Type of visit: Required - 1 Year

On 9/24/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Kobi Walker. LPA was granted entry. 1 clients was present during inspection. The other 7 clients were attending Day Program. Administrator Camalah Kopacz (A1) was called but was not able to arrive for visit. LPAs toured facility with S1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 37 degrees F. Cleaning chemicals was observed stored and locked under kitchen sink. Fire extinguisher was observed with a service date of: 7/1/25. Fire drill last completed on 07/28/25. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature tested at a range of 109.4 to 1116.7 degrees F. in 2 bathrooms. Outside of facility toured. Side gates were self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Medications were checked and observed kept locked in medication cart. LPA observed a resident’s pill/tablet on the floor of the kitchen and accessible to residents. Clients’ MARS was reviewed. Carbon monoxide and smoke detectors were tested and observed to be operational. Samples clients’ file reviewed and was missing some required documents. All of the staff files were reviewed and observed to have all the required documents. The following deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 10/8/25: Lic 308, Lic 500, Lic 610D, and Lic 9020. LPA received a copy of current Administrator certificate. Type of visit: Required - 1 Year

On 11/21/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. introduce self, stated the purpose of the visit and met with staff Kobi Walker (S1). Administrator Camalah Kopacz was called and unable to attend meeting. Administrator authorized and stated designee able to sign report. One resident was present upon LPA arrival in the common area. 9 residents arrived later during inspection. LPA toured facility with S1. The facility was observed to be at a comfortable temperature and no passageway obstructions or fire hazards were observed inside or outside. Video camera with audio was observed in the living room and dining room. Fire extinguisher was observed with a service date: 12/03/23. An adequate supply of perishable and non-perishable food was observed. Refrigerator temperature is maintained at 30 degrees F and freezer at -4 degrees F. Cleaning supplies and chemicals stored and unlocked in laundry room. Dryer was operational during visit. Extra linens were observed in hall closet. All bedrooms were observed to have the required furnishings and with adequate lighting. All bathrooms were toured and observed operational during inspection. Non-skid mat and grabbed bars were observed. Hot water temperature was tested at 105.3 degrees F in bathroom 1, 105.8 degrees F in bathroom 2, and 106.2 degree F in bathroom 3. Outside of facility toured and observed to be free of debris. Adequate outdoor seatings available for residents. Medications were observed locked in cabinet in dining room. Sample of residents’ and staff files were reviewed. Smoke detectors and carbon monoxide were observed operational during visit. A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 12/02/24. The following updated forms were requested: Lic 308, Lic 400, Lic 402, Lic 500, Lic 610E, current liability insurance, and current Administrator certificate. Designee signed report. A during a scheduled Office visit a Complaint visit was conducted. Licensing Program Manager (LPA) S. Moua & Licensing Program Analyst (LPA) K. McClurg met with Assistant Administrator Camalah Kopacz. Records reviewed. It was confirmed that staff has yelled at & touched head of residents in a manner that is not in compliance with the personal rights of residents. The Department has investigated the above allegation & determined it to be Substantiated. Deficiency issued. An Informal Office Meeting was conducted on this date to discuss concerns at the facility. Present during the meeting were: Licensing Program Manager, See Moua Licensing Program Analyst, Kelly McClurg Administrator, Camalah Kopacz The following issues were discussed: -The accessibility of chemicals and hazardous materials to residents at the facility. -The personal rights of the residents because of inaccessibility to hygiene products (toilet paper, soap etc). -Staffing at the facility. Inspection tool was discussed for Annual visits. During the course of the meeting, findings were delivered for Complaint #24-AS-20240209144355, received on 2/9/24. LPA introduced self & stated purpose of visit. Acting Administrator (AD) Camalah Kopacz not available @ time of visit. LPA conducted visit with HM. Facility toured. Physical plant reviewed with HM. Shed in backyard containing hazardous items such as paint unlocked. Lock placed on shed @ time of visit. Smoke detectors & carbon monoxide detectors throughout facility. Fire extinguisher service date: June 2023. Facility will require physical plant follow-up & record review @ a different date. Deficiency issued & cleared @ time of visit. Exit interview done with HM.
