Type of visit: Required - 1 Year
At approximately 12:15 PM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year visit and was greeted by Administrator Armela Monte. Alta Care Home is Licensed as a Residential Care Facility for the Elderly (RCFE). The facility is a single story ranch house. The facility has an approved fire clearance for six (6) non-ambulatory residents. The facility has a Hospice Waiver for two (2) residents. Upon arrival, LPA was informed that there were five (5) residents in care. At approximately 12:25 PM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 12:30 PM, LPA toured the facility wiih Administrator Monte. All exits were clear and unobstructed. The facility's fire extinguisher was last serviced and tagged on 2/19/2026. The facility was sufficiently lighted. LPA inspected four (4) resident bedrooms and found all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Toxins were observed to be stored inaccessible to residents. Facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility is conducting fire and emergency drills quarterly. The last disaster drill was conducted on 2/20/2026. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors are hard wired and were observed to be operational. Door alarms were observed to be operational. Continued on 809-C... ...Continued from 809 At approximately 1:10 PM, LPA reviewed five (5) resident files. Five (5) of five (5) resident files were observed to be well organized and thorough with all required documentation. LPA reviewed five (5) staff files. Five (5) of five (5) staff files were observed to be well organized with all required documentation including First Aid and CPR certification and proper training documentation. LPA audited Medication for three (3) residents. LPA observed all medications to be centrally stored, secure and with proper documentation. The facility does not handle resident’s monies for personal and incidental items. Armela Monte’s Administrator Certification 6071239740 is current with an expiration date of 7/8/2026. LPA requested the following documents be submitted to Community Care Licensing by 5/17/2026: LIC 500 Personnel Report LIC 610E Emergency Disaster Plan Proof of Liability Insurance The facility is in compliance. No deficiencies cited during today's visit. Exit interview conducted. Copy of report discussed and provided to Administrator Monte. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Hansen arrived unannounced to conduct an Annual Required inspection and met with Licensee, Laarni Lockerbie & Administrator, Armela Marie Monte. Facility is single story with 5 bedrooms & 3 bathrooms with an approved Fire clearance for 6 Non-Ambulatory residents & Hospice Waiver for 2. There is a total of 5 residents, 1 under hospice care & 3 with diagnosis of dementia. LPA initiated a tour of the facility at approximately 8:25 am and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Facility has at least two days of perishable and one week of non-perishable foods which appeared to be of quality and stored per regulation. Cleaning supplies and toxins observed in locked cabinets in garage & under kitchen sink not accessible to residents in care. Resident’s rooms were furnished per regulation. Water temperature in bathrooms measured at 118 degrees F and 121.2 degrees F, not within the range of 105 to 120 degrees F allowed per regulation; although Administrator provided proof new water heater was just installed prior day and is adjusting (LIC9102 TA). Resident bathrooms had required slip resistant mats and grab bars. Extra hygiene products and linens were available. Cabinet containing cleaning supplies in garage was locked. There was enough lighting in all common areas, resident rooms, and hallways. Fire extinguisher was last inspected 3/4/2025. Smoke and Carbon Monoxide detectors located throughout the facility were tested and operational. At approximately 9:15 AM, LPA reviewed 5 of 5 resident records and found 5 of 5 residents have current physician’s reports or care plans. 5 of 5 resident records contained current and signed admission agreements and physician’s orders on file. Continue on LIC809C Continue from LIC809: Medication records are thorough and contained physician’s orders for each resident. At approximately 10:30 AM, LPA review 4 of 4 staff records. 2 of 4 staff records (S1 & S2) only contained 12 of required 20 annual completed training as required (see LIC809D). Evidence of current first aid and CPR training were observed. 4 out of 4 staff have required Health Screening records. Medications were centrally stored in locked cabinet in the facility living room. The Medications of 2 out of 2 residents were found to be given according to physicians’ directions on 4/22/2025 at 12:30 PM. At approximately 12:45 PM, LPA reviewed the facility emergency disaster plan with staff. Facility does not have a generator to supply power during an outage. Administrator informed they are currently deciding how to handle emergency outages with residents on oxygen and other electrical issues (see LIC9102 TA) The plan outlines evacuation routes, which are shown on facility sketch and has alternative meeting locations. Facility has supplies enough to operate for more than 72 hours in an emergency. Facility conducts disaster drills quarterly and documented last disaster drill on 2/24/2025. Administrator Certification for Armela Marie Monte #6071239740 expires 07/08/2026. Appeal Rights Given. The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal rights provided. Type of visit: Required - 1 Year
Licensing Program Analyst’s(LPA’s) Shannan Hansen & Julie Florio arrived unannounced to conduct an Annual Required inspection and met with Licensee, Laarni Lockerbie & Administrator, Aira Monte. There is a total of 6 residents, 1 under hospice care & 2 with diagnosis of dementia. LPA’s initiated a tour of the facility at approximately 8:45 am and made the following observations: Facility was a comfortable temperature and passageways were free from obstructions. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Facility has at least two days of perishable and one week of non-perishable foods which appeared to be of quality and stored per regulation. While touring kitchen at approximately 8:50am LPA’s observed cleaning supplies in unlocked cabinet under sink accessible to residents in care (see pic & LIC809-D). Resident’s rooms were furnished per regulation. Water temperature in bathrooms measured at 111.3, 111.7, and 112.1 degrees F which is within the range of 105 to 120 degrees F allowed per regulation. Bathrooms used by resident to shower were equipped with necessary grab bars and non-slip floors/mats. While touring bathrooms LPA’s observed with staff (S2) supplements in unlocked drawer and immediately removed by staff(see pics & LIC809-D). Extra hygiene products and linens were available. Cabinet containing cleaning supplies in garage was locked; although upon entering, door to garage was unlocked and alarm was not turned on, LPA’s found razor cutting knife accessible to residents in care and removed by staff (see pics & LIC809-D). There was enough lighting in all common areas, resident rooms, and hallways. Fire extinguisher was last inspected 3/11/2024. Smoke and Carbon Monoxide detectors located throughout the facility were tested and operational. At approximately 9:15 AM, LPA’s reviewed 6 of 6 resident records and found 6 of 6 residents have current physician’s reports and care plans. 6 of 6 resident records contained current and signed admission agreements and physician’s orders on file. Continue on LIC 809-C At approximately 10:20 AM, LPA’s review 5 of 5 staff records. 5 of 5 records contained documentation of completed training as required. Evidence of current first aid and CPR training were observed. 4 out of 5 staff did not have required Health Screening records (see LIC 809-D). Medications were centrally stored in locked cabinet in the facility living room although second cabinet containing overflow of medications was not locked and accessible to residents in care (see pic & LIC809-D). The Medications of 2 out of 2 residents were found to be given according to physicians’ directions on 3/27/2024 at 11:30 AM. At approximately 12:10 PM, LPA reviewed the facility emergency disaster plan with staff. Facility does not have a generator to supply power during an outage. Administrator informed they are currently deciding how to handle emergency outages with residents on oxygen and other. The plan outlines evacuation routes, which are shown on facility sketch and has alternative meeting locations. Facility has supplies enough to operate for more than 72 hours in an emergency. Facility conducts disaster drills quarterly and documented last disaster drill on 2/23/2024. Administrator Certification for Aira Melanie U Monte 6056005740 expires 08/29/2024. LPA reviewed Licensing Information System (LIS) with Licensee who stated that is correct and updated at this time; no need to change any of the information. Appeal Rights Given. The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal rights provided.
