Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced for a required annual inspection and met with Mona Lee Bodden, who contacted the Administrator. Administrator, Claudia Mihai arrived shortly to the facility. The facility is approved for (6) non-ambulatory residents and room #2 was approved for two occupants by the local fire department. There is a hospice waiver for (6) non-ambulatory residents and there is (1) resident currently under hospice care. LPA observed (5) residents resting in their rooms and (1) resident return to the community around 4:00 pm. LPA and the Administrator toured the interior/exterior of the facility including the common areas, (2) shared resident bedrooms, (3) private resident bedrooms, (2) resident bathrooms (2), and laundry/garage area. LPA observed the facility to be clean, in good repair and odor-free. The bathrooms have the necessary grab bars, slip-resistant flooring and hygiene products. There is sufficient 2+day perishable, including fresh produce, and and 7+day non-perishable supply of food. Hot water measured 108*F in the kitchen. The smoke/monoxide are working. There is a complete first aid kit. LPA reviewed (2) resident files and (2) staff files. Files are organized and contain current documentation. Staff are completed training as required, including First Aid/CPR and are cleared/associated. Administrator has current RCFE Administrator Certificate #7019737740 (exp 7/31/27). LPA obtained updated facility land line and mobile contacts. Discussed multiple topics, including putting medication documentation in one binder. LPA to email forms. Annual fees are due by Oct 22, 2026. The administrator will contact the county public Guardian for one resident for additional information. Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced for a Post-Licensing Inspection and met with Mona Lee Bodden, who contacted the Administrator. Administrator, Claudia Mihai arrived at 3:00 pm. LPA observed (1) resident in the common area and (4) residents resting in their rooms. At the end of the inspection, (1) resident returned around 4:15 pm. The facility is approved for (6) non-ambulatory residents and room #2 was approved for two occupants by the local fire department. There is (1) resident currently under hospice care and (1) resident under years. LPA and the Administrator toured the interior/exterior of the facility including the common areas, resident bedrooms (5), resident bathrooms (2), kitchen, staff room and laundry/garage area. LPA observed the facility to be clean, in good repair and odor-free, and the bathrooms to have the necessary grab bars, slip-resistant flooring and hygiene products. There is sufficient 2+day perishable, including fresh produce, and and 7+day non-perishable supply of food. Hot water measured 114*F in the kitchen. LPA reviewed (2) resident files and (3) staff files. Files were organized contained current documentation. Staff are completed training as required, including First Aid/CPR and are cleared/associated. Administrator has current RCFE Administrator Certificate #7019737740 (exp 7/31/27). LPA to add cell phone and email address. Current copy of liability insurance obtained. LPA and administrator discussed a question about a resident who is a fall risk and is waiting for approval from the physician for a foot bed rail. Administrator will also request an exception as a next step. There were no deficiencies observed or citations issued in this report. Exit interview. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Sabrina Calzada arrived announced for a scheduled Pre-Licensing inspection and met with Administrator, Claudia Mihai. Also present was staff, Nanalie Bodden. The reason for this Pre-Licensing inspection is due to a change in ownership. There were (5) residents present during today's inspection. A required annual was recently completed on 10/3/25 just prior to the Regional Office being requested to schedule a Pre-Licensing Inspection. The facility is approved for (6) non-ambulatory residents and room #2 was approved for two occupants by the local fire department. LPA and the Administrator toured the interior/exterior of the facility including the common areas, resident bedrooms (5), resident bathrooms (2), kitchen, staff room and laundry/garage area. LPA observed the facility to be clean, in good repair and odor-free, and the bathrooms to have the necessary grab bars, slip-resistant flooring and hygiene products. There is sufficient 2+day perishable, including fresh produce, and and 7+day non-perishable supply of food. There are locked sharps in the kitchen, locked toxins in the laundry area and medications secured nearby. The inside temperature measured 70*F, and the fire extinguisher was last serviced 5/5/2025. The smoke monoxide alarms are in working order, and the hot water measured 113*F in the kitchen. There is a covered patio and one unlocked exit gate and alarms on the exit doors. There are no pools/ponds but a non-operational fountain that is fenced. Required postings are viewable in the common area. LPA reviewed (3) resident files and (4) staff files. Files were organized contained current documentation. Staff are completed training as required, including First Aid/CPR and are cleared/associated. Medications were reviewed for (2) residents- orders match medications and documentation is maintained. Component III was completed. This Pre-Licensing inspection is complete and this facility has no deficiencies. LPA to notify the analyst in the Centralized Applications Bureau. Exit interview.
