On 5/4/26, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced required annual visit using the full CAREs Inspection Tool. LPA met with the Administrator, Mariel Ventura and explained the purpose of today’s visit. The facility is licensed to serve elderly developmentally disabled adults ages 60 and above. Currently they are (5) residents living in the facility. LPA reviewed five (5) staff files, and five (5) residents files which were maintained and in order. LPA reviewed Medication Administration Records for 2 residents. During file review, LPA reviewed Liability insurance documents, Surety Bond, and Emergency and Disaster Plan. LPA Shirley and Mariel toured the facility. The facility is a single-story structure located in a residential neighborhood. The facility consists of (4) resident bedrooms, (3) bathrooms, living room, kitchen, and dining room. Facility maintains all required posting throughout the facility. LPA Shirley inspected all bedrooms. Beds and bedding supplies were in good condition. LPA observed ample lighting throughout the house. Bathrooms were found to be within Title 22 regulations and were operational. LPA observed that medications were safe, locked and inaccessible to clients in care. Last Disaster drill was conducted on 3/6/26. First aid kit is fully stocked with manual. No firearms are stored at this facility. Con'd on 809-C LPA Shirley and Mariel walked through all common areas and there is ample seating and space for all residents. All sharps were locked in a drawer in the kitchen. All rooms and walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. There is a charged fire extinguisher in the kitchen. There are no bodies of water present. The water temperatures range between 107.8 F. The landlines were working and are available on-site. There were no deficiencies issued. On 4/30/25, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced required annual visit using the full CAREs Inspection Tool. LPA met with the Administrator, Mariel Ventura and explained the purpose of today’s visit. The facility is licensed to serve elderly developmentally disabled adults ages 60 and above. LPA reviewed five (4) staff files and all resident’s files and found that they contained all required documents. During file review, LPA reviewed Liability insurance documents and Surety Bond. LPA Shirley and Mariel toured both inside and outside of the facility. The facility is a one-story structure located in a residential neighborhood. The facility consists of (4) resident bedrooms, (2.5) bathrooms, living room, kitchen, dining area, and shaded deck with a ramp in the backyard. Facility maintains all required posting throughout the facility. All bedrooms were toured. Bedrooms 1-4 are occupied by residents and contain the mandated furniture. LPA observed all room to have the required furniture including a bed and chair(s). All beds had the required linens including a mattress cover, fitted sheets, blanket, comforter, and pillow. LPA observed ample lighting in all the bedrooms. LPA Shirley and Mariel toured the kitchen and found it to be clean and sanitary. All appliances were in good working order. Knives were locked and stored in a drawer in the kitchen. The medications were locked and stored in the cabinet located in the living room. LPA observed a 3 day supply of perishable and a 7 day supply of nonperishable foods. The water temperature measured at 109 F. Con'd on 809-C The bathrooms were clean and operational. First aid kit is fully stocked with manual. No firearms ae stored at facility and no bodies of water present. LPA Shirley and Mariel walked through all common areas. In the living room, kitchen, dining room there is ample seating and space for all residents. All rooms and walkways were clean and clear of obstructions and hazards. All areas have ample lighting. All rooms, hallway, and living room have working smoke detectors. There is a charged fire extinguisher in the kitchen. The backyard is clean and clear of obstructions and hazards, and there are no bodies of water present. There were no deficiencies issued. On 4/3/24 Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced visit to this facility. The purpose of today’s visit was to conduct an Annual Inspection. LPA met with Mariel Ventura, Administrator. Facility is licensed for (6) residents (3) non-ambulatory and (3) ambulatory residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: 4 bedrooms, 2 bathrooms, living room, kitchen, dining room/laundry area, covered front porch and back porches both with ramps, and an attach 2 car garage. LPA Shirley and Mariel toured the physical plant. There are no bodies of water or firearms on the premises. All client rooms were checked. Beds and bedding were in good condition, adequate lighting provided, storage for client personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured 109.2 F. A comfortable temperature is maintained in the facility. LPA observed the facility to be clean and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning agents, toxins, and sharps were inaccessible to clients. The kitchen was inspected and there is enough perishable and non-perishable food available which is stored properly. The fire extinguisher was charged, smoke detectors and carbon Monoxide were operable. LPA checked first aid kit; and found that it was compliant with a manual. LPA also observed that the facility has a 30-day supply of Personal Protective Equipment (PPE). And all mandated posters were posted. There were no deficiencies observed during today’s visit.
