Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct a case management visit to follow up on the Technical Advisories from the annual visit conducted on 02/10/2026. LPA Valerio was met by facility staff, and explained the purpose of the visit. LPA Valerio was later met by Administrator Simranjit Bhatia. LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed exit and fire doorways clear and free from obstructions. LPA Valerio observed five (5) residents in care. LPA Valerio observed home health attending to resident in care. LPA Valerio inpsected the shed in the back to be utilized for storage.LPA Valerio discussed incident reports submitted in the last few months. According to Administrator Simranjit, there had been one recent fall, which did not result in any injuries. LPA Valerio and Administrator discussed the option to refer facility to the Technical Assistance Program (TSP). TSP was discussed offered during the 02/10/2026 visit and via e-mail. Administrator stated she would like the regional office to refer her to TSP. LPA Valerio reviewed facility files, which were out of compliance of Title 22 regulations. Resident file reviewed did not have a centrally stored medication log. Staff files reviewed did not have a copy of annual training, such as First Aid and Dementia Care. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Christina Valerio arrived to the facility unannounced to conduct an annual required inspection. LPA Valerio was met by facility staff Melkeet, and explained the purpose of today's visit. Facility staff contacted the administrator who stated she would come to meet LPA. LPA Valerio and facility staff toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed resident bedrooms. Resident bedrooms were observed to free from odors and fully furnished. The bathrooms were observed to stocked with hygiene supplies, a skid mat, a garbage can, and handrails. Common areas were observed to be fully furnished and organized. The thermostat inside the home was set at a comfortable temperature of 72 degree F. The kitchen was observed to have working appliances and an adequate supply of food. The fire extinguisher located in the kitchen was observed to have an annual inspection date of October 15, 2024. Administrator was able to provide LPA proof that they had their service for the fire extinguisher on today's date. LPA was provided a copy of the service with an updated service tag. LPA Valerio observed three resident files and two staff files. LPA Valerio provided Technical Assistance. LPA Valerio provided guidance on resident annual documentation for residents, appraisals, and exception process. LPA Valerio discussed the Technical Support Program and will send information to Administrator via email. LPA Valerio requested the following documentation be sent to CCLASCPSacramentoSouthRO@dss.ca.gov: LIC 500, Facility Sketch, LIC 308, LIC 610, and Copy of Liability Insurance. Type of visit: Prelicensing

On 01/07/25, Licensing Program Analyst (LPA) Kimberly Viarella made an announced visit to this facility to conduct a pre-licensing inspection. LPA identified herself upon arrival and stated the purpose of the visit. LPA met with Licensee/Administrator, Simranjit Bhatia and a brief interview followed. LPA and Licensee toured and inspected the physical plant inside and outside to ensure there were no health and safety concerns. LPA observed the kitchen area, dining area, bedrooms and bathrooms, storage areas, and laundry room. LPA observed knives/sharps area to be locked. LPA observed required furniture, and lighting throughout the facility. LPA measured the hot water to ensure it was between the required range of 105 and 120 degrees Fahrenheit. The first aid kit included supplies such as sterile first aid dressings, bandages or roller bandages, adhesive tape, scissors, tweezers, thermometer, and antiseptic solution. LPA observed area for centrally stored medications to be locked. LPA observed the fire extinguisher(s), smoke and carbon monoxide detector(s). Facility also has central heating and air. LPA reviewed fire clearance and room allocations for ambulatory and non-ambulatory residents.
