Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. The Administrator, Cybele Magtibay has an Administrator's certificate that expires on September 27, 2026. The facility is a single story home with 8 bedrooms, 6 bathrooms, an office, living/dining room, kitchen, and an attached 3 car garage. Garage is being used for storage of food and supplies and is inaccessible to residents. Two of the bedrooms are for caregivers. Facility is licensed for 6 non-ambulatory residents and a hospice waiver for 6. LPA met with the Administrator and explained the reason for the visit. LPA and Administrator toured the facility. LPA observed the See Something, Say Something Poster (PUB 475) posted next to the front door of the facility. LPA observed the required postings on the wall of the kitchen, visible from the dining room. LPA observed the fire extinguisher in the kitchen and resident hallway are fully charged. LPA observed a two day perishable and a seven day non-perishable food supply in the kitchen. The 4 burner gas stove lights unassisted. Knives and sharp objects are kept locked in a kitchen drawer. LPA observed the kitchen is clean and organized. Medications are kept locked in a kitchen cabinet. LPA observed the garage is kept locked and used for storage. LPA observed emergency food and water (3 day supply) stored in the garage. LPA observed old furniture and supplies stored in the garage. LPA observed all resident rooms have the required furnishings and linens. Smoke detectors/carbon monoxide detectors tested operational. LPA observed all bathrooms are clean and operational. Hot water measured 114.2 degrees Fahrenheit in bathroom 1. The last fire drill was conducted on December 15, 2025. LPA observed the backyard has no bodies of water. The exit gate is operational. There is a table with 4 chairs and an umbrella for residents to sit outside. No obstacles or hazards observed in the backyard. LPA reviewed 6 resident files and medications, no discrepancies observed. LPA reviewed 2 staff files, both staff members have the required training including CPR training. Both staff members are background cleared and associated to the facility. LPA inspected the First Aid kit and it has all the required elements. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Cristina Valerio and explained the reason for the visit. Cristina Valerio's Administrator's certificate expires on September 1, 2026. The facility is a single story home with 8 bedrooms, 6 bathrooms, an office, living/dining room, kitchen, and an attached 3 car garage. Garage is being used for storage of food and supplies and is inaccessible to residents. Two of the bedrooms are for caregivers. Facility is licensed for 6 non-ambulatory residents and a hospice waiver for 6. LPA and Administrator toured the facility. LPA observed the See Something, Say Something sign posted in the main entry way of the facility. LPA observed the kitchen is clean and organized. The 4 burner gas stove lights unassisted. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed a 3 day emergency supply of food and water in the garage. LPA observed medications are kept locked in the kitchen pantry. LPA observed the first aid kit has all the required elements. LPA toured the resident bedrooms and bathrooms. Smoke detectors/carbon monoxide detectors tested operational. The last emergency drill was conducted November 21, 2024. LPA observed all resident bathrooms are clean and operational. Hot water measured 115.1 degrees Fahrenheit in all bathrooms. LPA observed all resident rooms had the required furnishings. LPA toured the backyard. The backyard has a seating area with an umbrella and chairs for the residents. There is a small fountain in the backyard. Both exit gates are operational. LPA reviewed staff files. All staff members are background cleared and associated to the facility. All staff members have the required training including CPR training. LPA reviewed 6 resident records and medications. No discrepancies observed. No discrepancies are being cited as a result of this visit. LPA consulted with the Administrator concerning reporting requirements. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator Christina Valerio and explained the reason for the visit. Christina Valerio's Administrator's certificate expires on 9/01/2024. The facility is a single story home with 8 bedrooms (2 are for staff only), 6 bathrooms, an office, living/dining room, kitchen, and an attached 3 car garage. LPA and Administrator toured the facility. LPA observed the See Something, Say Something poster (PUB 475) posted in the entry way of the facility. LPA observed all resident rooms have the required furnishings. LPA observed a 2-day perishable and a 7 day non-perishable food supply on hand. The kitchen is clean and organized. LPA observed the refrigerator in the kitchen is operational. LPA observed the stove lights unassisted. Medications and sharp objects are kept secured and inaccessible to residents in the kitchen. The garage is kept locked and used for storage. LPA observed emergency food and water supply stored in the garage. Hot water in all bathrooms measured between 111.0 degrees Fahrenheit and 111.7 degrees Fahrenheit. The carbon monoxide/smoke detectors tested operational. LPA observed all fire extinguishers are fully charged. LPA and Administrator toured the backyard. The backyard has a table with an umbrella and chairs for residents to sit outside. There is a fountain the backyard. The fountain is raised and is five feet tall and poses no risk to residents, The exit gates on each side of the house are operational and self latching. No obstacles or hazards observed inside or outside of the facility. LPA reviewed 4 out of 4 resident files. All of the resident files had the required documents with no deficiencies observed. LPA reviewed all 4 residents medication, no discrepancies observed. LPA reviewed 3 staff files. All staff files had the required documents and no deficiencies observed. LPA inspected the first aid kit, the first aid kit had all the required elements. No citations are being issued as a result of this visit.
