On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry by staff and explained the reason for the visit. The facility is a single-story building with an approved fire clearance of six non-ambulatory residents, four of which may be on hospice. The facility currently has a census of one residents in care. The facility has six resident bedrooms, one staff bedroom, two and a half bathroom, living room, kitchen, dining room, and an attached two car garage. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors and testing hot water temperature in the bathrooms. The hot water temperature measured 109.7 to 111.5 degrees Fahrenheit and all smoke detectors were operational. The facility conducted an emergency drill in July 1, 2026. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed secured medication storage. LPA observed emergency food in facility garage and emergency water in facility garage. LPA reviewed three of three staff training and fingerprint records and conducted a complete review of one resident record. LPA confirmed that administrator has submitted for renewal of administrator certificate. Based on observations made no deficiencies cited. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors and testing hot water temperature in the bathrooms. The hot water temperature measured 108 and 116.6 degrees Fahrenheit and all smoke detectors were operational. The facility conducted an emergency drill in May 2025. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed secured medication storage. LPA observed emergency food in facility pantry and emergency water in facility garage. LPA reviewed three of three staff training and fingerprint records and conducted a complete review of four out of four clients files. LPA confirmed that administrator has a current administrator certificate which expires on 03/01/2026. LPA observed 1 staff member had obtained a background clearance but is not associated to the facility. Based on observations made deficiencies are cited per Title 22. Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. At around 8:00 AM, LPA Tea was greeted and granted entry into the facility by administrator (AD), Nicole Giovani and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents, with a hospice waiver for four. Currently there are three residents, of which one is on hospice during today's visit. Around 8:00 AM, LPA Tea reviewed three resident files and one staff file. Resident files and staff file contained all required documentation. Upon review of records, the facility is up to date with required quarterly fire drill, which was last conducted in April 2024. AD Giovani’s current certificate expires on March 01, 2026, but due to the department’s back log in processing administrator certificates she has yet to physically receive her administrator certificate. LPA Tea along with the administrator toured the facility at 9:25 AM. LPA toured the physical plant, checked food service, and the first aid kit. The facility is a single-story home that consists of 7 bedrooms, which 6 are private resident bedrooms and one is for staff, which is kept locked, 2 full bathrooms and 1 half bathroom, living room, dining room, and kitchen. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, and shower was free of mold/mildew. Water temperature measured between 111 F degrees and 116.4 F degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements including tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Annual continuation on LIC809-C LPA observed sharps locked in a kitchen cabinet. LPA also observed toxin substances to be locked and inaccessible to clients in care underneath the kitchen sink. Fire extinguishers are fully charged throughout the facility. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample seating underneath a shaded patio area in the backyard. There are two exit gates that are self-latching and operational on both sides of the house. LPA observed emergency food and water supply in the garage and in front of the house as well. There is PPE equipment in the kitchen. Facility provides activities in the form of arts and coloring and puzzles, based on their personal choice. The residents were watching television at the time of the visit. At 9:45 AM LPA reviewed medication storage and administration. Medications are stored in a locked cabinet in the kitchen. Medications are being administered per physician order. LPA interviewed residents regarding their quality of care and spoke to staff present regarding care provided. Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations.
