On July 10, 2026, Licensing Program Analyst (LPA) Eboni Bentley arrived unannounced for the purposes of conducting a required 1-Year annual visit using the CARE Inspection Tool. LPA Bentley was greeted and granted entry into the facility by staff, after stating the reason for the visit. Administrator (AD) Chin-Wen "Megan" Cheng arrived shortly to assist with the visit. Administrator Chin-Wen Cheng has an administrator certificate that expires on September 16, 2027. The facility is licensed to operate age 60 and over for (6) non-ambulatory only residents, with a Hospice waiver for three (3). The building is a single story structure located in a residential neighborhood, which consists of the following: five (5) resident bedrooms, one (1) staff bedroom, three (3) bathrooms, living area, dining area, kitchen, an outdoor covered seating area, and an attached two car garage. The facility currently has a census of six (6) residents in care with one receiving hospice services. LPA Bentley toured the inside and outside of the physical plant with AD Cheng. All rooms were inspected and the facility was observed to be appropriately furnished at the time of visit. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Additional linens, comforters, and bath towels were adequately stocked and available. Bathrooms were found to be clean and operational with water temperatures measured between 105 degrees F to 111.7 degrees F. The kitchen was observed clean, all appliances were operational, and there was a two-day supply of perishable and seven-day supply of non-perishable food available. Toxins, disinfectants, sharps, and medications were secured. The backyard was observed with a shaded seating area for residents and the facility has one exit gate that was operational. CONTINUE TO LIC809-C...... The smoke alarms and carbon monoxide detectors were operable. An emergency safety drill was last conducted on April 9, 2026 and are conducted quarterly. Emergency food, emergency water, and emergency supplies were stored in the garage and in a bench in the backyard. The facility has two (2) fire extinguishers that were charged, mounted, and last serviced on December 12, 2025. First aid kit is maintained and contains all the necessary elements. A working telephone (714-894-2835) remains available and the facility has a device that can be used for teleconference purposes. Liability Insurance is effective December 24, 2025 and expires on December 24, 2026. LPA Bentley conducted an audit of six (6) resident files (R1-R6), four (4) staff files (S1-S4), and medication and medication administration records review. Interviews were conducted with staff and residents. Based on today’s observations, deficiencies are being cited during the visit, per Title 22, Division 6, Chapter 8 of the California Code of Regulations. Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1. During today’s visit, LPA met with Chin-Wen "Megan" Chen, Administrator (AD). The facility is a five bedroom, one office room and three bathroom single story home with an approved fire clearance of six non-ambulatory residents of which three may be on hospice. The facility currently has a census of four residents in care with one receiving hospice services. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in two of two resident bathrooms, and testing auditory devices on all exits. The hot water temperature measured between 108.5 and 114.2 degrees Fahrenheit and all smoke detectors were operational. The fire extinguisher is charged and was serviced on June 27, 2025. The facility’s last fire drill was conducted on July 9, 2025. The facility has video surveillance in common areas and all resident Admissions Agreements are signed, giving permission for use. The cameras do not have audio. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. Sharps and knives were secured in a locked drawer. All appliances were in working order and chemical/cleaning supplies were secured under the sink. Kitchen was clean and no odors were detected. Washer and dryer were observed in garage with additional emergency food supplies. (Continued on LIC 809-C) (Continued from LIC 809) The exterior of the facility had artificial turf with a mini-putting area, plants and a shaded seating area. Exterior gate was self-latching and in working order. There is plenty of outdoor space for activities. LPA observed board games, books and a tactile activity dartboard. LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. The First Aid Kit had all the required elements and a First Aid Manual, by the American Red Cross. The PUB 475 poster, Theft and Loss and Resident Council information were all posted in the common hallway. LPA reviewed three of three staff training and fingerprint records and conducted a complete review of resident records. Staff had required hours for training and bed rail orders were on file for three residents. LPA interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPA confirmed that administrator has a current administrator certificate which expires on September 16, 2025. Based on the observations made during today’s visit, the facility appears to be in compliance with Title 22 Division 6 of the California Code of Regulations, no deficiencies cited on this date. On 9/11/2024, Licensing Program Analyst's (LPA's) Jenifer Tirre and William Venagas conducted an unannounced required visit using the CARE Inspection Tool. LPA's was greeted by staff and granted entry after stating the purpose of the visit. Administrator (Admin) Megan Chen was present to assist with the facility inspection on today's date. The facility is licensed for six (6) non-ambulatory residents with approved hospice waiver for three (3) residents. Currently, there are two (2) Hospice residents present during today’s visit. This is a single story with a two-car garage facility. The facility has four resident bedrooms (Two shared, two private), one staff room, one staff office, and three bathrooms. At around 2:00, LPA's conducted a tour of the physical plant accompanied by Administrator, and the following was observed: There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the client's personal belongings was observed. Bed linens, comforters, and bath towels were available during the visit. Bathrooms were operational with water temperature measured at 111.2 degrees F. LPA's observed the facility to be furnished at the time of the visit. Storage areas for personal hygiene and sharps objects were stored and not accessible to residents. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. Facility has two fire extinguishers that are mounted and charged. A review of the Medication Records Administration (MAR) was conducted, and LPA observed the records are in compliance. CONTINUED ON 809C During the visit, LPA's observed the facility's infection control practices. LPA's observed screening protocols for visitors, staff, and residents, and sanitizing stations in common areas and restrooms. LPA observed the facility has a supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. LPA observed First Aid Kit was maintained. A working landline phone was operational. The last fire drill was conducted on 9/4/2024. The facility had operational smoke and carbon monoxide in bedrooms and common areas. The facility has current liability insurance on file effective 12-24-23 - 12-24-24. The facility is current on Community Care Licensing annual dues. A review of five residents (R1-R5) service files and two staff (S1-S2) personnel files revealed to be complete. The facility has the current administrator's certification on file for Megan Chen # 6037769740 - Expiration 9/16/25 No deficiencies during this inspection visit.
