On 07/09/2026, Licensing Program Analyst (LPA) M Vega arrived at the facility unannounced to conduct Required Annual Inspection. LPA was greeted by receptionist and stated the purpose of the visit. LPA met with Administrator (AD) - Jennifer Marty. LPA conducted tour of facility with AD. Residents were observed at the facility after lunch and in common areas. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards observed. Fire extinguishers were observed with a service date of 11/13/2025. Dining area and Kitchen were toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in freezer, refrigerator, and pantry. Food is delivered twice a week by staff. Refrigerator temperature was maintained at 30 degree F. and freezer was maintained at 0 degree F. LPA toured a sample of resident bedrooms. Residents' rooms were toured and observed with adequately furnished with bed, dresser, and adequate lighting. Hot water temperature tested at 106.3 degrees F. LPA observed securely fastened grab bars and non-skid mat in all shower areas. Medications were stored in kitchen cabinet. Medications records were reviewed. First Aid Kit was stored in Kitchen cabinet with medications. LPA toured laundry room door was locked and accessible to staff only. LPA observed chemicals were stored and inaccessible to residents in care. Sample of records was observed from clients and staff to have required documentation. Type of visit: Prelicensing

On 06/24/2025 Licensing Program Analyst (LPA) M Vega arrived to the facility for an announced visit to conduct a pre-licensing inspection. LPA met with Administrator - Jennifer Marty at the time of inspection. Facility currently has no residents with a maximum capacity of 6. Facility is a single level residential facility with 5 bedrooms and 2 bathrooms. LPA toured the facility inside and out. LPA observed all fire cleared exits to be clear and free from obstruction. LPA observed fire extinguishers to be last serviced 03/2025 and are in good standing, bathroom water temperature read at 105 Degrees F. Locked centralized storage area for medications and First aid kit complete in kitchen. All rooms had proper furniture with bed, linen, lighting, drawers, night stand, and closet storage. All bedrooms have blinds for privacy. Sufficient lighting & furnishings in common area. Facility has a common areas for residents to interact with each other. Physical plant is consistent with the facility sketch/floor plan. Smoke & carbon monoxide detectors tested & determined to be operational. Component III was also conducted and completed. Pre-licensing requirements were met. An exit interview was conducted with Administrator.
