Type of visit: Required - 1 Year
On 5/21/26 Licensing Program Analyst (LPA) J. Leffall conducted an unannounced visit today for the facility’s annual inspection. LPA met with Administrator (A1), Zainab Younes and stated the purpose of the visit. There are currently 14 residents who reside at this facility. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, medication storage, kitchen, and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate of the right amount of perishable and non-perishable foods. LPA toured the facility with A1. Fire extinguisher is within the safety regulation period with a service date of 12/1/25. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Fire drill conducted on 3/8/26. Water temperature was tested at a range of 105 to 119.3 degrees F in 5 bathrooms. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA reviewed 2 facility staff files, and 5 facility resident files. Some staff files were missing documents upon inspection and observation. LPA confirmed all facility staff present is background cleared. No deficiencies issued during this inspection. Exit Interview conducted. LPA is requesting the following documents be submitted to the Fresno CCL office by 6/4/26: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E), Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020A) A copy of this report with 9102 was provided to Administrator, whose signature on this form confirms receipt of this report.the state’s words, verbatim · CDSS document, May 21, 2026

Type of visit: Prelicensing
A Prelicensing continuation visit was conducted by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Licensee (Mgr-Mbr) /Administrator (L/A) - Zainab Younes. LPA introduced self, provided business card, stated purpose of visit, & was allowed to proceed with visit. This is a change of ownership with residents in care. Fire cleared for All Non-Ambulatory. Hospice Waiver for five (5). Facility landline telephone (559) 294-9080. Physical Plant toured. Dining room, common room, TV room sufficiently furnished with adequate lighting. No unpleasant odors detected. Kitchen toured. Kitchen appeared to be clean & free of miscellaneous debris on counters & floors. Knives secured in locked area & inaccessible to residents. Refrigerators & freezers @ appropriate temperatures. Inside of appliances appeared to be clean. 2 day perishable & 7 day non-perishable food on the premises. Resident rooms & bathrooms toured. Resident rooms sufficiently furnished with adequate lighting. No unpleasant odors detected. Bathrooms appeared to be clean with clean fixtures. Grab bars in toilet, tub, & shower areas. Hot water measured at west end of facility at 113 degrees F. Hot water at measured on east end at 115 degrees F. Non-slip mats in all tub/shower areas. Continued (Continued) Medication organized & inaccessible to non-authorized persons. Facility records organized & maintained. Outside area toured. Backyard area toured. Landscape maintained & tidy. All interior & exterior passageways clear & unobstructed. Signal system tested & observed to be operational. Staff responded in a timely manner. Sufficient supply of PPE on the premises. Hazardous items, include cleansers, in secured areas inaccessible to residents. Component III (Comp III) completed during this visit. Exit interview conducted with L/A.
