On August 28, 2026, Licensing Program Analyst (LPA), Jarred Torres, arrived at the facility unannounced to conduct an annual inspection, and met with Administrator, Karin Vaughn. LPA introduced himself and explained the purpose of the visit. A facility file review was conducted at the regional office, and additional records were requested and reviewed at the facility. Their files were stored in a secure location. The facility is licensed for six residents and was operating at a census of six. LPA toured the facility and made observations pertaining to the annual inspection. LPA inspected the facility inside and outside. There were no obstructions to the indoor and outdoor passageways at the time of this visit. During the tour, LPA observed the facility's phone to be operational. The facility's phone number is 951-213-6591. LPA observed the residents' bedrooms which were equipped with the required furniture as stated in Tittle 22 of the California Code of Regulations (CCR). The furniture in all areas of the facility was observed to be in good condition. The facility's appliances were observed to be operational at the time of this visit. Required postings regarding safety and personal rights were posted in the home. Sharp items were kept locked and inaccessible to residents in care. The facility's cooling and heating system is operational and the air temperature was at 75 degrees Fahrenheit (F), and the hot water was within regulations. Caution signs were posted at non-client taps to warn of hot water. Continued on LIC 809-C... LPA observed the medications to be locked and inaccessible to the residents in care. The medication supply was sufficient for their current census, and there were no discrepancies with the centrally stored medications. In the kitchen, the food supply of non-perishable and perishable foods consisted of, but not limited to, frozen meats, vegetables, water, juice, soups, and fruits. The facility goes grocery shopping twice per week to maintain an appropriate food supply for their current census. The facility has a designated area for storing activity supplies, and activities are conducted inside and outdoors. There is a pool on the premises with a gate and signage. Two residents utilize the pool and is safe to do so according to their care plan. Client files and staff files were reviewed and no deficiencies were observed. The administrator had a valid administrator's certificate on file. LPA reviewed the emergency disaster plan and fire clearance. The last emergency drill was conducted on July 31, 2026. The fire alarms and carbon monoxide detectors were tested and operational. The fire extinguisher was in good condition and was inspected on June 30, 2026. Their emergency disaster plan meets the department's standards. LPA inspected the bathrooms and observed the hand washing stations to be stocked with soaps, bath tissue, and single-use hand towels. Furthermore, the bathrooms are free of dust and debris. Additionally, the facility had an approved infection control plan in their files. During this annual inspection, LPA did not observe any health, safety, or personal rights risks to residents in care. Licensing Program Analyst (LPA) Yolanda Delgado arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry by Karin Vaughn. LPA began inspection with introduction, visit purpose and provided the facility Manager with LPA identification and business card. Resident record review began- Five (5) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. This facility is meeting documentation requirements. Employee records review began- Three (3) records were reviewed. LPA reviewed employee record for first aid certification, fingerprint clearance, personnel/job application, health screening and TB test results, criminal record statement, employee rights, training verification, and current administrator certification. CPR and requirements have been met. The facility employs enough staff to maintain cleanliness and meet the needs of the clients in care. Administrator certification is present and current 06/02/2027. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The kitchen area has been under repairs since the end of May and is expected to be completed end of September. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 130.0 degrees F with warnings labels posted. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals in the garage. A locked area is provided for medications inside the laundry room and sharp objects have been temporary moved to the garage. LPA verified there is a telephone working at this location. This home does have a pool and is locked at all times with fencing that is more than 5 ft. high around the perimeter of the pool. Food Service- Food supply meets the of one week supply of nonperishable and 2-day supply of perishables food on hand. Due to the repairs of the kitchen, food supplies are housed inside the garage and staff rooms. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation, and the structure remains unchanged according to the approved floor plan. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Fire extinguishers are tested or replaced annually and were last done so on 6/27/2025. The facility conducting emergency disaster drills quarterly and last done on 7/1/2025. Corporation is active and in good standing. Based on the information received during this visit today, one (1) deficiency is being cited per Title 22, Division 6 of The California Code of Regulations. This facility is not meeting documentation requirements. Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured 127.9 degrees F. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals. All outdoor and indoor passageways are free of obstruction. A locked area is provided for medications and sharp objects. LPA verified there is a telephone working at this location. Food Service- Food supply meets the requirement of one week supply of nonperishable and 2 day supply of perishables food on hand. Foods are dated to assure safety. Type of visit: Case Management - Other

Licensing Program Analyst (LPA) Yolanda Delgado made an unannounced visit to the facility to conduct a Case Management visit in regards to RO receiving a telephone call for a self-reporting of financial abuse by a staff. LPA met with Karin Vaughn, Facility Manager and explained the the nature of the visit. LPA conducted an interview, requested and obtained copies of pertinent documentation, five residents in care with two (2) staff on duty. LPA did not observe any health and safety concerns during the visit. No immediate health and safety codes, sufficient food and working utilities. The facility will be cited for violation of California Code of Regulations, Title 22, Division 6, Chapter 3, Section 1550(c). This poses a health and safety risk to residents in care.
