Type of visit: Required - 1 Year
On 01/10/26 Licensing Program Analyst arrived at the facility to complete an unannounced annual visit. LPA met with Caregiver, Jessica Velasquez, explained reason for visit and was permitted entry into the facility. Administrator, Taelor OConnor arrived a short time later. LPA completed a tour of the facility. A health and safety check was completed on residents in care. There were 5 residents present during todays visit. 3 of 5 residents are receiving hospice services. Pathways and doors were clear and free from obstruction. Facility was without odor. Common areas were adequately furnished, and adequately lit. Smoke detectors and carbon monoxide detectors present and operational at time of visit. Fire extinguisher last serviced on 4/29/25. Last fire drill on conducted on 10/25/25. Resident rooms observed to have the required furnishings and with adequate lighting. Sharps, chemicals and medications were located in locked cabinets/closets and cupboards. LPA observed sufficient seating under covered patio areas. The following issues were observed during today’s visit: 3 of 3 residents receiving hospice services does not have a hospice care plan. 1 of 3 resident files reviewed does not have a current medical assessment completed within the last year. Medication observed present without prescription. Deficiencies cited per California Code of Regulations, Title 22. Deficiencies are being cited on the attached 809D. If not corrected, the violation with have a direct risk to the health, safety and/or personal rights of residents in care. LPA requested the following documents to be submitted to CCL by 01/16/2026: current copy of Administrator’s Certificate, Administrator Organization (LIC 309), Designation of Administrative Responsibility (LIC 308), Emergency Disaster Plan (LIC 610-E), Personnel Report (LIC 500), Register of Facility Clients/Residents (LIC 9020) and a current copy of liability insurance in order to update the facility file. Exit interview was conducted with Administrator, Taelor. A plan of correction was developed by Administrator and reviewed by LPA. Type of visit: Prelicensing
On 1/14/25 Licensing Program Analyst (LPA) M. Garza arrived to complete a pre-licensing visit. LPA met with Licensee/Administrator, Taelor OConnor explained reason for visit and was permitted entry into the facility. Facility is in the process of pre-licensing. The facility currently has 5 residents with private rooms residing in home. Pre-Licensing is being completed for new owner. Facility currently has 3 residents on hospice. Tour of facility inside and out was completed. Common areas observed with adequately furnishings and lighting. LPA observed an extra supply of linens and personal hygiene/grooming products. Kitchen observed with dishes, plates and utensils. Cleaning supplies/chemicals observed in a locked hallway closet. Medications locked in kitchen cabinet. Fire extinguishers present and charged. Purchased on 03-28-24. Fire alarm and carbon monoxide detectors are present and operational at time of visit. Water temperature observed at 118.9 degrees F. Outside of the facility toured. Exits open free of obstruction. Facility has a functioning phone. Phone number is: (559) 925-0878. First aid kits observed with required items. Component III completed during this pre-licensing visit. At this time the facility is ready to be licensed. LPA reached out to CAB regarding name of facility being incorrect. On 1/2/25 Licensing Program Analyst (LPA) M. Garza arrived to complete an announced visit with Licensees, Patrick & Taelor O'Connor. At the time of arrival LPA was informed that the facility was not ready to be pre-licensed. This pre-licensing is a Change in Ownership. Currently there are 5 residents in care. 3 on currently receiving Hospice services. Facility Pre-Licensing was rescheduled for January 14, 2025 @10 am. No deficiencies cited during todays inspection. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of the following areas: 1.
