On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to the facility today to conduct a required annual. LPA was greeted and granted entry by staff and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents of which one can be bedridden. Facility has an approved hospice waiver for 4 residents and the home currently has 4 residents. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in bathrooms. The hot water temperature measured between 106.2 and 109.8 degrees Fahrenheit and all smoke detectors were operational. The facility’s last fire drill was conducted on December 1st 2025. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed medication storage in locked kitchen cabinet. LPA observed emergency food in facility pantry and emergency water in staff bedroom. LPA reviewed three of three staff training and fingerprint records. Staff 1-3 (S1-S3) did not have the required training hours .LPA conducted a complete review of resident records. LPA observed Resident 3 (R3) to have half rails without a physician's order. LPA confirmed that administrator has a current administrator certificate which expires on 04/27/2026. Based on the observations made during today’s visit deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Tena Herrera conducted the required annual inspection. LPA arrived unannounced and met with Remigo Villanueva and explained the purpose for today’s visit, shortly after Administrator Wesley Pao arrived to assist with visit. The facility is licensed to serve 6 Non-Ambulatory Residents, 1 of which may be bedridden, and have an approved Hospice Waiver for 4 Residents. The facility currently has 4 Residents. The facility is a single-story home located in Fullerton, Ca. A tour of the facility includes: living area, dining area, kitchen, laundry room, 6 bedrooms (2 are used as staff rooms for now), 2 bathrooms, attached garage, front yard, back yard, and a detached shed (for storage). LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit todays visit and the initial visit and observed the following: Infection Control: Facility has sufficient PPE supplies and Infection Control Plan maintained at the facility. Operational Requirements: The facility has an approved fire clearance, and facility maintains the required liability insurance with the required coverage and expiration date of 6/1/25. Physical Plant & Environment Safety: LPA toured facility, residents’ bedrooms were checked and closet/drawer space to accommodate each resident comfortably was available. The front yard is free of debris/hazards and the outdoor and passageways are free of obstruction. No bodies of water were observed at the facility. There are no security bars or weapons on the premises. Hygiene products are readily available for clients. The hot water temperature was tested throughout the facility and measured within the required range of 105-120 degrees F. All storage areas for cleaning solutions, toxins, knives, and hazardous items are kept locked and are inaccessible to residents. Smoke detectors and carbon monoxide detectors are operable and in compliance. There fire extinguisher was observed and is fully charged. Last fire/disaster/earthquake drill was conducted on 12/16/24. (Continued on LIC809C) Personnel Records-Training: Staff has criminal record clearance, current First-Aid/CPR/AED and all training are documented in personnel files. LPA reviewed 4 staff files with no issues observed. Administrator Wesley Pao certificate expires on 4/26/2026. Staffing: There appears to be sufficient staffing at all times in the facility. Resident Records-Incident Reports: Resident files are kept in a secure location and have the following documents in their files - Pre-admission appraisal/Appraisal Needs & Services Plan, Admission Agreements, Identification & Emergency Information and current Physician's Report. LPA reviewed 4 Resident Files with no issues observed. Residents Rights-Information: Residents are provided with telephone and internet at the facility. The facility has the following posters posted: Residents Rights, Complaint Poster, and Ombudsman. Planned Activities: Facility provides activities and have a variety of activities to choose from within the facility. There is an outdoor activity area available for the residents. Food Service: The kitchen was observed for the ability to prepare and serve food. LPA observed an appropriate food supply of two (2) days of perishables and one week (7 days) of non-perishables. Incidental Medical & Dental: Medication is properly labeled and are centrally stored in a locked cabinet and are in their original containers. LPA reviewed 4 residents’ medications and there were no issues observed. Disaster Preparedness: The facility has an Emergency Disaster Plan with contact numbers and at least 2 relocation sites. Residents with Special Health Needs: There are no bedridden at this time. Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during the visit.
