Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kelly Dulek conducted a required annual visit. LPA was initially met by staff who called the Administrator Karthik "Raj" Kanakaraj, who arrived at the facility shortly after the visit began. Entrance interview conducted. Starting at 02:26PM, LPA along with Administrator, conducted a tour of the physical plant. The facility is single-story with six resident bedrooms, three bathrooms and one staff room. The fire extinguisher was purchased 11/05/2025 and appeared fully charged. No hazards were observed. Bedrooms: The resident bedrooms were properly furnished with clean linens and adequate lighting. Bathrooms: LPA observed bathrooms were clean, properly supplied and had functional fixtures. The LPA observed grab bars and slip-resistant surfaces in all bathrooms. Residents have sufficient amounts of supplies for personal hygiene. The hot water temperature was measured in a common bathroom at 109.6 degrees Fahrenheit, which is within the required limit of 105-120 degrees Fahrenheit. Kitchen: The kitchen appeared clean and the appliances and fixtures functional. The LPA observed a sufficient amount of perishable and non-perishable food at the facility. Knives are stored in a locked cabinet. Medications are stored in a separate locked cabinet in the kitchen. Common Areas: These included the living room and dining area. The common areas were checked for cleanliness and furniture was checked for functionality during time of visit. There is a fireplace in the living room, which is covered with a screen. The facility maintained a comfortable temperature. Report continued on LIC809C Garage/Laundry room: The facility has a laundry room where the washer and dryer are held. The laundry room leads to the garage where cleaning supplies and disinfectants are kept, and additional PPE supplies are stored. The Garage remains locked and inaccessible to the residents in care. Surrounding Grounds (Outdoors): There was a shaded area with proper furniture for outdoor use. There are no bodies of water on the premises. All passageways were clear and free from hazard. Interviews: During the facility tour the LPA conducted one (1) resident interview and two (2) staff interviews. No immediate concerns were identified. Records: LPA reviewed five (5) residents' files and five (5) staff files. All files observed were complete and contained all required documents. Medications: Medications review began at 03:15PM. LPA reviewed medications for 2 (two) residents. Medications are centrally stored and locked in a cabinet in the kitchen area. Medications are labeled and checked for expiration dates. Medications are properly documented on the centrally stored medications and destruction record. Emergency Disaster/Infection Control Plan: During today's visit, LPA reviewed the facility's emergency disaster plan, which was complete and updated annually as required. The facility conducts evacuation drills quarterly with staff on all shifts, with the last completed drill on 09/30/2025. The facility's infection control plan was observed to be complete, reviewed and updated as required. No deficiencies were observed. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Teresa Camara conducted a required annual visit. LPA was initially met by staff who called the back-up administrator Karthik "Raj" Kanakaraj. The administrator arrived at the facility at approximately 11:45 a.m. LPA explained the reason for the visit. Starting at 11:14 a.m. LPA conducted a tour of the physical plant accompanied by staff. The facility is single-story with six resident bedrooms, three bathrooms and one staff room. The smoke detectors and carbon monoxide detector were tested and functioned properly during the visit. The fire extinguisher was last serviced 4/1/2024 and appeared fully charged. No hazards were observed. Kitchen: The kitchen appeared clean and the appliances and fixtures functional. The LPA observed a sufficient amount of perishable and non-perishable food at the facility. Knives are stored in a locked cabinet. Bedrooms: The resident bedrooms were properly furnished with clean linens and adequate lighting. Bathrooms: LPA observed bathrooms were clean, properly supplied and had functional fixtures. The LPA observed grab bars and non-skid mats in all bathrooms. Residents have sufficient amounts of supplies for personal hygiene. The hot water temperature was measured in two common bathrooms at 110.0 to 112.1 degrees Fahrenheit, which is within the required limit of 105-120 degrees Fahrenheit. Common Areas: These included the living room and dining area. The common areas were checked for cleanliness and furniture was checked for functionality during time of visit. There is a fireplace in the living room, which is covered with a screen. The facility maintained a comfortable temperature. The laundry room leads to the garage where cleaning supplies and disinfectants are kept, and additional PPE supplies are stored. The Garage remains locked and inaccessible to the residents