Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual and met with Administrator, Titus Popa. LPA stated the reason for today's inspection. LPA observed (3) residents in the common area and (3) residents in their rooms during the inspection. Also present were staff, Ted Popa and Valerie Popa. The facility is licensed for (6) non-ambulatory residents and has an approved hospice waiver for (4) residents. Currently there are (0) residents on hospice. LPA and Administrator toured the interior/exterior of the facility including the common areas, (4) private resident bedrooms (1) shared resident bedroom, (3) resident bathrooms, kitchen, office, staff rooms and laundry/garage area. The laundry/garage and staff room area are always locked. The facility was clean, in good repair and odor-free. Bathrooms have the necessary grab bars, skid-resistant flooring, and hygiene supplies. There is sufficient 2+day perishable, including fresh produce, and 7+day non-perishable supply of food. Sharps are locked in the kitchen and medications are secured nearby. Toxins are locked in the laundry area. Inside temperature measured 76*F and hot water measured 116* in a resident bathroom. Fire extinguisher was last serviced on 12/10/2025, and the smoke alarms are functioning. All exit door alarms are working, and there are no blockages. Staff are aware where the utility shut-off valves are. There is sufficient indoor/outdoor space, including a walking track on front patio and covered seating on back patio. There is (1) unlocked exit gate and (1) open gate. All required postings are visible in the common area. Three (3) resident files and (8) staff files were reviewed. Files are organized, and with current documentation, including training certifications. Administrator has current RCFE Administrator certificate #7007834740 (exp 3/7/27), and the Administrator Designee also has a current certificate (exp 3/1/27). LPA obtained a current copy of current liability insurance. LIC500 to be emailed by 2/10/26. There were no deficiencies observed. Exit interview. There is sufficient 2+day perishable and 7+day non-perishable supply of food, including fresh produce, and sharps are locked in the kitchen. Medications are secured in a nearby cabinet and toxins are secured in the laundry room. Inside temperature measured 75*F and hot water measured 108*F in the kitchen. Fire extinguisher was last serviced on 1/21/2025, and the smoke alarms are functioning. There are activities/games with sufficient indoor/outdoor space, including a walking track on front patio and covered seating on back patio. There are (2) unlocked exit gates. Updated lain-line obtained. (3) resident files were reviewed and found to be organized and contain current documentation. The care plans were last updated in January 2025 and physician reports were completed within the last (12) months. Medications and orders were reviewed for (1) resident orders matched medications being administered and documentation is complete. (8) staff files were reviewed. Files were organized and contained current training documentation, including First Aid/CPR. Annual staff training is completed in January. RCFE Administrator certificates are pending renewal. LPA obtained a current copy of current liability insurance and LIC308. LIC500 to be emailed by 3/14/25. There were no deficiencies observed. Exit interview. Also present was caregivers, Valerie Popa and Ted Popa. LPA observed (3) resident in the common area and (2) residents in their rooms at the start of the inspection. The facility is licensed for (6) non-ambulatory residents and has a hospice waiver for (4). Currently, there are (2) residents on hospice. LPA and Administrator toured the interior and exterior of the facility including the common areas, (4) private resident bedrooms (1) shared resident bedroom, (2) resident bathrooms, kitchen, office, staff rooms and laundry/garage area. LPA observed the facility to be clean, in good repair and odor-free. LPA observed sufficient 2+day perishable and 7+day non-perishable supply of food, and locked sharps in the kitchen. LPA observed locked medications in a separate cabinet near the kitchen and locked toxins in the laundry room. LPA observed the inside temperature to be 76*F and hot water measured 105*F in a resident bathroom. LPA observed games/activities on site. The fire extinguisher was last serviced on 1/5/2024. There are sufficient towels, linens, paper products and PPE. First Aid kit is complete. There is a walking track in the front patio area and covered patio with seating on the back patio. There are (2) unlocked exit gates. (3) resident files were reviewed and found to be organized and contain current documentation, including care plans/physician's reports. Medications were reviewed for (2) residents- orders matched medications being administered and documentation is complete. (5) staff files were reviewed and found to be complete, organized and contain current training documentation, including First Aid/CPR. RCFE Administrator certificates are current. All staff are cleared and associated to the facility. Infection Control Plan was reviewed/approved. Emergency Disaster Plan was reviewed and is posted. Administrator to ensure current list of personal rights is posted and included in admission agreement. Obtained copy of current liability insurance and LIC308. LIC500 to be provided by 3/13/24.
