On 4/2/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Ernie Espiritu, LPA was granted entry. 5 residents were present during inspection. Administrator (A1) Eddiemer Sison arrived shortly after LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Samples of resident’s medications were checked and observed locked in kitchen cabinet. Clients’ MARS was reviewed. Fire extinguisher reviewed with a service date of: 12/22/25. Fire drill completed on 1/9/26. Clients' bedrooms were toured and reviewed. Cleaning chemicals were observed stored and locked in closet. Resident’s bedrooms observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 107.2 to 116.4 degrees in 2 bathrooms. Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature observed at -1 degrees F and refrigerator temperature maintained at 34 degrees F. Smoke detectors and carbon monoxide were tested and observed to be operational. All client’s and samples of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. Administrator (A1) Eddiemer Sison arrived shortly after LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Samples of resident’s medications were checked and observed locked in kitchen cabinet. Clients’ MARS was reviewed. Fire extinguisher reviewed with a service date of: 12/19/24. Fire drill completed on 1/14/25. Clients' bedrooms were toured and reviewed. Cleaning chemicals were observed stored and locked in closet. Resident’s bedrooms observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 105 to 117.5 degrees in 2 bathrooms. Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Type of visit: Prelicensing

Licensing Program Analyst (LPA) K.Kaur conducted a Pre-licensing Inspection on this date. LPA met with Licensee Innocent Idusuyi. A tour of the facility was conducted together. This is a new facility with no residents in care. The facility was observed to be at a comfortable temperature, clean, and in good repair. No passageway obstructions or fire hazards were observed inside or outside. Common areas were properly furnished and well-lit throughout. The dining room is equipped with a table and chairs, living room is equipped with adequate sofas and chairs for residents. Front den has additional seating. Adequate outside space for rest and recreational under a covered patio. Gate is self-closing and self-latching. Perishable and non-perishable food supply appeared adequate. Knives/ sharps will be locked in the kitchen cabinets under the microwave. Medication will be locked in a cabinet next to dinning room. Cleaning and Chemical supplies will also be kept locked in cabinet in the garage. Residents' bedrooms were observed to be adequately furnished with bed, dresser, chair and adequate lighting. Mattresses and linen were in good condition. Extra linen and towels are available. Hot water temperature measured at 115 degrees F. Carbon monoxide and smoke alarm detectors installed and operational. Bathrooms have grab bars installed in shower. Toilet equipped with toilet safety rail. Non-skid mats in place, hand soap and paper towels available for use. Trash cans with tight fitting lids are in place. Bathroom next to laundry may be designated for staff. Fire extinguisher was serviced and fully charged. Complaint poster posted, resident council info posted, residents' rights posted; emergency disaster plan posted. Component III was completed by Licensee. Exit interview was conducted. Pre-licensing requirements were met. An exit interview was conducted with Licensee.
