Type of visit: Required - 1 Year

On 06/11/26 Licensing Program Analyst (LPA’s) J. Leffall and A. Porter arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Ke’mani Curry. 0 residents were present during inspection as they are attending day program. Licensee (L1) Ceiara Sparks attended, but had to leave during inspection LPA’s toured facility with S1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at -3 degrees F and refrigerator temperature was maintained at 39 degrees F. Cleaning chemicals was observed stored and locked under kitchen sink. Medications were checked and observed kept locked in kitchen shelves. LPA observed MARs and samples of medications. Sharps were observed stored and locked in kitchen shelf. Fire extinguisher was observed with a service date of: 4/24/26. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 115.8 to 120 in 2 bathrooms. Washer and dryer observed to be operational. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Carbon monoxide and smoke detectors were tested and observed to be operational. All residents’ file and a samples of staff files were reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. LPA is requesting the following documents be submitted to the Fresno CCL office by 6/25/26: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E), Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020) A copy of this report was provided to Staff, whose signature on this form confirms receipt of this report.the state’s words, verbatim · CDSS document, Jun 11, 2026

Type of visit: Case Management - Other

On 1/26/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct a Case Management visit. LPA introduced self, stated the purpose of the visit, and was greeted by Licensee (L1) Ceiara Sparks. 0 residents were present during inspection and all 5 residents are attending day program. Licensee departed the facility to transport residents to appointments. Staff (S1 and S2) Kemani Curry and Barbara Andrews proceeded to count all of the resident's monies. The purpose of the visit is to review all P&I monies and personal monies for all residents. LPA reviewed Financial Ledgers and current bank statements for each resident to ensure all monies for each resident is accounted for. After careful observation and review of all resident's monies. LPA observed all monies for all of the residents are accounted for. No deficiencies issued during this inspection. Exit interview conducted. An Office Meeting was conducted at the Regional Office on this date regarding information provided to the Department regarding licensee Ceiara Sparks. The licensee also reported the incident in person at the RO on 1/7/26. Present during the meeting were: Brenda White, Regional Manager See Moua, Licensing Program Manager Jacques Leffall, Licensing Program Analyst Ceiara Sparks, Licensee The following was discussed during the meeting – -Incident that occurred on 1/4/26 -Licensee’s intent regarding the facility Licensee provided the following information – -A statement of what occurred -A copy of the restraining orders the licensee filed against the persons involved in the incident -The incident did not take place at the facility, but her personal home -No residents were involved or present and there was no impact on the services to the residents -Has requested the police report and was informed the police report is not ready -Has a backup Administrator with certificate who can step in -Has started the necessary steps, if needed, for a Change of Ownership and to continue the operations The licensee was informed that the facility will be subject to increased monitoring and additional follow-up may be required. Licensee will provide a statement to ensure the care and safety of the residents. Licensee will provide the police report once available. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 37 degrees F. Cleaning chemicals was observed stored and locked under kitchen sink. Medications were checked and observed kept locked in kitchen shelves. LPA observed MARs and samples of medications. Sharps were observed stored and locked in kitchen shelf. Fire extinguisher was observed with a service date of: 6/22/24. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at 114.4 in 1 bathroom. Washer and dryer observed to be operational. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Carbon monoxide and smoke detectors were tested and observed to be operational. LPAs introduced selves, stated the purpose of the visit, and was greeted by Licensee Ceira Sparks granted entry. No clients were present during inspection. Staff and clients’ files were reviewed and observed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 08/31/24: Lic 308, Lic 500, Lic 610D, and Lic 9020. LPA received a copy of current Administrator certificate. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Licensee, Ceiara Sparks, Continual Administrator's Certification expired 03/27/2023. Licensee provided emails from Administrators Certification Bureau acknowledging she has submitted her ECU and paid the required fees. There are currently 6 residents who reside at this home and there is 0 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested at 110 degrees. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA Hurt will return at a later date to review staff and client files. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file.
