Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct an annual inspection and met with Lourdes de la Paz, Administrator, and stated the reason for today's inspection. Also present were staff, Jovylito Madum, Fe Luzse Abata, and Dennis Abadilla. The facility is a specialized RCFE facility vendorized through Alta California Regional Center, licensed for (4) non-ambulatory residents, (2) of whom can be bedridden. There were (4) residents present. There is an approved hospice waiver for (2). Currently (1) resident under hospice care. LPA and the Administrator toured the interior/exterior of the facility including the common areas, (4) resident bedrooms (2.5) bathrooms, kitchen, activity room, staff room, laundry area and garage. The facility was clean, very organized, in good repair and odor-free. The bathrooms have the necessary grab bars, slip-resistant flooring, and hygiene supplies. There is sufficient 2+day perishable, including fresh produce, and 7+day non-perishable food. There are alerts on each exit door. There is a music therapist, recreational therapist and nurse that visit weekly. (3) clients currently attend day program. The smoke/monoxide alarm is in working order. Fire extinguisher was last serviced 6/24/25. Sharps, toxins and medications are locked in the kitchen. Inside temperature measured 70*F, and the hot water temperature measured 114*F in a resident bathroom. There are (2) unlocked exit gates and outside seating with shade. All required posters are visible in the common area. There is a complete First Aid kit. LPA reviewed (2) client files and (4) staff files. Files were organized, complete and had current documentation. Medications and orders were reviewed for (1) client -no discrepancies noted. P&I was confirmed for (2) residents. The surety bond- expires 4/26/26. Staff has completed the required continuing education. Administrator RCFE Certificate #7035764740- exp 7/12/26. Allegation investigated: Uncleared adult(s) are present in the facility. Residents were exposed to a staff member with Tuberculosis.

Licensing Program Analyst (LPA) Sabrina Calzad arrived unannounced to deliver findings to a complaint received on June 2, 2025 and met with Lourdes dela Paz, Administrator and Dennis Abadilla, Administrator Designee. (3) clients were attending day program and (1) client was present during today's inspection, who LPA observed. The findings are follows: During the investigation, it was determined that the alleged uncleared adult (S1) was never present, or worked, at this facility but at a related facility. The Department received documentation (S1) was finger print cleared before being hired, as well as all other staff. Individual (S2) was also never present at this facility but at a related one. The facility provided negative results of TB testing for all clients and staff at this location, and at the related location. A follow up case management will be conducted at the related location. Based on information obtained, both (2) allegations were determined to be UNFOUNDED- An unfounded allegation means that the allegation was false, could not have happened and/or is without a reasonable basis. Allegation investigated: Staff pinch a resident while in care. Staff inappropriately shove a resident while in care. Staff deny a resident a resident from using their walker. Staff are mistreating a resident while in care.

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a 10-day inspection and met with Lourdes dela Paz, Administrator. Staff, Jovylito Madum, was also present. LPA observed (3) clients present and (1) client return from day program during today's inspection. LPA discussed the allegations with the Administrator, including the first name of the client who the allegations pertain to. The Administrator and staff stated there is not currently a client with this name living at the care home. Also there are no clients who currently use a walker, and all clients are essentially non-verbal. It was also confirmed the police did not visit the care home last night. Staff, Richard Durmol, was contacted by phone today and confirmed the Ombudsman also visited this care home yesterday due to the incorrect address listed. Based on information obtained, this complaint was inadverently opened under the incorrect facility and findings are UNFOUNDED- An unfounded allegation means that the allegation was false, could not have happened and/or is without a reasonable basis. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct an annual inspection and met with Lourdes de la Paz, Administrator, and stated the reason for today's inspection. Also present was caregiver, Richard Dumol. The facility is a specialized RCFE facility vendorized through Alta California Regional Center, licensed for (4) non-ambulatory residents, (2) of whom can be bedridden. There are currently (4) residents. At the start of the inspection, (1) client was present. (3) clients returned from day program during the inspection. LPA and the Administrator toured the interior/exterior of the facility including the common areas, (4) resident bedrooms (2.5) bathrooms, kitchen, activity room, staff room, laundry area and garage. The facility was clean, organized, in good repair and odor-free. The bathrooms have the necessary grab bars, non-skid flooring, paper towels and 20-second hand-washing posters. There is sufficient 2+day perishable and 7+day non-perishable food. Each resident room door has a locking mechanism, per Title 17, and there are alarms on each exit door. There is a music therapist and recreational therapist that visit weekly. (3) clients currently attend day program. The smoke/monoxide alarms are in working order, and the fire extinguisher was last serviced 7/19/24, Toxins are locked in the kitchen and laundry area. Medications sharps are locked in the kitchen. The inside temperature measured 70*F, and the hot water temperature measured 106*F in a resident bathroom. There is one unlocked exit gate and sufficient outside seating with shade and a garden. All required posters are visible in the common area. There is a complete First Aid kit. LPA reviewed (2) of (4) client files and found them to be organized, complete and current. Medications were checked for (1) client- orders matched medications being given and documentation is current. P&I was confirmed for (2) residents. There is also a current surety bond- exp 4/26/25. (4) staff files were reviewed- documentation on file for initial and/or ongoing training, including current First Aid/CPR certifications. Administrator RCFE Certificate #7035764740- exp 7/12/26. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Melissa Parks arrived on Monday October 30, 2023 to conduct the annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA Parks reviewed client (4) and staff (4) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training (initial and ongoing). Facility has supply of PPE including shields, N95s, gowns, gloves, and covid tests. Facility is clean, organized, and well maintained. LPA Parks and Administrator Lourdes toured the facility together to ensure the health and safety of residents in care. The areas toured included bedrooms, bathrooms, kitchen, common areas, garage, and backyard. In the areas toured, there were no health or safety violations observed. LPA obtained a copy of current liability insurance and LIC500. No deficiencies cited. Exit interview conducted.
