Type of visit: Case Management - Health Checks

On 05/05/2026, Licensing Program Analyst (LPA) Graham Gunby arrived unannounced at the facility to conduct a case management health check. LPA met with staff Darius Magda and explained the purpose of the visit. LPAs conducted a visit to this facility today to follow up on R1 who was relocated to this facility on 05/04/2026. R1 was relocated due to a Temporary Suspension Order issued at another licensed facility by the Department. LPA spoke to R1 who indicated that they adjusting to the facility as best as can be expected with the sudden change. No deficiencies are cited during today’s visit. Licensing Program Analyst (LPA) Graham Gunby arrived at the facility unannounced to conduct a Required-1 Year Inspection utilizing the inspection tool. LPA met with Administrator, Mary Madga and explained the purpose of the visit. LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured include but not limited to resident bedrooms, bathrooms, kitchen, common areas and backyard. LPA observed required furniture, and lighting throughout the residents' bedrooms and facility. LPA observed residents' bathrooms to be clean, sanitary, and in good repair. LPA observed food supplies of non-perishables for a minimum of seven (7) days and perishable foods for a minimum of two (2) days. Toxins and cleaning supplies are locked and inaccessible to residents in care. The hot water temperature was measured in the kitchen sink at 120.1 degrees Fahrenheit. LPA observed fire detectors and carbon monoxide alarms to be operable. The fire extinguisher is located in the kitchen and was last serviced on 03/20/2026. LPA observed required Licensing posters posted in the entry of the facility. LPA observed medications and sharps to be locked and inaccessible to residents in care. LPA reviewed three (3) resident files. Resident files contained all required documentation. LPA reviewed three (3) staff records. Staff have all required documentation and first aid training. No deficiencies are being cited during today's inspection. LPA Hiratsuka conducted this unannounced annual visit. Caregivers Nakita Johnson and Raisa Drokina were present. Back-up Administrator Daniel Magda, arrived during visit. LPA toured facility with administrator to ensure health and safety of residents in care. LPA toured four resident rooms, one staff room, two bathrooms, kitchen, common living spaces, backyard and the garage area. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA toured the backyard and all exits are accessible and unlocked. There is a locked storage for medications and toxins. Food supply is adequate for 2-day perishable and 7-day nonperishable. LPA reviewed two of five resident files and two staff files. LPA reviewed medications of two residents comparing with Centrally Stored Medication Record and physician orders. A review of staff records indicates that all facility staff has received criminal record clearances and/or are associated to this facility and required training. Multiple topics were discussed. On 4/25/2024 LPA Tryon visited the facility to do an annual visit. LPA met with Mary Magda. LPA toured the house including common areas, kitchen, bedrooms, hallways, bathrooms, medication storage, food storage, garage, and yard. The home appears to be clean and nicely furnished. Smoke and carbon monoxide detectors installed and functioning. Fire extinguishers present and charged. Food supplies appear appropriate to meet the requirement of 2 days perishable and 7 days non-perishable. Food appears varied and fresh. Medications are centrally stored and locked. Bathrooms are in good condition, fixtures new, clean and functional. LPA reviewed the CARE Tool with licensee. LPA reviewed 2 resident files and 2 staff files and completed review forms. Files contain appropriate documentation. LPA interviewed 2 residents and completed interview forms. LPA completed staff interview form with Mary Magda. Administrator has completed all course work for the 2 year period and and submitted documentation and payment for renewed Admin. Certificate. Administrator is waiting for the CCL Administrator Certification Unit to review documentation and renew certificate. Proof of submission was shown during visit. At this time, the home appears to be in substantial compliance with the regulations. No deficiencies were cited at this visit.
