Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Bennette Pena attempted an Annual/Required -1 year visit at the above-mentioned facility. Upon arrival at 1:08pm, no one was in the home to let the LPA in. The Administrator, Sang Heng was contacted by phone at 213-910-0442 and LPA was informed that they are out of town and will return the following week. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Luis De Leon conducted the required annual inspection. LPA arrived unannounced and met with Administrator Sang Heng who allowed the access to facility. The purpose for the visit was explained. The facility is licensed for adults sixty (60) years and over. Six (6) Residents may be non-ambulatory, of which one (1) may be bedridden. Facility is approved for three (3) hospice residents. Currently, the facility houses NO residents. LPA use the Compliance & Regulatory Enforcement Tool (CARE) during today’s inspection. The observations were made during the tour of the facility: Ms. Heng informed LPA that there are currently no residents residing in Little Villa and informed LPA that facility is currently going through a remodeling. The facility has three (3) bedrooms, two bathrooms, kitchen, living room, outdoor swimming pool, and dining room. LPA toured the house and did not observe any residents living in the facility. LPA observed throughout facility interior boxes containing building material for the remodeling taking place. The water was turned off during the remodeling, but it has power turned on. The facility exterior has a lot of debris around facility and yard has not been trimmed. LPA advised Administrator to inform CCLD of ongoing construction project. In addition, LPA inform administrator that annual fees are overdue as of 4/10/25. There was no review of staff records and resident records since the facility is not in operation. Deficiencies were noted and discussed with Administrator. Deficiencies cited under California Code of Regulations, Title 22, Division 6, Chapter 8 and Health and Safety Code, Title 22, Division 6, Chapter 03.2. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Christine Wong conducted the required annual inspection. LPA arrived unannounced and met with Administrator Sang Heng allowed the entry of the facility. The purpose for the visit was explained. The facility is licensed for AGE RANGE 60 AND OVER. 6 NON-AMBULATORY, OF WHICH 1 IS BEDRIDDEN. HOSPICE WAIVER FOR 3. Facility has fire clearance for 6 non-ambulatory residents, one of which is bedridden. Currently there's no resident reside in the facility. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Infection Control: The facility does not have a infection control plan in file and administrator does not have a health screening report with chest x ray and the administrator certificate is not updated. Operational Requirement: The facility does not have any proof of liability insurance for the facility. The facility does not have at lease one person has CPR certificate. Physical Plant and Environmental Safety: The facility includes family room, dining area, kitchen, living room, three residents rooms and two bathrooms and storage room. Currently one bathroom is renovated and the bathroom is not in a operable condition. The bathrooms also have grab bars and non-skid mat. LPA was was not able to check the hot water temperature due to the water was shut off now due to the renovation. Staffing: Currently no staff is working at the facility. Personnel Records-Training: The administrator (Sang Heng) certificate is expired and currently no staff is working at the facility. Resident Records/Incident Reports: Currently facility has no residents reside and no file to be reviewed. Residents Right Information: LPA did not observe any poster for complaint's information or resident right in the facility Planned Activities: Currently there's no residents reside in the facility Food Service: Currently there's no resident reside in the facility. (See LIC 809C for continuation) Incident Medical and Dental: Currently there's no resident reside in the facility and therefore there's no medication in the facility. Disaster Preparedness: Facility does not have an updated emergency and disaster plan posted in the facility Residents with Special Health Needs: Currently there's no resident reside in the facility.
