Type of visit: Required - 1 Year

On 9/15//2026, LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Executive Director Samuel Faye and Health Services Director Adrienne Fair and explained the purpose of the visit. LPA toured the facility including resident’s bedrooms and common areas. Facility found to be clean and at a comfortable temperature with all exits free from obstruction. No accessible bodies of water or fire safety hazards observed. Hot water was tested at 110 degF. All personal belongings of residents are intact. Facility has sprinkler system. All fire extinguishers have been checked and current. Food supply is replenished constantly throughout the week and stored properly. Cleaning supplies and other toxins are safely stored in locked closets throughout each floor, and housekeeping carts all of which were secured upon inspection. Five resident records and five staff records were reviewed. Resident records are updated, complete and signed. Staff records are complete, with training logs. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. No deficiencies are cited at this time. Type of visit: Case Management - Incident

On 10/09/2025, Licensing Program Analyst (LPA) Yi Sam Jian conducted an unannounced case management visit to follow up on an incident report that was submitted to CCLD on 09/25/2025. LPA met with Assistant Health Care Director, Derrick Nguyen, and Community Director, Sam Faye, arrived later during the visit. LPA explained the purpose of the visit. The facility submitted a written incident report indicating that on 09/25/2025, at approximately 10:37 AM, Resident R1 pushed resident R2, causing R2 to fall backwards onto floor. On 9/8/2025, Licensing Program Analyst (LPA) Grace Donato conducted an unannounced Annual Required – 1 yr. inspection for this facility. LPA met with Administrator, Adrienne Fair and explained the purpose of the visit. LPA continued with a tour of the facility with staff, facility found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. Fire Extinguishers located on each resident floor, kitchen and common spaces were found to be charged. Smoke and carbon monoxide detectors were present. There was a sufficient supply of both perishable and nonperishable foods. Food supply is replenished constantly throughout the week and stored properly. Cleaning supplies and other toxins are safely stored in locked closets throughout each floor, and housekeeping carts all of which were secured upon inspection. All resident’s bedrooms have lighting & appropriate furnishings and bedding items. Residents are doing activities, eating in the dining area and meeting with visitors. Residents are encouraged to participate and showcase in art exhibits and displays for resident engagement. Ten resident records and ten staff records were reviewed. Resident records are updated, complete and signed. Staff records are complete, with training logs. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. No deficiencies cited today. Type of visit: Required - 1 Year

On 9/18/2024, Licensing Program Analyst (LPA) Tobola conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by Health Services Manager, Samantha Curro & Assistant Executive Director, Adrienne Fair. The facility currently provides care for 148 residents, 10 of which are receiving hospice services, along with a designated memory care unit. LPA continued with a tour of the facility with staff, facility found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. Fire Extinguishers located on each resident floor, kitchen and common spaces were found to be charged. Smoke and carbon monoxide detectors were present with fire safety Inspection completed in July 2024 indicating all fire safety devices and systems are in order. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations, with food stored in the kitchen, sufficient for residents in care. Food supply is replenished constantly throughout the week and stored properly. Facility provides a wide variety of meal preferences and preparation while also ensuring proper dietary restrictions are followed. Cleaning supplies and other toxins are safely stored in locked closets throughout each floor, and housekeeping carts all of which were secured upon inspection. There was a supply of hygiene products and paper products available for residents. All resident’s bedrooms have lighting & appropriate furnishings and bedding items. Residents that were out in the community the inspection were observed interacting with staff, fellow residents and visitors in the common areas, or in their bedrooms resting. The facility encourages regular family visits and utilizes a wide variety of activities with LPA observing staff engaging continuously with residents, offering activities based on individualized preferences and abilities. LPA found that staff and resident engagement is well practiced with activity calendars developed on a monthly basis. Residents are encouraged to participate and showcase in art exhibits and displays for resident engagement. Residents were observed to have a positive and personable relationship with staff and Assistant Executive Director. There is a single outdoor patios for resident use, all equipped with appropriate shading Continued onto LIC809-C LPA conducted a sample file review for residents and found all items to be in order including needs & service plans and physician's reports. Upon a spot check of staff files, LPA found that caregiver staff have current 1st aid and CPR and annual training on track for completion. In addition, newly hired staff were found to have appropriate health screening and tuberculosis results on file. LPA toured facility bus and found observed first aid kit and charged fire extinguisher to be present. Lastly, A spot check of medications including narcotic was conducted and found that all medication counts and records are in order. Adrienne Fair's Administrator Certificate 7008764740 is currently active through 11/26/2025. Feb 15, 2024Complaint investigation reportUnsubstantiated

Allegation investigated: - Facility staff caused bruising to resident - Facility staff did not notify resident's physician of injury - Facility staff did not notify resident's family of injury - Resident sustained multiple falls due to lack of supervision - Facility staff sexually abused resident - Resident sustained a head injury due to lack of supervision

On 02/15/2024, Licesning Program Analyst (LPA) Jaime Vado conducted an unannounced complaint investigation visit to deliver the findings regarding the allegations above. LPA met with Adrienne Fair and explained the purpose of today's visit. The Department investigated the allegations recieved and discovered that in regards to the resident being sexually abused, it was discovered that during resident transfers R1 would constantly cover her "private area" and would keep her legs close shut when staff would change her and transfer her and the bruisng would be caused where her hands were located during those transfers. No other residents reported abuse, mistreatment, or any complaints regarding the facility or staff. Continued on next page... Unsubstantiated Page 2 - LIC9099 Regarding resident sustaining a head injury, it was reported that on 09/19/2023, R1 sustained a witnessed fall from her wheelchair in an activity room. Staff did an assessment of R1 and sent R1 to the hospital for evaluation. Medical record from the hospital reflected that R1 did suffer an injury, but based on interviews R1 leaned too far forward and fell out of the wheelchair hitting their head. Staff were unable to get to R1 fast enough to prevent her from falling. As a result R1 obtained a new cushion for their wheelchair that prevents R1 from being able to rock or lean forward far enough to fall out again. In regards to other bruising of the resident, through interviews it was discovered that staff technique in rolling and positioning the resident may have caused bruising accidentally by pulling the clothes or sheets to help position the resident. There was no other evidence of abuse taking place that would intend it was done purposely. According to interviews, and hospital records, the physician of R1 was notified of injuries when they occurred as well as the facility notifying the responsible party of R1. These allegations are unsubstantiated. Based on these observations, the above allegations are UNSUBSTANTIATED. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the above allegations are unsubstantiated at this time.the state’s words, verbatim · CDSS document, Feb 15, 2024 · control 14-AS-20231004150448
