On 12/4/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Licensee (L1) Balwinder Kaur. LPA was granted entry. 1 client was present during inspection. The other 2 clients attended day program. Administrator (A1) Jaswant Bajwa arrived shortly after LPA's arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Cleaning chemicals was observed stored and locked under kitchen sink. Fire extinguisher was observed with a purchase date of: 12/10/25. Fire drill last completed on 7/2/25. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at 118.5 degrees F in 1 bathroom. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Medications were checked and observed kept locked in medication cart. Clients’ MARS was reviewed. Carbon monoxide and smoke detectors were tested and observed to be operational. All clients’ files reviewed. 1 resident was missing documents in resident’s file. See 9102. Samples of staff files were reviewed. 1 staff is missing 2 LIC documents. See LIC-809D. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 12/24/25: Lic 308, Lic 500, Lic 610D, and Lic 9020. LPA received a copy of current Administrator certificate. Licensing Program Analyst (LPA) K. Kaur arrived at the facility for a subsequent visit to deliver findings. LPA met with Licensee Balwinder Kaur and Administrator Jaswant Bajwa and explained the purpose of the visit and reviewed the elements of the allegations. LPA delivered the following complaint investigation findings. The Department investigated the allegations listed above. Based on interviews and record review the facility was administering medications as prescribed. Records of Centrally Stored medication were accurate. Medication was changed for R1 mid-month from 1 to 1.5 tablet. The correct dosage of medication was being administered without the MARs showing the change. Based on observation and interview of staff and residents, the above allegations are UNSUBSTANTIATED. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore these allegations are unsubstantiated. On 12/26/2024, Licensing Program Analyst (LPA) K.Kaur arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit and was allowed entry by Licensee Balwinder Kaur. Facility tour conducted. LPA toured the facility kitchen. LPA observed 7-day supply of non-perishable foods and a 2-day supply of perishable foods. Medications checked. Medication observed to be locked and inaccessible to residents in care in the closet. Fire extinguisher observed in kitchen which was last serviced on 06/22/2024. The dining area is equipped with a table and chairs, the living room is equipped with adequate sofas and recliners for seating. At 10:45 AM LPA observed the hallway closet to have an odor and observed water around the bathroom floor. At 11:14 AM LPA observed two window screen frames for bent. Medications, first aid kit observed locked in the hallway closet. Residents' bedrooms were observed to be adequately furnished with beds, dresser, and adequate lighting. Mattresses and linen were in good condition. LPA observed grab bars installed by toilet. At 11:48 AM LPA did not observe a non-skid mat or strips in the shower. Smoke alarm detectors installed and operational. At 12:29 PM LPA observed Carbon monoxide detector was not operational LPA toured outside and observed covered area for rest and recreational. Sufficient seating observed under a covered patio. LPA observed unlocked bleach and bug spray in the backyard that Licensee locked immediately. LPA reviewed Resident and staff files and observed admission agreements, ID forms. 2 out of 5 residents did not have Appraisal and needs service plans. At 1:19 PM LPA observed 1 out of 5 residents did not have TB clearance documentation at the facility. LPA observed different administrator documentation than the one on file. LPA emailed Licensee the required documentation to make the change. Fire drill conducted on 8/29/2024. Deficiencies are being issued in accordance to California Code of Regulations, Title 22, Division 6 on the attached 809D. LPA is requesting the following documents be submitted to the Fresno CCL office by 01/02/2025: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Liability Insurance, Emergency and Disaster Plan (LIC 610E) Personnel Report (LIC500), Register of Facility Clients/Residents for (LIC9020A), Surety Bond. Type of visit: Case Management - Deficiencies

On 2/27/2024, Licensing Program Analyst (LPA) K.Kaur arrived unannounced for complaint inspection and conducted a case management in conjunction. LPA was allowed entry by Administrator Rosemarie Riemer. Licensee Balwinder Kaur was also available at the facility. LPA observed on a pervious visit on 11/15/2023 staff member Leonardo Yap (S1) and his Spouse (S2) at the facility. Spouse is not finger cleared and is not associated with the facility. Deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. An Immediate civil penalty of $100 assessed for Caregiver Background Check. An exit interview was conducted with Licensee discussing the plan of corrections. On 12/28/2023, Licensing Program Analyst (LPA) K.Kaur arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit and was allowed entry by staff and requested to meet with the Administrator. Facility staff contacted Administrator, Rosemarie Reimer via telephone. Administrator and Licensee, Balwinder Kaur, arrived a short time later. Facility tour conducted. LPA toured the facility kitchen. 7-day supply of non-perishable foods and a 2-day supply of perishable foods. At 9:10 AM LPA observed cleaning supplies to be accessible in a cabinet under the kitchen sink. Medications checked. Medication observed to be locked and inaccessible to residents in care in the closet. Fire extinguisher observed in kitchen which was last serviced on 01/18/2023. LPA toured outside and observed covered area for rest and recreational. LPA observed a pile of clothing outside the staff bedroom. LPA inquired about the clothing and was informed the facility washer is non-operating. When staff try to use washer, it backs up the water in the bathrooms and toilet and leads to water overflowing. At 10:10 AM LPA continued tour to resident rooms and observed hallway lights to be non-operating. At 10:25 LPA observed bedroom fan and ceiling to have caked on dirt and debris. Debris observed throughout the facility flooring. LPA reviewed Resident and staff files and observed missing admission agreement for one resident. Fire drill conducted on 12/18/2023. Due to time constraints, LPA will return at a later date for an annual continuation.
