Type of visit: Required - 1 Year
On 5/19/26, 8:15 am, Licensing Program Analyst, (LPA) Raymond Comer, conducted an unannounced annual inspection visit to the facility. LPA met with the Administrator and reason for the visit was discussed. Facility is licensed as a single-story residence, six (6) non-Ambulatory, of which, three (3) may be bedridden. Hospice waiver for six (6). Facility has six (6) private resident bedrooms and four (4) bathrooms. At 8:25 am, LPA conducted a tour of the physical plant with the Administrator and observed the following: Physical plant On 8:35 am, plant was inspected for cleanliness and condition. Facility’s main door is the primary entry/exit access. Screening area is located immediately upon entrance. Visitor Sign-in sheet, hand sanitizer, gloves and masks are available. Hand washing, coughing etiquette, and other necessary signage are posted in the bathroom and throughout the facility. Room temperature is comfortable; wall thermostat displays a setting of 76.0°F., within the required range. Side door is located west of house which exits to the back yard. Required postings are prominently displayed and observed to be current. Disaster drills were last conducted on 3/15/2026. Fire Detection/Protection system is present at facility. Multiple smoke/carbon monoxide detectors are installed, hardwired and interconnected. Detectors were tested and function properly. Two (2) fire extinguishers, located in Kitchen and Hallway, are fully charged, with inspection service date: 05/17/2026. [LIC 9099-C] Continued- KITCHEN area is clean and clear of clutter. At 8:50 am, LPA observed refrigerator, microwave, stove/oven, dishwasher and sink to be operational. Knives/Sharps are stored in a locked top kitchen drawer inaccessible to residents. Plates, cups, utensils, and two-day supply of perishable food is properly stored and labeled. A seven day supply of nonperishable food is located in detached garage and properly stored. Dish Soap, cleaning solutions, and toxins are stored in locked lower cabinet underneath the kitchen sink. Medications are stored in a secured medications cart in dining room area and are inaccessible to residents. A First Aid kit is accessible and stored in a dining room area cabinet. Medications are listed in centrally stored medication cart and destruction record log. Laundry area is located and secured in hallway storage compartment near the kitchen. Laundry soaps and other cleaning agents are stored and inaccessible to residents. Linen storage observed to have adequate supply of linen and towels. Bedrooms are observed as clean with sufficient lighting, properly furnished with bedding, linens, at least one chair, and night stand. Bathrooms At 9:20 am, LPA observed bathrooms to be clean and sanitary with necessary supplies and required safety fixtures (grab bars, anti-slip floor stripping). Hot water temperature measured at 116.5°F. Within the required range. Garage is detached from house and observed to be locked and inaccessible to residents. Garage is also used as storage for water, PPE supplies, etc. Outdoor (backyard) area observed to have a shaded patio, with table, and sufficient seating for residents and visitors. Outdoor furniture observed to be in good condition. All trash cans were observed to be covered; there are no bodies of water in the facility. Resident records: LPA observed records stored in a locked and secured hallway closet, inaccessible to residents. Resident files were reviewed for current IPP and/or Needs and Services plans, physician report, and admission agreements. Resident records appeared to be current and complete. [LIC 9099-C] Continued- Staff records: At 11:15 am, LPA observed records are secured and inaccessible to residents. Criminal record clearances were present, and Staff are associated to this facility. Staff records appear to be complete and current. There were no health and safety hazards observed during the inspection. Type of visit: Case Management - Health Checks
At approximately 11:00 a.m. on 04/08/26 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit. LPA met with the assistant administrator and disclosed the reason for the visit. Today’s case management visit was conducted to ensure the health and safety of all residents. LPA toured the facility inside and out at 11:05 a.m. today, interviewed six (06) out of six (06) residents between 11:00 a.m. and 11:30 a.m., and requested pertinent records, including but not limited to admission agreements, medical assessments, care plans, and staff and client rosters at 3:00 p.m. No immediate health or safety concerns were observed during today’s visit. On Thursday, 12/11/25, 12:15 pm, Licensing Program Analyst, (LPA) Ray Comer, arrived for an unannounced case management visit to conduct health and safety observations. LPA met with Administrator, and reason for the visit was discussed. During the visit, LPA received the following documentation: 1. LIC 500 (Personal Roster) 2. Resident Roster During this visit, LPA conducted a tour of the facility, and interviewed resident and staff. Two (2) facility staff and two (2) private caregivers were observed providing care and supervision to residents. Staffers were observed assisting residents with eating. Two home health/private caregivers were also present at the facility providing assistance to residents in (Room#1) and (Room#7) Residents were observed to be comfortable watching television in their room, eating lunch, or receiving caregiver assistance. Two (2) facility caregiver staff were observed providing care and supervision to residents. Caregivers were conversing with residents in the living room and doing checks on residents in their bedrooms. Two (2) Home Health staff were present assisting their assigned residents. On Friday, 09/19/25, 8:30 am, Licensing