Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a required 1 year annual inspection. LPA met with Licensee Cathy Marvos and Administrator Liza Seguban and explained the purpose of the visit. LPA, Licensee and Administrator conducted a tour of the facility. Areas toured include but not limited to resident bedrooms, bathrooms, kitchen, common areas and garage . LPA observed required furniture, and lighting throughout the residents' bedrooms and facility. LPA observed food supplies of non-perishables for a minimum of seven (7) days and perishable foods for a minimum of two (2) days. Toxins, cleaning supplies and knives are locked and inaccessible to residents in care. Fire extinguisher was last inspected on 08/14/2025. LPA observed fire detectors and carbon monoxide detectors to be operable. LPA observed drill logs which are being conducted quarterly. LPA observed required Licensing posters posted throughout the facility. LPA reviewed four (4) resident files. Resident files contain signed admission agreements, physician's reports, appraisals, identification sheets, releases, and resident's rights. LPA reviewed four (4) staff files. A review of staff records indicates that all facility staff has received criminal record clearances and/or are associated to this facility. No deficiencies are being cited as a result of todays inspection. Type of visit: Post Licensing
On 01/08/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a post licensing inspection. LPA met with Administrator Liza Seguban and explained the purpose of the visit. Today's census is three (3) residents with two (2) resident on hospice services. Facility is licensed for six (6) residents and hospice waiver of two (2). LPA and Administrator conducted a tour of the interior and exterior of the facility to ensure the health and safety of residents in care. Areas toured include but not limited to resident bedrooms, bathrooms, kitchen, dinning room, garage and common areas. LPA observed sharps, toxins and medication to be locked and secured. LPA observed the facility to have two (2) days of perishable and seven (7) days of nonperishable foods. LPA observed fire extinguisher to be last serviced on 08/20/24. LPA and Administrator completed the post-licensing inspection tool and facility was found to be in compliance. LPA conducted a file review of three (3) residents files and (2) personnel files. Type of visit: Prelicensing
On 09/26/24 Licensing Program Analysts (LPAs) Cheyenne Ratajczak and Graham Gunby arrived at the facility announced and met with Applicant Cathy Marvos to conduct a Pre-licensing visit. The facility has an approved fire clearance for six (6) non ambulatory residents. During today's inspection, LPAs and Applicant conducted a tour of the interior and exterior of the facility to ensure there is no immediate health, safety and personal rights violation. LPAs conducted a tour of three (3) shared bedrooms, two (2) bathroom, laundry room, kitchen, garage, backyard, and common areas. LPAs observed resident bedrooms to have the proper furniture. LPAs observed the facility to have a proper storage for medications, toxins and sharps which is locked and inaccessible. LPAs observed facility to have ample supply of personal hygiene, linen, and non-perishable foods. LPAs observed the exterior of the facility to be free of obstruction. LPAs observed the facility to have the required posters of Community Care Licensing. LPAs observed fire extinguisher serviced on 8/20/24. The temperature in the facility was 73 degrees Fahrenheit during the time of visit LPAs and Applicant completed the inspection tool and Comp III. Pre-Licensing completed and no deficiencies was observed. LPAs will forward findings to the Centralized Application Bureau (CAB) that facility met all the pre-licensing components. Applicant has satisfied all requirements in accordance to Title 22, California Code of Regulations on today's pre-licensing inspection. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22. Signed LIC 809 with copy of photo ID have been obtained. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of following areas: 1.