in care. Surrounding Grounds (Outdoors): There was a shaded area with proper furniture for outdoor use. There are no bodies of water on the premises. Interviews: During the facility tour the LPA attempted to speak with residents but due to cognitive issues was unable to interview the residents. LPA conducted interviews with two staff. No immediate concerns identified. Records: LPA reviewed five residents' files and reviewed medications. Files were complete and medications appear to be given as prescribed. LPA reviewed files for three caregivers and the administrator. Files were complete with no immediate concerns. LPA reviewed the emergency disaster plan and evacuation drills are conducted quarterly with staff on all shifts. No deficiencies were observed. Type of visit: Case Management - Incident
Licensing Program Analyst (LPA) Teresa Camara conducted a subsequent Case Management-Incident visit. The purpose of the visit is to issue final findings related to the Case Management-Incident visit initiated by LPA Zabel Chochian on 03/20/2024. LPA Camara met with administrator Karthik Kanakaraj. Entrance interview conducted. On 03/15/2024, the Department received an incident report, noting that on 03/08/2024, Resident #1 (R1) suffered a fall in their private room, which resulted in a fractured hip and hospitalization. On 03/20/2024, from 3:00 p.m. to 5:00 p.m., LPA Zabel Chochian conducted a Case Management - Incident visit to the facility. During the visit, LPA Chochian and staff toured the facility at 3:15 p.m.; the LPA conducted a file review at approximately 3:30 p.m. and obtained pertinent documentation related to R1; and interviews with staff and the Administrator were held regarding the incident at approximately 4:00 p.m. The LPA determined further investigation was needed and informed the Administrator that the case was referred to the Community Care Licensing Investigations Branch (IB) and assigned to Investigator Dennis Seng. On 05/15/2024, at approximately 10:45 a.m., Investigator Seng conducted an interview with R1’s resident representative; on 05/16/2024, from approximately 4:02 p.m. to 5:35 p.m., with staff, Administrator, and residents; on 07/11/2024, contacted Ventura Police Department (VPD) and was informed there was no incident report taken or on record for R1. In addition, Ventura Community Memorial Hospital (VCMH), Victoria Care Center, and Viva Home Health medical records and facility file documents related to R1 were reviewed. According to R1’s Preplacement Appraisal Information, R1 was diagnosed with schizophrenia. R1 was unable to manage their own treatment and has a mild cognitive delay. It lists R1 as unable to walk without any physical assistance. R1 is unable to use evacuation routes including stairs and is unable to quickly (continued on LIC809C) (continued from LIC809) evacuate. Per R1’s Physician Report, dated 09/19/2023, R1 also has unspecified dementia, and was listed as non-ambulatory. Under the Medical Emergencies portion of when to call 911, it states if a resident has fallen and may have broken bones, to call 911. Per R1’s Needs and Services Plan and Resident Appraisal, dated 09/07/2020, it shows R1 was diagnosed with COPD, Alzheimer’s disease, schizophrenia, hypertension, and dementia. The VCMH medical records revealed R1 was brought in by ambulance to the emergency room on 03/09/2024 at approximately 8:39 a.m. with the chief complaint of left hip pain. The records documented R1’s “pain is currently rated a "7/10" in severity. Patient’s increasing pain prompted today's Emergency Doctor’s visit for further evaluation.” The records further documented R1 sustained a left femoral neck fracture (hip fracture) after a mechanical fall when tripping over a wheelchair at their assisted living facility. R1 underwent left proximal femur open reduction and placement of a cephalomedullary nail with orthopedic surgery on March 9, 2024. On 03/14/2024, R1 was discharged to Victoria Care Center for further rehabilitation. Based on files reviewed and interviews conducted, the Department found sufficient evidence to prove that the facility was responsible for the neglect and lack of care and supervision of R1 by failing to contact medical services in a timely manner. This caused R1 to experience extended pain. R1 sustained a broken left hip from a fall dated 03/08/2024 at approximately 11:30 p.m. Facility staff both admitted to not calling 911 or notifying the Administrator of R1’s fall until the following morning on 03/09/2024 at approximately 8:00 a.m. Once the Administrator was notified of the fall, they immediately called 911 and notified R1’s resident representative. R1 was not taken to Ventura Community Hospital until 03/09/2024, at approximately 8:38 a.m. The facility staff both stated that the fall occurred at approximately 11:30 p.m. on 03/08/2024. They both admitted they should have notified the Administrator of the fall immediately and called 911 to have R1 checked, as R1 stated R1 was experiencing pain on their left hip immediately after the fall and complained of pain on several occasions throughout the night. Based on the evidence received from the facility staff, and the medical records; the department has sufficient evidence to determine that the facility staff did not obtain timely medical attention for R1. Type of visit: Case Management - Incident