Program Analyst, (LPA) Ray Comer, arrived for an unannounced case management visit to conduct health and safety observations. LPA met with Administrator designee, Kevin Moscoso, and reason for the visit was discussed. During the visit, LPA received the following documentation: 1. LIC 500 (Personal Roster) 2. Resident Roster During this visit, LPA conducted a tour of the facility, and interviewed residents and staff. Two (2) staff were observed providing care and supervision to residents. One (1) maintenance staff was observed completing scheduled maintenance tasks (Minor interior wall patch/paint work) at the facility. Residents were observed to be comfortable watching television, eating breakfast, or resting in their room. No health and safety issues observed. On 09/04/25, at 10:00 am, Licensing Program Analyst, (LPA) Ray Comer, arrived for an unannounced case management visit to conduct a facility health and safety observation. LPA met with the Administrator, and reason for the visit was discussed. During the visit, LPA received the following documentation: 1. LIC 500 (Personal Roster) 2. Resident Roster 3. Resident POA /Conservator contact Information 4. Resident Admission Agreements (which include rent payment source information) Physical plant inspection: Facility is licensed as a single-story residence, six (6) non-Ambulatory, of which, three (3) may be bedridden. Hospice waiver for six (6). Facility has six (6) private resident bedrooms, and four (4) bathrooms. Three (3) residents are receiving hospice services at this time. During this visit, LPA conducted a tour of the facility, and completed interviews with staff and residents. LPA also observed the facility's medication cart, and conducted an audit of resident medications, and associated records; Facility uses "Yardi EHR" software system. Type of visit: Required - 1 Year
On 06/26/25, 10:00 am, Licensing Program Analyst, (LPA) Raymond Comer, conducted an unannounced annual inspection visit to the facility. LPA met with the Administrator and reason for the visit was discussed. Facility is licensed as a single-story residence, six (6) non-Ambulatory, of which, three (3) may be bedridden. Hospice waiver for six (6). Facility has six (6) private resident bedrooms and four (4) bathrooms. At 10:15 am, LPA conducted a tour of the physical plant with the Administrator and observed the following: Physical plant was inspected for cleanliness and condition. Facility’s main door is the primary entry/exit access. Screening area is located immediately upon entrance. Visitor Sign-in sheet, hand sanitizer, gloves and masks are available. Hand washing, coughing etiquette, and other necessary signage are posted in the bathroom and throughout the facility. Room temperature is comfortable; wall thermostat displays a setting of 76.0°F., within the required range. Side door is located west of house which exits to the back yard. An approved Mitigation and Infection Control plan is on file. Required postings are prominently displayed and observed to be current. Disaster drills were last conducted on 5/25/2025. Fire Detection/Protection system is present at facility. Multiple smoke/carbon monoxide detectors are installed, hardwired and interconnected. Detectors were tested and function properly. Two (2) fire extinguishers, located in Kitchen and Hallway, are fully charged, with inspection service date: 03/04/2025. [LIC 9099-C] Continued- KITCHEN area is clean and clear of clutter. LPA observed refrigerator, microwave, stove/oven, dishwasher and sink to be operational. Knives/Sharps are stored in a locked top kitchen drawer inaccessible to residents. Plates, cups, utensils, and two-day supply of perishable food is properly stored and labeled. A seven day supply of nonperishable food is located in detached garage and properly stored. Dish Soap, cleaning solutions, and toxins are stored in locked lower cabinet underneath the kitchen sink. Medications are stored in a secured medications cart in dining room area and are inaccessible to residents. A First Aid kit is accessible and stored in a dining room area cabinet. Medications are listed in centrally stored medication cart and destruction record log. Laundry area is located and secured in hallway storage compartment near the kitchen. Laundry soaps and other cleaning agents are stored and inaccessible to residents. Linen storage observed to have adequate supply of linen and towels. Bedrooms are observed as clean with sufficient lighting, properly furnished with bedding, linens, at least one chair, and night stand. Bathrooms were observed to be clean and sanitary with necessary supplies and required safety fixtures (grab bars, anti-slip floor stripping). Hot water temperature measured at 113.5°F. Within the required range. Garage is detached from house and observed to be locked and inaccessible to residents. Garage is also used as storage for water, PPE supplies, etc. Outdoor (backyard) area observed to have a shaded patio, with table, and sufficient seating for residents and visitors. Outdoor furniture observed to be in good condition. All trash cans were observed to be covered; there are no bodies of water in the facility. Resident records: LPA observed records stored in a locked and secured hallway closet, inaccessible to residents. Resident files were reviewed for current IPP and/or Needs and Services plans, physician report, and admission agreements. Resident records appeared to be current and complete. Criminal record clearances were present, and Staff are associated to this facility. Staff records appear to be complete and current. Allegation investigated: Staff did not reposition a resident, resulting in an unstageable pressure injury. Staff did not seek timely medical attention for a resident. Staff did not adequately bathe a resident.
