Licensing Program Analyst (LPA) Alviso conducted a Required- 1 Year visit, on 7/16/26 at approximately 1:50pm, and met with Lead Caregiver Charles Lectan. LPA was greeted at the front door by caregiver Paul. Licensee/Administrator was out of town during the inspection,but the LPA did speak with Licensee by phone. Fire clearance is approved for six (6) non-ambulatory, of which one(1) may be bedridden. Facility has six (6) residents in care; .All smoke alarms, which does check for carbon monoxide, were working properly during the inspection. Facility has an approved hospice waiver for three (3) residents. Facility has an approved dementia plan of operation. Facility has an emergency disaster plan as required. Facility has an infection control plan as required. Per record reviews, last emergency disaster drills were conducted on 5/8/25 a water, electrical, and gas shut off/outage, and on 2/17/25 an evacuation drill. LPA reviewed six (6) resident files, including medication records, and observed storage of medications. LPA reviewed four (5) staff files, including training. All staff have criminal record clearance. All staff have first aid certification and CPR certification as required. Hot water was measured at 112.4 degrees Fahrenheit. All exit doors had auditory alarms. All exits were free and clear of obstructions. Continued on LIC809C... Sifficient food supply. Emergency food supply, emergency water, and other supplies were sufficient to meet the "72-hour shelter in place" requirement. Fire extinguishers were serviced and tagged as required. All medications were centrally stored as required. All common areas, resident rooms, hallways, and bathrooms had sufficient lighting for residents use. Sufficient linens, hygiene products, paper products, cleaners/disinfectants, and personal protective equipment (PPE). Bathrooms had required grab bars, and non-slip flooring/non-slip mats for resident use in showers. The home was observed to be clean and orderly. The backyard was clean and orderly, with all walkways free and clear of obstructions. The fire exit paths were clear, and the fire gates were working properly during the inspection. Administrator arrived to meet with the LPA shortly after being contacted by the staff. Facility has an approved hospice waiver for three (3) residents. Facility has an approved dementia plan of operation. Facility has an emergency disaster and evacuation plan as required. Facility has an infection control plan as required. Per record reviews, last emergency disaster drills were conducted on 5/8/25 a water, electrical, and gas shut off/outage, and on 2/17/25 an evacuation drill. All staff have first aid, and CPR certification. Facility water was measured at 120. degrees Fahrenheit, which is within regulation. All exits were free and clear of obstruction. Smoke alarms are also carbon monoxide detectors, all worked properly during the inspection. Continued on LIC809C.. Food supply was sufficient. Facility had sufficient furnishings for resident use, and all areas of the facility have sufficient lighting for residents in care. All bathrooms had grab bars, and showers had non-slip flooring/mats for resident use. Facility had a sufficient supply of linens, paper products, cleaners/soaps/disinfectants, and personal protective equipment (PPE). Facility was observed by the LPA to be clean and orderly, and at a comfortable temperature during the inspection. Facility had a supply of emergency food and water supplies to meet the "72 hour shelter in place" requirements. Facility has an approved dementia plan of operation. Facility has an emergency disaster and evacuation plan as required. Facility has an infection control plan as required. Fire clearance is approved for six (6) non-ambulatory, of which one(1) may be bedridden. LPA reviewed five (5) resident files. Medication records and storage of medications were checked. All medications were locked and inaccessible to residents in care. LPA reviewed three (3) staff files. All staff have required criminal record clearance. Staff have required training. Staff have first aid certification and cpr certification as required. Facility has emergency drills quarterly as required, per record reviews. LPA toured the facility with Administrator. Hot water was checked at 110. degrees Fahrenheit. All exits were free and clear of obstruction. All exit doors had auditory alarms. Fire extinguishers, two (2), were showing fully charged, were inspected and tagged. Smoke alarms are hard wired, and is also a carbon monoxide detector. Food supply was sufficient. All medications were locked and inaccessible to residents in care. All cleaners/disinfectants were locked and inaccessible to residents in care. Facility was clean and orderly. Facility had a sufficient supply of personal protective equipment (PPE), paper products, hygiene products, linens, and furnishings for resident use.
