Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced annual required visit. LPA was granted entry and met with Staff, Amyrose Undag, who was informed of the purpose of the visit. At the time of the visit there was (3) staff and (6) residents present. The facility is a one story home with (5) resident bedrooms and (2) full baths and (1) half bath. No pools or firearms are housed at the facility. The facility is approved for (6) non ambulatory residents. Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair were present. The outdoor area and passage ways were observed to be free of hazards. LPA observed hygiene supplies for residents, PPE, clean linens. There are designated locked areas for sharps, dangerous objects, cleaning supplies, and medications. The smoke detector and carbon monoxide are operational, and the hot water temperature 111F. There are emergency supplies such as emergency food and water. LPA observed facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. LPA observed the facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. LPA reviewed the facility's emergency and disaster plan and infectious control plan. LPA reviewed documentation showing the facility's last fire drill 09/02/2025, which met the department requirements. Required postings were found in the facility. The listed administrator, possesses a current administrator's certificate. LPA reviewed staff files and training. All staff have criminal clearance and updated training along with CPR/First Aid Certification. Resident files were reviewed, and possessed all required paperwork. LPA observed resident medications which are locked, filled, and stored in their originally received containers. The medication was accounted for on Medication Administration Records for residents. Licensing Program Analyst (LPA) Sara Martinez conducted an unannounced annual required visit. LPA was granted entry and met with caregiver Lorie Bagasbas who was informed of the purpose of today's visit. Assistant Administrator Amyrose Undag arrived during LPA's visit. At the time of the visit there was three (3) staff and five (5) residents present. LPA conducted a tour of the interior and exterior, reviewed facility documents and conducted interviews. LPA observed the following: Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were in good repair were present. LPA observed outdoor furniture and shaded area for clients. Detergents, cleaning solutions, and sharp and dangerous objects were observed to be locked and inaccessible to residents. The smoke detector and carbon monoxide was operational, and the hot water temperature met department requirements. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. LPA observed the facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. LPA reviewed staff files and training. All staff have the required personnel records on file and criminal record clearance and updated training. Three (3) resident files were reviewed and possessed all required paperwork which included Admissions Agreement, Needs and Service Plan, and updated Physician's Report. The listed administrator possesses a current administrator's certificate. Resident medication was centrally stored and locked in a medication cabinet and medication refrigerator. LPA reviewed medications prescribed to residents and found all medication with required labeling found to be in place. LPA reviewed the facility's emergency and disaster plan and infection control plan. Facility conducts quarterly fire drills with the last fire drill being conducted on 09/06/2024. All facility exits were clear from obstructions. LPA observed emergency supplies, charged fire extinguishers, and first aid kit with all required items. No deficiencies were cited at the time of the visit. Type of visit: Case Management - Licensee Initiated

Licensing Program Analyst (LPA) Sara Martinez conducted an unannounced case management visit to change the ambulatory status of the facility. LPA met with Assistant Administrator Amyrose Undag and explained the purpose of the visit. At the time of visit there were five (5) residents. Licensee requested a change in the facility's ambulatory status from six (6) non-ambulatory residents to five (5) non-ambulatory status and one (1) bedridden. The facility capacity has not changed. A Fire Clearance was approved on 07/31/24 for a room addition for the one (1) bedridden resident, no change in capacity. Facility sketch on file shows sufficient square footage in the facility and activity rooms to accommodate the ambulatory status change with Bedroom Four having a sliding door exit/entrance approved by the fire department. LPA toured the interior/exterior of the building and visually inspected the resident bedrooms. LPA confirmed that all identified shared rooms are large enough to accommodate the required furniture for two residents without inhibiting movement into and throughout the rooms. The facility has four (4) bedrooms and two (2) bathrooms for the residents. The facility has 1 additional bedroom and bathroom for staff. The physical plant is ready for change in ambulatory status. LPA will submit file for approval. The final approval of capacity increase is contingent upon LPM's final file review. Licensee will be notified by LPA once capacity increase has been approved by licensing. If ambulatory status is approved, new license will follow in the mail after phone notification by LPA with Licensee.
