LPA Hou Vang and LPM Shawna Doucette arrived at the facility unannounced to conduct an annual inspection. LPA explained the purpose of the visit and was granted in by Staff Elizabeth Juarez Vela. Staff contacted Licensee/Administrator Sarah Mazibuko who responded to the facility to assist with the visit. A tour of the facility was conducted with the Administrator. The residence was set at 74 F temperature and free of passageway obstructions inside and outside. Facility is clean and free of odors. Kitchen toured, supply of food observed and food stored properly for perishable and nonperishable. Medications were stored in a locked in a closet. Cleaning supplies were locked in a cabinet in the garage. Smoke detectors and carbon monoxide detectors were checked and operating. Fire extinguisher was last serviced 5/31/2026. LPA checked water temperature which measured at 114.2 F. Fire drill was last conducted 7/16/26. Resident, medication and staff records were reviewed. R1's PRN was taken, but was not properly documented and did not indicate time taken and reason for taking the medication. Current first aid and CPR were reviewed and current for staff. Staff contacted Licensee/Administrator Peter and Sarah Mazibuko who responded to the facility to assist with the visit. A tour of the facility was conducted with the Administrator. The residence was set at 73 F temperature and free of passageway obstructions inside and outside. Facility is clean and free of odors. Kitchen toured, supply of food observed and food stored properly for perishable and nonperishable. Medications were stored in a locked in a closet. Cleaning supplies were locked in a cabinet in the garage. Smoke detectors and carbon monoxide detectors were checked and operating. Fire extinguisher was last serviced 5/25. LPA checked water temperature which measured at 109.3 F. Fire drill was last conducted 4/1/25. Resident, medication and staff records were reviewed. R2 has over the counter medications that does not have directions from doctor on how often medication should be administered or the strength of the medication. Current first aid and CPR were reviewed and current for staff. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Shawna Doucette arrived to the facility announced to conduct the Pre licensing visit. LPA Shawna Doucette met with Licensee/Administrator Sarah Mazibuko who granted LPA's entry into the facility. LPA toured facility. Common rooms have adequate furnishings and lighting. All of the resident bedrooms have all the required furnishings and adequate lighting. Hot water temperature in bathrooms measured at 105.6 degrees F. LPA observed a supply of extra bed linens and personal hygiene and grooming products. Kitchen observed to have dishes, plates, utensils ect. Cleaning supplies are stored in a locking cabinet in the garage and under the kitchen sink. Medications are locked in a medication closet. A fire extinguisher is present and has a service date of 03/20/2024. Smoke detectors and carbon monoxide were operating. Outside of the facility toured. Exits open free of obstruction. The residence has a delay egress which sounds the indoor alarm. No outside hazards were observed. No pools or bodies of water. All required postings are posted. Facility phone number will be (661) 556-4752. Component III was conducted during pre-licensing visit with Applicants. I have found that applicant has met all pre licensing requirements.