Licensing Program Analyst (LPA) Mariana Agban conducted an unannounced subsquent complaint visit to investigate the above stated allegations. LPA met with the Administrator and explained the reason for the visit. LPA conducted a physical plant tour, to ensure the health and safety of the residents are protected and the physical plant is in compliance with Title 22 Regulations. LPA obtained copies of the resident roster and staff roster. Allegation: Staff did not reposition a resident, resulting in an unstageable pressure injury. It is alleged that the R1 developed a pressure injury while living at the facility. It is also alleged that the pressure injury worsened due to the failure of repositioning. LPA obtained hospital records which indicate that on 09/11/23 R1 presented with stage 3 or 4 pressure injury on the coccyx. Interview with Staff 1 (S1) revealed that it was reported to S1 sometime on or around September 8th, 2023 that there was slough coming out of R1's patch foam on the coccyx. R1 received Home Heath services and staff was instructed to reposition R1 every 2 hours. (Conintue on 9099C) Substantiated LPA obtained records of Home Health and of the Staff Daily Log. Documents revealed that Staff didn't reposition R1 every 2 hours as instructed resulting in an unstageable pressure injury. Based on information obtained the allegation is deemed Substantiated at this time. Allegation: Staff did not seek timely medical attention for a resident. It was alleged that staff did not seek timely medical attention for R1. On 09/10/23 R1 was taken to the hospital by a family member and was diagnosed with UTI, sepsis, and pressure injuries. S1 denied the allegation. Interview with S1 revealed that facility staff didn’t know the stage of the wound. S1 stated that there was a miscommunication between home health and facility staff. Interview with S2 revealed that they were unaware of R1's medical condition and that resident's medical conditions are being supervised by S1. Based on information obtained the allegation is deemed Substantiated at this time. Allegation: Staff did not adequately bathe a resident It was alleged that staff did not adequately bathe R1. Interviews with 4 out of 4 staff denied the allegation. Records review indicated that R1 had received a one time shower since their admission at the facility. S2 stated that facility staff failed to document daily showers and thus S2 conducted an in-service training to address the issue. Based on information obtained the allegation is deemed Substantiated at this time. Per California Code of Regulations (CCR), Title 22, see LIC 9099-D for deficiencies cited. An immediate civil penalty of $500 is also assessed. The licensee was informed that a civil penalty might be assessed based on the Health and Safety Code 1569,49(e) or (f), or 1548(e) or (f), 1568.0822(e) or (f). Brian Rosales (The Administrator) arrived at the Facility @ 9;15AM, and reason for the visit was discussed. Facility is licensed as a single-story residence, six (6) non-Ambulatory, of which, three (3) may be bedridden. Hospice waiver for six (6). Facility has six (6) private resident bedrooms and four (4) bathrooms. At 9:45AM, LPA conducted a tour of the physical plant with the Administrator and observed the following: Physical plant was inspected for cleanliness and condition. Facility’s main door is the primary entry/exit access. Screening area is located immediately upon entrance. Visitor Sign-in sheet, hand sanitizer, gloves and masks are available. Covid 19 prevention protocols are posted. Hand washing, coughing etiquette, and other necessary signage are posted in the bathroom and throughout the facility. Room temperature is comfortable; wall thermostat displays a setting of 78.0°F. within the required range. Side door is located west of house, exiting to back yard. All trash cans were observed to be covered. The facility has submitted an approved Mitigation and Infection Control plan. Required postings are prominently displayed and observed to be current at the facility. Disaster drills were last conducted on 4/02/2024. Fire Detection/Protection systems are present at facility. Multiple smoke alarms are installed, hardwired and interconnected. Two (2) Carbon monoxide alarms are installed. Both Smoke and Carbon monoxide detectors were tested and function properly. Fire extinguishers are located in Kitchen area and Hallway areas and are fully charged, with inspection service date: 03/26/2024. Medications are stored in a secured medications cart in dining room area and are inaccessible to residents. A First Aid kit is accessible and stored in a dining room area cabinet. Medications are listed in centrally stored medication cart and destruction record log. Laundry area is located and secured in hallway storage compartment near the kitchen. Laundry soaps and other cleaning agents are stored and inaccessible to residents. Linen storage observed to have adequate supply of linen and towels. Within the required range. Garage is attached to the house and observed to be locked and inaccessible to residents. Garage is also used as storage for frozen foods, extra water and PPE supplies. Outdoor (backyard) area observed to have a shaded patio, with table with sufficient seating for the residents. Outdoor furniture observed to be in good condition. There are no bodies of water in the facility. Resident records were reviewed for current IPP and/or Needs and Services plans, physician report, and admission agreements. Resident records appeared to be complete and current. Staff records were checked. Staff files contain criminal record clearances and are associated to this facility. Staff records appear to be complete and current. There were no immediate health and safety hazards observed during the day of inspection.
