Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Angela Barutyan arrived at the facility unannounced to conduct a required annual visit at 11AM. LPA met with House Manager Ana Marie Leyva upon arrival. Administrator Ann-Hazel Lopez was contacted by phone during the visit but was unable to be present for the visit. Entrance interview conducted. At 11:06AM, the LPA along with the House Manager, toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. KITCHEN/GARAGE: LPA inspected the kitchen/food service area at 11:06AM. Kitchen appliances appeared clean and were in operable condition at the time of the visit. The facility has a sufficient supply of perishable and nonperishable food. Food labels were inspected and checked for expiration dates and food labels had expiration date clearly marked. Knives and chemicals were locked and inaccessible in kitchen drawer and cabinet under the sink. LPA observed a locked garage by the living room containing emergency food and water supply, additional refrigerator and freezer, washer and dryer, and supplies. BEDROOMS: There are five (5) private resident bedrooms. There is one (1) staff bedroom which was observed locked. LPA observed resident bedrooms to be furnished appropriately with clean linens, appropriate furnishings, sufficient lighting, and equipped with functioning auditory exit alarms. RESTROOMS: There are four (4) bathrooms; three (3) are attached to resident bedrooms and one (1) is a full bathroom in the hallway for residents, staff, and guests. LPA observed resident restrooms equipped with grab bars and slip-resistant surfaces. Hot water temperature was measured in all restrooms and were between 106.0-111.2 degrees F, which is within the required range. Report Continued on LIC 809-C. COMMON AREAS: The common areas were appropriately furnished, and the lighting was adequate. The facility smoke alarm system is hard wired; the smoke and carbon monoxide detectors were tested at 11:19AM and were operable at the time of the visit. The fire extinguishers were fully charged and last purchased 02/28/2026. LPA observed required postings posted in the hallway and upon entry into the facility. OUTDOOR AREA: The backyard has a covered outdoor area equipped with furniture for resident use. There were no bodies of water on the premises. There are a self-latching gates on the side passageways designated for an emergency exit. Passageways were free and clear from obstruction. MEDICATION REVIEW: At 11:24AM, LPA reviewed medications for two (2) residents. Medications are centrally stored and locked in the medication room accessed through the kitchen/laundry. All medications including PRNs were labeled, stored, and locked inaccessible to residents. PRNs were properly documented and logged. No errors observed during the medication review. RECORD REVIEW: Beginning at 11:41AM, LPA reviewed four (4) out of four (4) resident files and three (3) personnel files for documents including but not limited to: medical records, resident Admission Agreement, TB test, staff training, first aid, and fingerprint clearance. LPA observed Staff #1 (S1) without a criminal record clearance. Administrator stated that S1 will not work at the facility until a background clearance is obtained and S1 is associated to the facility. All resident files were in order. INFECTION CONTROL/EMERGENCY DISASTER PLANNING: During today’s visit, the LPA reviewed the facility's infection control policy and emergency disaster plan. Emergency disaster plan is updated annually as required and emergency disaster drills are conducted quarterly as is required, with the last drill conducted on 05/28/2026. All documents reviewed were updated and in compliance. The following deficiency was observed (See LIC 809-D) and cited from the California Code of Regulations, Title 22 and/or California Health and Safety Code. Civil penalty was issued in the amount of $500 for criminal record clearance violation. Administrator was informed that failure to correct the deficiency may result in additional civil penalties. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Zabel Chochian arrived at the facility today to conduct a required annual visit. Upon arrival LPA was greeted by Staff/Caregiver Ana Leyva. LPA introduced self. Caregiver contacted the Assistant Administrator Connie Roush by phone and was informed of the LPA's visit. Assistant Administrator arrived to the facility shortly after LPA. At approximately 11a.m., the LPA met with and interviewed three (3) residents. A physical plant tour was conducted with caregiver Ana Marie Leyva at approximately 11:45 a.m.. Facility is a single-story residence that consists of five (5) resident bedrooms and four (4) bathrooms. There is one (1) additional bedroom for staff use. The facility serves residents with dementia, the auditory alarms on the exit doors were tested and functioned properly at the time of visit. Kitchen: The kitchen appeared clean and the appliances and fixtures functional during the time of visit. LPA observed a sufficient amount of perishable and non-perishable food at the facility; Sharp objects are stored in a locked cabinet to the left of the dishwasher. At 12:30 p.m. hot water measured at 115.2 degrees Fahrenheit. Bedrooms: All resident’s bedrooms were properly furnished with at least one chair, a bed, night stand, chests of drawers, and sufficient lighting for each residents. The bedrooms had appropriate and adequate bedding and linens such as sheets, pillowcases, mattress pads, and blankets. The bedrooms were