected the facility and found it to be clean, organized and well-maintained. Residents' rooms were clean and were furnished as required per Title 22, with personal touches showing the residents' personalities. The living room had a large TV and computer monitor for residents use. There were games and arts and crafts supplies which the residents enjoy with staff each day upon their return from Program. Inspection of the kitchen found it to be clean and food and other storage was safe and well-maintained. Staff have completed the required training for 2024. LPA discussed the new regulations regarding care for residents with Dementia and Neuro-Cognitive Disorders. LPA left a copy of the PIN 24-09. There was one staff member at the facility at the time of inspection; Administrator was in a training class at the time. LPA toured the facility with carestaff at approximately 2:17 PM. Facility was found to be clean, orderly, and at a comfortable temperature with all exits free from obstruction. The facility was decorated for Christmas with two trees in the living room. The kitchen was clean and well organized but there were several insects found. Licensee has contracted a pest control company and set up non-toxic traps to eradicate the problem. Toxins and sharps for the kitchen are stored and locked in laundry room. There was an adequate supply of perishable and non-perishable food, which was stored appropriately and dated and bug-free. Medications were stored in locked medication closet making them inaccessible to residents and staff that do not handle medications. All bathrooms had grab bars, and non-slip mat/flooring for bathing as needed. There was a sufficient supply of hygiene products, cleaners, and paper products for use as needed. Each bathroom had soap and paper towels. Facility has a sufficient supply of personal protective equipment (PPE). Facility has an Infection Control Plan. The fire extinguisher was serviced on 09/23/2024 and was fully charged. The carbon monoxide detector was tested and operational. The facility has a central fire alarm system. Fire drills are done monthly; the last one was on 11/15/2024. The water temperature was 117.1 degrees F. LPA reviewed 4 of 4 resident files and found them to be complete. One resident's updated physician's report was not in the file. Administrator to submit to LPA by 12/17/2024. Personnel files will be reviewed at a later date. There were two caregivers at the facility at the time of inspection. LPA toured the facility with carestaff and residents on 11/03/2023 at approximately 3:15 PM. Facility was found to be clean, orderly, and at a comfortable temperature with all exits free from obstruction. Toxins and sharps for the kitchen are stored and locked in laundry room. There was adequate dry food for at least a week and fresh foods for at least 2 days. All food was stored appropriately and dated. Medications were stored in locked medication closet making them inaccessible to residents and staff that do not handle medications. All bathrooms had grab bars, and non-slip mat/flooring for bathing as needed. There was a sufficient supply of hygiene products, cleaners, and paper products for use as needed. Each bathroom had soap and paper towels. All postings were up and visible as required. Facility has a sufficient supply of personal protective equipment (PPE). Facility has an Infection Control Plan. The fire extinguisher was serviced on October 16, 2023. The carbon monoxide detector was tested and operational. The facility has a central fire alarm system. The last Fire Drill was on 10/02/2023. The water temperature was 115.3 F. Residents were clean and appropriately dressed. No deficiencies during today's inspection.