Licensing Program Analyst (LPA) Zabel Chochian conducted a Case Management - Incident visit to the this facility. Upon arrival LPA met with staff and reason for visit was explained. Staff contacted Administrator who arrived at approximately 4pm. Reason for the visit was discussed with the Administrator Kathiga V. The facility self-report an incident, noting that on 03/08/2024, resident #1 (R1) suffered a fall in the private room, which resulted in fractured hip and hospitalization. According to the staff, they heard noises from R1's room and found R1 sitting on the carpet. R1 reported to staff that they lost their balance. it was also noted that staff checked R1's body, found no bruises, but R1 was in pain so staff gave R1 pain medication and R1 went to bed. According to staff and Administrator R1 slept well through the night. However, in the morning, R1 complained of having pain, therefore Administrator called 911 and paramedics took R1 to Community Memorial Hospital. R1 sustained a fractured left hip and undergone surgery. During today's visit, LPA and staff toured the facility which consist of six (6) resident bedrooms, three (3) bathrooms, garage, and kitchen at approximately 3:15pm. Also during todays visit LPA conducted a file review at approximately 3:30pm and obtained pertinent documentation relating to R1. Interview with staff and Administrator was held regarding this incident at approximately 4pm. According to staff and Administrator R1 was not considered a fall risk; R1 was able to ambulate with a walker with no assistance. According to Administrator and staff no history of any other falls at the facility for R1 since date of admission 02/10/2018. Prior to issuing final licensing report, it has been determined that further investigation is needed at this time. This case was referred to Community Care Licensing Investigation's Branch (IB) for further investigation. Licensing Program Analyst (LPA) Esther Cortez arrived at the facility unannounced to conduct a required annual visit at 2:45 p.m. When the LPA arrived, there was two staff and six residents present. The LPA was greeted by Caregiver Emma Cacho and informed them of the reason for the visit. Administrator Karthiga Vijayakumar shortly arrived. At 02:54 p.m. the LPA conducted a tour of the physical plant with the Administrator. The following was noted: Facility is a single-story residence that consists of six (6) resident bedrooms and three (3) bathrooms. There is one (1) additional bedroom for staff use. LPA observed (1) fully charged fire extinguisher purchased in March of 2023. All smoke alarms and carbon monoxide detectors were tested and functioned properly during the time of visit. The LPA observed all required postings in the hallway. The facility serves residents with dementia, the auditory alarms on the exit doors were tested and functioned properly at the time of visit. Kitchen: The kitchen appeared clean and the appliances and fixtures functional. The LPA observed a sufficient amount of perishable and non-perishable food at the facility; Sharp objects are stored in a locked cabinet. Bedrooms: The resident bedrooms were properly furnished with at least one chair, nightstand and sufficient lighting for each resident. The bedrooms had appropriate and adequate bedding and linens such as sheets, pillowcases, mattress pads, and blankets. Bathrooms: LPA observed all bathrooms clean, properly supplied and had functional fixtures. The LPA observed grab bars and non-skid mats in all bathrooms. Residents have sufficient amounts of supplies for personal hygiene. The hot water was measured in the communal bathroom during physical plant tour. Hot water measured 105.6 degrees Fahrenheit, within the required limit of 105-120 degrees Fahrenheit. Report will continue on LIC809-C. Common Areas: These included the living room and dining area. The common areas were checked for cleanliness and furniture was checked for functionality during time of visit. There is a fireplace in the living room, which is covered with a screen. The facility maintained a comfortable temperature of 76 degrees. Garage/Laundry room: The facility has a laundry room where the washer and dryer are held. The laundry room leads to the garage where cleaning supplies and disinfectants are kept, and additional PPE supplies are stored. The Garage remains locked and inaccessible to the residents in care. Surrounding Grounds (Outdoors): There was a shaded area with proper furniture for outdoor use. There are no bodies of water on the premises. Interviews: During the facility tour the LPA conducted two (2) resident Interviews. At 3:23 p.m. the LPA conducted two (2) staff interviews. No immediate concerns were voiced.