large enough to allow for easy passage between the beds and furniture. In addition, no bedroom was used as a passageway to another room, bath, or toilet. All rooms were free of odors. All window screens were clean and maintained in good repair. Bathrooms: LPA observed all bathrooms were clean, properly supplied and had functional fixtures. The LPA observed grab bars and non-skid mats in all bathrooms. Residents have sufficient amounts of supplies for personal hygiene. Hot water was measured in two (2) bathrooms and observed within the required limit of 105-120 degrees Fahrenheit (116*5-118.6*f). Continue to LIC809c.. Common Areas: These included the living room and dining area. The common areas were checked for cleanliness and furniture was checked for functionality during time of visit. The facility maintained a comfortable temperature of 75 degrees. The LPA observed the required postings in the entry way and common sitting area. LPA observed video cameras (no audio) in common areas. Garage: The garage is where the washer and dryer are held, including additional non-perishable emergency food items and emergency water. Cleaning supplies and disinfectants are kept in locked cabinets in the garage all cleaning compounds were stored in areas separately from food supplies. Surrounding Grounds (Outdoors): The front yard is free of obstructions, the two side gates have a self-latching door. There is a yard area in the front of the house. LPA also observed a patio in the back yard which had shade and seating areas and proper furniture for residents to enjoy. There were no bodies of water noted. File review: A review of facility files was initiate at 12p.m. and the following was observed. LPA reviewed three (3) of three (3) residents files and three (3) staff file including the administrator’s. All documents reviewed appeared complete and current. Additionally, LPA observed documentation of Infection Control, Disaster prevention and last fire drill conducted on 03/06/2025. Interview conducted with two (2) staff at approximately . Medication audit: Medications review began at 1p.m.; medications are centrally stored and locked in a cabinet in the living area; medications are labeled and checked for expiration dates. Medications are properly documented on the centrally stored medications and destruction record. No errors observed during the medication review. Combination smoke alarms and carbon monoxide detectors were tested at approximately 2pm and were operational during the visit. LPA observed (2) fully charged fire extinguishers purchased on 02/18/2025. LPA requested an updated LIC500 and facility Liability Insurance be emailed to the Regional Office. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Valeria Conway arrived at the facility unannounced to conduct a required annual visit at 9:30 a.m. When the LPA arrived, there was one staff and four clients present. The LPA was greeted by Caregiver Ana Marie Leyva and informed the reason for the visit. Caregiver contacted the Administrator by phone, Ann-Hazel Lopez, and subsequently transferred the call the LPA. During the call the administrator informed the LPA that she would arrive at the facility in a few hours due to personal reasons. At 1:50 pm Administrator arrived at the facility. At 12:20 p.m. the LPA conducted a tour of the physical plant with caregiver Ana Marie Leyva. The following was noted: Facility is a single-story residence that consists of five (5) resident bedrooms and four (4) bathrooms. There is one (1) additional bedroom for staff use. The facility serves residents with dementia, the auditory alarms on the exit doors were tested and functioned properly at the time of visit. Kitchen: The kitchen appeared clean and the appliances and fixtures functional during the time of visit. LPA observed a sufficient amount of perishable and non-perishable food at the facility; Sharp objects are stored in a locked cabinet to the left of the dishwasher. At 12:30 p.m. hot water measured at 115.2 degrees Fahrenheit. The facility maintained a comfortable temperature of 75 degrees. The LPA observed the required postings in the entry way and common sitting area. Combination smoke alarms and carbon monoxide detectors were tested at 10:10 am and were operational at this time. LPA observed (2) fully charged fire extinguishers purchased on 01/28/2024. LPA observed cameras in common areas. Garage: The garage is where the washer and dryer are held, including additional non-perishable emergency food items and emergency water. Cleaning supplies and disinfectants are kept in locked cabinets in the garage all cleaning compounds were stored in areas separately from food supplies. LPA obtained Client Roster, Staff Roster, and facility Liability Insurance. Continued from LIC809-C Continued from LIC 809-C OFFICE AREA: There is an office area with a computer for residents and staff to use. Above the computer desk, there is a cabinet where all resident’s medications are kept. LPA observed a complete 1st Aid Kit inside such cabinet. Medication audit: Medications review began at 10:37am; medications are centrally stored and locked in a cabinet in the living area; medications are labeled and checked for expiration dates. Medications are properly documented on the centrally stored medications and destruction record. No errors observed during the medication review. Interviews: Between 9:45 am and 10:30 am the LPA conducted two (2) staff and three (3) residents’ interviews. No deficiencies cited at this time. Exit interview conducted.
