On 3/26/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Annie Togonon, LPA was granted entry. 6 residents were present during inspection. Licensee (L1) Sundari Susan Kendakur arrived shortly after LPA’s arrival. LPA toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Samples of resident’s medications were checked and observed locked in kitchen cabinet. Clients’ MARS was reviewed. Fire extinguisher reviewed with a service date of: 7/28/25. Fire drill completed on 1/10/26. Clients' bedrooms were toured and reviewed. Cleaning chemicals were observed stored and locked in kitchen cabinet. Resident’s bedrooms observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 110.4 to 114.6 degrees in 2 bathrooms. Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature observed at 4 degrees F and refrigerator temperature maintained at 40 degrees F. Smoke detectors and carbon monoxide were tested and observed to be operational. Client’s files and samples of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. Type of visit: Case Management - Deficiencies

On 6/4/25, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up and confirm details of an incident report that was received by the Department. The incident occurred on 5/29/25, in which R1 eloped from facility. LPA met with Administrator Shannon Steele. A1 reported to LPA that battery on the auditory device was low in power and staff was not able to hear door open when Resident left the facility. A citation and is being issued on the attached 809-D regarding the auditory device at the facility. Administrator (A1) Shannon Steele arrived shortly after LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Samples of resident’s medications were checked and observed locked in kitchen cabinet. Clients’ MARS was reviewed. Fire extinguisher reviewed with a service date of: 7/2/24. Fire drill completed on 2/4/25. Clients' bedrooms were toured and reviewed. Cleaning chemicals were observed stored and locked in kitchen cabinet. Resident’s bedrooms observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 105 to 112.8 degrees in 2 bathrooms. Outside of facility toured. Outside observed free of debris. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Freezer temperature observed at 0 degrees F and refrigerator temperature maintained at 40 degrees F. Smoke detectors and carbon monoxide were tested and observed to be operational. On 2/21/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced for a case management visit regarding a Default Decision and Order for Staff 1 (S1). LPA met with Staff (S1) Marcos Garcia. Administrator Shannon Steele was called and arrived shortly and verified Rommel Cortez is not employed. LPA conducted visit to verify an individual that no longer has a criminal record clearance is working at the facility. LPA was informed by Administrator that Staff member is not employed at the facility and no longer associated with the facility. Exit interview was conducted. On 05/31/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA met with staff Rogelio Canchela. LPA introduce self, stated the purpose of the visit and request to meet with Administrator. Administrator was called and Designee (D1) Shannon Steel arrived shortly and conducted tour with LPA. Designee 2 (D2) LuiJean “Jean” DeCastro arrived later during tour. All four residents were present during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. Temperature maintained for refrigerator at 28 degrees F and freezer at 0 degrees F. Fire extinguisher was observed with a service date of: 06/8/23. Cleaning supplies and chemicals stored and locked under kitchen sink. Knives observed stored and lock under kitchen counter. Medications observed kept locked in kitchen shelf. Medications and MARs were reviewed. All bedrooms were observed to have required furnishings and with adequate lightening. Bathrooms were properly equipped, and the hot water temperature was tested at 109.7 and 107.2 degrees in bathroom 1 and tested between 110.8 and 110.5 degrees F in shared master bathroom. Outside of facility toured and observed to be free of debris. Adequate outside seatings observed available for residents. Side gate observed self-closing and self-latching.Carbon monoxide and smoke detectors were tested and observed to be operational. All clients’ and sample of staff files reviewed to have all the required documents. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 06/06/24. Forms requested: Lic 308, Lic 500, Lic 610E, current liability insurance and current Administrator certificate.
